RESUMO

Normal bladder function is achieved by most in childhood. Stroke affects 15 million people worldwide every year, with incontinence affecting over half these individuals in the initial post-stroke phase. Statistically it has been shown that incontinence can increase the morbidity risk of stroke victims. The social taboo surrounding continence issues has been a challenge for many years with individuals experiencing shame and isolation. This article looks at the normal physiology associated with continence and stroke. It suggests possible treatment options during the post-stroke rehabilitation phase to encourage improved patient experience and professionals' confidence and knowledge base when treating this group of patients.

RESUMO

Intermittent catheterisation is considered the gold standard for bladder drainage. This article describes intermittent self-catheterisation (ISC) and which patients are likely to benefit from it. Before deciding on a patient's suitability for ISC, health professionals should carry out a full assessment and ensure the patient and/or carers are taught the skills necessary to perform the procedure. ISC enables patients to be more independent because they can choose how often and where to undertake it, which encourages adherence. The range of products is vast; the nurse can guide patients to select the catheter most suited to their individual needs.

RESUMO

Urinary catheters are one of the most common invasive devices used in the NHS. The risks are well-documented and the practice of catheterisation of the bladder has been around for years. Traditionally catheter fixation devices have not been widely used, despite many best practice guidelines advising on their use as a standard within catheter care and maintenance. This article considers some of the evidence around urinary catheterisation and the benefits and limitations of fixation devices. There will be a product focus on the Optimum Medical Ugo Fix Gentle catheter fixation device, with case histories to demonstrate patient satisfaction with the product.

Incontinence-associated dermatitis (IAD) is a common skin disorder experienced by people who suffer from faecal and/or urinary incontinence. It is painful and in some cases accompanied by significant secondary infections. The prevalence is higher in those people receiving long-term care at home. IAD is often misdiagnosed and confused with pressure ulcers. Care providers now crossover health and social care boundaries and education on continence-related issues is often not prioritised. This article looks at normal skin physiology and the aetiology of IAD versus pressure ulcers. It suggests prevention and management strategies in relation to IAD; in particular, the use of barrier creams. A selection of case studies evaluating the efficacy of using Cutimed PROTECT cream in cases of IAD are presented.

Incontinence-associated dermatitis (IAD) is a common skin disorder affecting patients with urinary and/or faecal incontinence. Maintaining the skin's integrity is a challenge, particularly in the elderly and individuals with medical or surgical comorbidities. It is widely reported that the issue is complex and recognition is inconsistent, with symptoms often being confused with those of pressure ulcers. This article explores the causes of IAD, looking at the structure of healthy skin and the pathology that occurs during skin breakdown. It identifies risk factors and prevention and management strategies, including the use of barrier creams. The article then presents the results of a large product evaluation that took place with Cavilon Durable Barrier Cream (3M). The barrier cream was shown to be more effective in treating and managing patients with IAD than the previous product that patients had been using. A case study is also included to demonstrate the efficacy of the newest version of Cavilon Durable Barrier Cream.
