OBJECTIVE: We aim to evaluate the indication and use of genomic signatures in breast cancer patients and outcomes who in patients undergoing adjuvant chemotherapy or not. METHODS: This is a retrospective study of breast cancer patients managed in a private oncology clinic in Teresina, from November 2014 to February 2021. All patients with an indication of genomic signature were included. Clinical and pathological variables, use of genomic signatures, treatment and follow-up were obtained. The nomogram to predict Oncotype DX results (University of Tennessee Medical Center) was also calculated. Clinical risk calculation was based on MINDACT, using the modified version of Adjuvant Online. The genetic signatures performed were: the Oncotype, MammaPrint and EndoPredict. RESULTS: Fifty (50) female patients were included in the study. The mean age of the participants was 57.1 years. Among the patients receiving a genomic signature (26-52.0%), there was a change in treatment in 8 (30.7%) cases. Chemotherapy was indicated in four patients, It was contraindicated in another four patients. Treatment changed in 30.7% of the tested patients. Chemotherapy was indicated for those who would not receive it before. It was contraindicated in patients who would previously undergo chemotherapy.

BACKGROUND: Studies have questioned the necessity of restoring cavitated carious lesion on primary teeth, once the control of biofilm is the most important factor to arrest these lesions. This randomized clinical trial aimed to compare the survival of teeth treated with a non-restorative cavity control (NRCC) compared to resin composite restorations (RCR) on proximal carious lesion in anterior primary teeth, as well as the impact of these treatments on patient-centered outcomes. METHODS: A randomized clinical trial with two parallels arms (1:1) will be conducted. Children between 3 and 6 years old will be selected from the Center of Clinic Research of Pediatric Dentistry of Ibirapuera University (UNIB), a dental trailer (FOUSP) located on Educational Complex Professor Carlos Osmarinho de Lima, the Pediatric Dentistry Clinic of Santa Cecília University and from the Pediatric Dentistry Clinic of University Center UNINOVAFAPI. One hundred and forty-eight teeth will be randomly distributed in two experimental groups: (1) Selective removal of carious tissue and RCR; or (2) NRCC through cavity enlargement using a metallic sandpaper. The primary outcome will be tooth survival after 6, 12, 18 and 24 months. The duration and the cost of dental treatments will be considered for the estimation of the cost-effectiveness of the evaluated treatments. The discomfort reported by the participants will be measured after each treatment using the FIS scale. The participants' satisfaction and perception of the parents/legal guardians will be evaluated through questionnaires. For the primary outcome, Kaplan-Meier's survival and Long-Rank test will be used for comparison between the two groups. All the variables will be modeled by Cox regression with shared fragility. Significance will be considered at 5%. DISCUSSION: The NRCC could be an option to manage carious lesions on proximal surfaces of primary teeth, and the approach could be well accepted by the children and parents/legal guardians. Staphylococcus spp. vem ganhando destaque em infecções na corrente sanguínea (ICS), apresentando alta prevalência, multirresistência e considerável poder de letalidade. O presente estudo teve como objetivo analisar a prevalência e traçar o perfil de sensibilidade das espécies de Staphylococcus spp. isoladas de amostras de hemoculturas positivas obtidas de um hospital de atenção terciária da rede pública do Ceará, no período de janeiro de 2015 a dezembro de 2018. Dos 3292 exames de hemocultura realizados, apenas 15,88% tiveram resultado positivo, dos quais 24,85% eram cocos Gram positivos. S. aureus representou 1,53% das hemoculturas positivas com 50% das cepas resistentes à oxacilina. Os isolados de Staphylococcus sp. coagulase negativo obtiveram prevalência de 10,89%, representados por: S. epidermidis (n=23), S. haemolyticus (n=17), S. hominis (n=13), S. saprophyticus (n=2) e S. warneri (n=2). Verificou-se multirresistência em diversos isolados analisados, apresentando variações intra e interespécies. Portanto, nossos achados melhoram o entendimento da epidemiologia das ICS causadas por Staphylococcus spp., na instituição de estudo, bem como seu perfil de sensibilidade. A identificação precoce do agente infeccioso auxilia na escolha adequada do tratamento, aumentando as chances de cura e reduzindo o tempo de internação do paciente. (AU)


Staphylococcus spp. has been highlighted among bloodstream infections (BI), presenting high prevalence, multidrug resistance and considerable lethality. The present study aimed to analyze the prevalence and the susceptibility profile of Staphylococcus spp. isolated from positive blood cultures from a tertiary care public hospital of Ceará, from January 2015 to December 2018. Of the 3292 blood cultures performed during this period, only 15.88% were positive, of which 24.85% were Gram positive cocci. S. aureus represented 1.53% of positive blood cultures, of which 50% were oxacilin resistant. Isolates of coagulase-negative Staphylococcus spp. showed a prevalence of 10.89%, represented by: S. epidermidis (n = 23), S. haemolyticus (n = 17), S. hominis (n = 13), S. saprophyticus (n = 2) and S. warneri (n = 2). Multiresistance occurrence was verified in several of the analyzed isolates, presenting intra and inters species variations. Therefore, our findings improve the understanding of the epidemiology of BI caused by Staphylococcus spp. in the studied institution, as well as its susceptibility profile. Early identification of the infectious agent might aid in the appropriate choice of treatment, increasing the chance of cure and reducing the patient length stay in hospital. (AU)

