RESUMO

A new strain gauge plethysmography was developed to permit accurate electrical calibration and automatic calculation of limb blood flow from a panel meter. This instrument was used to quantitate maximum venous outflow (MVO) in both limbs of 20 normal volunteers and 387 limbs of 195 patients with clinically suspected leg vein thrombosis. The MVO of normal subjects averaged 45 +/- 18 cc/minute/100 cc of calf tissue (mean +/- 1 SD). In 69 limbs with deep vein thrombosis documented by Doppler ultrasound and/or phlebography, the MVO averaged 17 +/- 13 cc/minute/100 cc, which was significantly less than that of normal limbs (p less than 0.001). Only three of 31 limbs with venous thrombosis above the knee had MVO above 20 cc/minute/100 cc, the lower limit of normal, for a diagnostic sensitivity of 90%. The specificity of a normal MVO in excluding deep vein thrombosis was 81%, with only a 1% rate of false-negative diagnoses above the knee. This automatic strain gauge plethysmograph has the attribute of providing rapid quantitation of limb venous hemodynamics which facilitates noninvasive diagnosis and follow-up assessment of venous disease.

RESUMO

The device described here continuously mixes two solutions in any required ratio and has been used at flow rates of 6 ml/min in tissue perfusion experiments. Its low-cost construction consists of a three-way miniature solenoid valve driven by a simple electronic circuit. Mixing is stable, reproducible and accurate to +/- 1% of dial setting.

RESUMO

Pulsed ultrasonic images of the carotid bifurcation in 82 vessels of 43 patients were compared independently with contrast arteriograms for stenosis (by percentage quartiles) or occlusion of the internal carotid artery. All 14 occluded vessels were identified correctly by ultrasound but were visualized on repeat examination. Estimation of percentage stenosis on ultrasonic images agreed with the quartile determination by contrast arteriography in 35 of 68 (51 percent) vessels and was within one quartile of correct interpretation in 48 of 68 (71 percent). The interpretative error of grading stenosis of ultrasonic images was due to vascular wall calcification which inhibited ultrasound transmission. This limitation was overcome by sound spectral (sonographic) analysis of distal internal carotid flow velocity which allowed estimation of stenosis within one quartile of that determined by contrast arteriography in 46 of 47 (98 percent) vessels.

RESUMO

The pressure-strain elastic modulus and compliance of 20 reversed saphenous femoropopliteal vein grafts have been measured by a noninvasive, ultrasonic method. The grafts had been in place for an average of 33.6 months, 11 days to 128 months. In this study no correlation was demonstrated between wall stiffness and duration of implantation. It is concluded that the elastic properties of vein grafts may remain stable for considerable periods of time.

The measurement of blood velocity fields, volume flow, and arterial wall motion in the descending thoracic aorta provides essential hemodynamic information for both research and clinical diagnosis. The close proximity of the esophagus to the aorta in the dog makes it possible to obtain such data nonsurgically using an ultrasonic esophageal probe; however, the accuracy of such a probe is limited if the angle between the sound beam and the flow axis, known as the Doppler angle, is not precisely known. By use of a pulsed Doppler velocity meter (PUDVM) and a triangulation procedure, accurate empirical measurement of the Doppler angle has been obtained, allowing quantification of blood velocity scans across the aorta. Volume flow is obtained by integration of blood velocity profiles and arterial wall motion is measured with an ultrasonic echo tracking device. Accuracy of the probe was substantiated by comparison with ultrasonic and electromagnetic implanted flow cuff measurements. Use of the probe in measurement of blood velocity, volume flow and arterial wall motion at various locations along the 8- and 10-cm length of the descending thoracic aorta in adult beagle dogs is detailed. The simplicity, accuracy, and nontraumatic aspect of the technique should allow increasing use of such a probe in numerous research and clinical applications.
