RESUMO

AIMS: To evaluate the long-term outcomes of patients with chordoma and low-grade chondrosarcoma after surgery and high-dose radiotherapy. MATERIALS AND METHODS: High-dose photon radiotherapy was delivered to 28 patients at the Neuro-oncology Unit at Addenbrooke's Hospital (Cambridge, UK) between 1996 and 2016. Twenty-four patients were treated with curative intent, 17 with chordoma, seven with low-grade chondrosarcoma, with a median dose of 65 Gy (range 65-70 Gy). Local control and survival rates were calculated using the Kaplan-Meier method. RESULTS: The median follow-up was 83 months (range 7-205 months). The 5 year disease-specific survival for chordoma patients treated with radical intent was 85%; the local control rate was 74%. The 5 year disease-specific survival for chondrosarcoma patients treated with radical intent was 100%; the local control rate was 83%. The mean planning target volume (PTV) was 274.6 ml (median 124.7 ml). A PTV of 110 ml or less was a good predictor of local control, with 100% sensitivity and 63% specificity. For patients treated with radical intent, this threshold of 110 ml or less for the PTV revealed a statistically significant difference when comparing local control with disease recurrence (P = 0.019, Fisher's exact test). Our data also suggest that the probability of disease control may be partly related to both target volume and radiotherapy dose. CONCLUSION: Our results show that refined high-dose photon radiotherapy, following tumour resection by a specialist surgical team, is effective in the long-term control of chordoma and low-grade chondrosarcoma, even in the presence of metal reconstruction. The results presented here will provide a useful source for comparison between high-dose photon therapy and proton beam therapy in a UK setting, in order to establish best practice for the management of chordoma and low-grade chondrosarcoma.

OBJECTIVE: Optimisation of imaging protocols is essential to maximise the use of image-guided radiotherapy. This article evaluates the time for daily online imaging with TomoTherapy® (Accuray®, Sunnyvale, CA), separating mechanical scan acquisition from radiographer-led image matching, to estimate the time required for a clinical research study (VoxTox). METHODS: Over 5 years, 18 533 treatments were recorded for 3 tumour sites of interest (prostate, head and neck and central nervous system). Data were collected for scan length, number of CT slices, slice thickness, scan acquisition time and image matching time. RESULTS: The proportion of coarse thickness scans increased over time, with a move of making coarse scans as the default. There was a strong correlation between scan time and scan length. Scan acquisition requires 40 s of processing time. For coarse scans, each additional centimetre requires 8 s for acquisition. Image matching takes approximately 1.5 times as long, so each additional centimetre needs 20 s extra in total. Modest changes to the imaging protocol have minimal impact over the course of the day. CONCLUSION: This work quantified the effect of changes to clinical protocols required for research. The results have been found to be reassuring in the busy National Institutes of Health department. ADVANCES IN KNOWLEDGE: This novel method of data collection and analysis provides evidence of the minimal impact of research on clinical turnover. Whilst the data relate specifically to TomoTherapy, some aspects may apply to other platforms in the future.

AIMS: Image-guided radiotherapy (IGRT) and intensity-modulated radiotherapy (IMRT) represent two important technical developments that will probably improve patient outcome. Helical tomotherapy, provided by the TomoTherapy HiArt system, provides an elegant integrated solution providing both technologies, although others are available. Here we report our experience of clinical implementation of daily online IGRT and IMRT using helical tomotherapy. MATERIALS AND METHODS: Methods were needed to select patients who would probably benefit. Machine-specific commissioning, a quality assurance programme and patient-specific delivery quality assurance were also needed. The planning target volume dose was prescribed as the median dose, with the added criterion that the 95% isodose should cover 99% of the target volume. Although back-up plans, for delivery on conventional linear accelerators, were initially prepared, this practice was abandoned because they were used very rarely. RESULTS: In the first 12 months, 114 patients were accepted for treatment, and 3343 fractions delivered. New starts averaged 2.6 per week, with an average of 17.5 fractions treated per day, and the total number capped at 22. This has subsequently been raised to 24. Of the first 100 patients, 96 were treated with radical intent. Five were considered to have been untreatable on our standard equipment. IGRT is radiographer led and all patients were imaged daily, with positional correction made before treatment, using an action level of 1mm. A formal training programme was developed and implemented before installation. The in-room time fell significantly during the year, reflecting increasing experience and a software upgrade. More recently, after a couch upgrade in April 2009, the mean in-room time fell to 18.6 min. CONCLUSIONS: Successful implementation of tomotherapy was the result of careful planning and effective teamwork. Treatment, including daily image guidance, positional correction and intensity-modulated delivery, is fast and efficient, and can be integrated into routine service. This should encourage the adoption of these technologies.