De forma general, las microalgas son fuente de alimento indispensable durante las primeras fases de desarrollo de los camarones marinos. Algunas microalgas, como la Arthrospira platensis, son ricas en substancias antioxidantes, y tienen la capacidad de retirar compuestos tóxicos del agua en los sistemas de producción acuícola. De este modo, constituimos un experimento con dos tratamientos triplicados, donde se evaluó la aplicación de biomasa filtrada de microalga A. platensis como suplemento alimenticio para camarón blanco del Pacífico, Litopenaeus vannamei. Se utilizó un sistema de cultivo estático de seis unidades, con volumen útil de 250 litros de agua marina (30 ppt), donde se sembraron 300 camarones m-2 con peso inicial de 0,7±0,2 g. Al final del experimento (42 días) se evaluaron parámetros inmunológicos y de desempeño zootécnico. Los camarones alimentados con A. platensis como como suplemento fueron signicativamente mayores que los camarones del tratamiento control y no presentaron diferencias en la supervivencia. Igualmente, se obtuvieron mejores resultados en los parámetros inmunológicos del tratamiento con inclusión de microalga. De esta forma, al suministrar la microalga como alimento suplementario al camarón L. vannamei, crecimiento y sistema inmunológico se vieron favorecidos.(AU)


Microalgae is an important food source during the early stages of marine shrimps. Some microalgae such as Arthrospira platensis, are rich in antioxidant substances and also have the capacity to remove toxics compounds from the water in aquaculture systems. The trial consisted in two treatements by triplicate, where the inclusion of filtered microalgae A. platensis as feed supplementation during production of juvenile Litopenaeus vannamei, was assessed. The experimental desing was based on a static system with six 250-L tanks with marine wáter (30 PSU), stocked 105 shirmps (300 shrimp m-2) with an average wheight of 0,7 ±0,2 g. At the end of the fed trial (42 days), the zootechnical and immunological parameters of shrimps were evaluated. Significatives differences among the treatments were found for both parameters. Shrimps that received supplementation of microalgae A. platensis showed higher growth and better response of the immune system. Thus, these results allow to conclude that the supplementation of filtered microalgae A. platensis improve growth and immunological parameters of white shrimp L. vannamei.(AU)

O Platynosomum spp. é um trematódeo que habita os ductos biliares e a vesícula biliar de gatos, encontrado mundialmente em áreas com climas tropicais e subtropicais. Os gatos que vivem nestas áreas são infectados ao ingerirem vertebrados inferiores, como lagartixas e sapos. Muitos casos de platinossomose são assintomáticos, todavia, gatos com infecções graves podem exibir icterícia generalizada, vômito, diarreia mucoide, anorexia, emagrecimento, depressão e morte. Os sinais clínicos não são específicos e podem passar despercebidos, dificultando assim seu controle e tratamento. O objetivo do presente trabalho é relatar o primeiro caso confirmado desta enfermidade em gato doméstico ocorrido no município de Fortaleza estado do Ceará. Portanto, pode-se concluir que, a platinossomose deve ser investigada nos felinos da região que apresentem a sintomatologia característica.(AU)


Platynosomum spp. is a trematode that inhabits bile ducts and gall bladder of cats, and can be found in tropical and subtropical areas of the world. The infection occurs when the cat living in these areas ingest lower vertebrates, such as lizards and frogs. Although many cats are asymptomatic, cats with severe infection may show generalized jaundice, vomiting, mucoid diarrhea, anorexia, weight loss, depression and death. The clinical signs are nonspecific and the disease may often continue unnoticed, thus hindering its control and treatment. Accordingly, this study aimed to report the first confirmed case of Platynosomum ssp. infection in a domestic cat occurred in the city of Fortaleza, in the state of Ceará, Brazil.(AU)


El Platynosomum spp. es un trematodo que habita en los ductos biliares y en la vesícula biliar de gatos, que se encuentran en todo el mundo en áreas con climas tropicales y subtropicales. Los gatos que viven en estas áreas se infectan al ingerir los vertebrados inferiores, como los lagartos y las ranas. Muchos casos de platinosomosis son asintomáticos, sin embargo, gatos con infecciones graves pueden presentar ictericia generalizada, vómitos, diarrea mucoide, anorexia, pérdida de peso, depresión y la muerte. Los signos clínicos no son específicos y pueden pasar desapercibidos, dificultando así su control y tratamiento. El objetivo de esta investigación ha sido reportar el primer caso confirmado de esta enfermedad en gato doméstico ocurrido en el municipio de Fortaleza, Ceará. Por lo tanto, se puede concluir que platinosomosis debe ser investigado en gatos de la región, que presenten señales característicos.(AU)