Quantitative assessment of target volume contouring in radiotherapy treatment planning is an important aspect of quality assessment and educational exercises. The Conformity Index (CI) is a volume-based statistic frequently used for this purpose. Although the CI is relatively simple to understand and can be calculated using most treatment planning systems, it does not provide any information on the differences in shape between the two volumes. We present a new morphometric (shape-based) statistic known as the "mean distance to conformity" (MDC). For a specific volume that is being evaluated against a reference volume, the MDC represents the average distance that all outlying points in the volume must be moved in order to achieve perfect conformity with the reference volume. The MDC comprises a component related to under-contouring (where the evaluation volume is smaller than the reference volume) and a component related to over-contouring (where the evaluation extends beyond the reference volume). Furthermore, voxel-by-voxel information on conformity errors can also be displayed using a volume-error histogram. Calculation of MDC statistics is achieved using a three-dimensional grid search algorithm. By using a range of scenarios comprising both theoretical and actual clinical volumes, we demonstrate the increased utility of the MDC for the detection of contouring errors.

A common clinical problem in IMRT, especially when treating head and neck cases, is that the clinical target volume (CTV) stops short of the skin surface, whilst the margin for geometric uncertainties may take the planning target volume (PTV) to the skin surface or beyond. In these cases, optimization leads to over-dosing of the skin, unless the planner resorts to procedural tricks to avoid this, such as the use of pretend bolus or reduction of the PTV followed by adding 'flash' after optimization. This paper describes a method of avoiding the need for these tricks by using a multiple-isocentre CTV-based objective function. This enables plans to be produced that will give good coverage of the CTV for all the geometrical uncertainties that would have been covered by the PTV without causing the problem of over-dosing the skin. Eight isocentre shifts, equally distributed on the surface of a sphere with a radius equal to the CTV-PTV margin, are shown to be adequate for the optimization process. The resulting fluence maps are much simpler than those resulting from PTV optimization and will therefore be simpler to deliver. The method also permits better sparing of organs at risk such as the spinal cord.

We present a method to incorporate geometrical uncertainties into dose-volume histogram evaluation: the dose-volume population histogram (DVPH). For each dose-volume point, the probability of the plan DVH meeting the constraint is calculated. The use of DVPH for the target shows that the minimum dose to the PTV might not be representative of the minimum dose to the CTV considering geometrical uncertainties when the PTV extends into the build-up region. For OARs, the DVH obtained from DVPH with 90% confidence level is found to be different to the planning organ at risk volume (PRV) DVH recommended by ICRU 62, especially for parallel organs.

AIMS: To study the feasibility of using implanted gold seeds in combination with a commercial software system for daily localisation of the prostate gland during conformal radiotherapy, and to assess the effect this may have on departmental workload. MATERIALS AND METHODS: Six patients had three gold radio-opaque seeds implanted into the prostate gland before starting a course of radiotherapy. The seeds were identified on daily portal images and an automated online system provided immediate vector analysis of discrepancies between the planned and actual daily position of the intraprostatic seeds. In total, 138 interfractional displacements were analysed. The workload impact for the department was assessed using the basic treatment equivalence model, by comparing measurements of daily treatment session durations with a control group of patients receiving standard conformal radiotherapy, matched for treatment complexity. RESULTS: No acute complications of seed insertion were observed. A number of developmental issues required solutions to be identified before clinical implementation was possible. The standard deviations of the set-up and organ motion systematic errors in the left-right, superior-inferior and anterior-posterior directions were 2.4, 3.0 and 2.5 mm, respectively. The standard deviations of the set-up and organ motion random errors calculated were 2.5, 2.9 and 3.7 mm. The mean treatment session duration with this daily prostate localisation system was increased by 3 min compared with matched controls using standard imaging practice. If all radical prostate patients in our department were to receive image-guided radiotherapy in this way, this would increase machine workload time by 2.2 h/day. CONCLUSIONS: The implementation of this image-guided system is feasible. No additional linear accelerator modification is required and standard imaging devices can be used. It would be a useful addition to any department's image-guided radiotherapy developmental strategy.
