[Treatment of hernia ventralis permagna]. / Resení obrovských ventrálních hernií.

The authors present case report about two patients with hernia ventralis permagna. Both patients were treated urgently for strangulation in hernial bag. The elective surgery was delayed for various reasons and, at the end, they were operated acutely using the polypropylene net. The authors prefer the more active approach to the indication of elective surgery.

[Conversion of an external fixation to that with an intramedullary pin in cases of complicated diaphyseal fractures]. / Konverze zevní fixace na nitrodrenový hreb u komplikovaných diafyzárních zlomenin.

In this study the authors present a method of conversion of external fixations to closed intramedullary ones in the treatment of complicated diaphyseal fractures of the calf and femur. According to the conditions, the fixation is secured by a pin. The above mentioned procedure is advantageous especially in cases of patients with open fractures, in patients with polytraumas as well as in patients with a threatening or with a present compartment syndrome. Treatment of the above fractures is not always simple and represents a serious problem in everyday traumatological practice. Treatment of fractures using the above procedure has very good results with a minimum of complications. Although, nowadays, the skeletal defect can be solved and therefore the bearing capacity of the extremity be renewed, damages to the muscles are often irreparable and cause a permanent invalidity.

[Surgical treatment of peritoneal adhesions--personal experience]. / Chirurgická lécba peritoneálních srustu--nase zkusenosti.

Peritoneal adhesions develop virtually in all abdominal operations. In about 30% they cause subsequent complications, especially disorders of intestinal passage, spasms and female infertility. The authors describe their first experience with implantation of resorptive foil Seprafilm and recommend general scheme for the approach in prevention and subsequent solution of the problem of adhesion-lysis.

Unusual Bochdalek hernia in puerperium.

The authors present an unusual case of a true left-sided posterolateral (Bochdalek) hernia containing stomach, transverse colon and spleen within a hernial sac in a 25-year-old female patient. They presume this voluminous herniation took place due to a rupture of congenitally preformed hernial ostium on account of increased intraabdominal pressure during repeated pregnancy. They describe an effective surgical transabdominal laparoscopic treatment.

[Indication criteria for surgical treatment of thoracic outlet syndrome]. / Nase indikacní kritéria chirurgického lécení thoracic outlet syndromu.

When formulating indications for surgery of the thoracic outlet syndrome (TOS) the authors use as a basis their own experience with treatment of 42 patients and data from the literature. The basis is the fact that to the great majority of patients at the time after establishment of the diagnosis comprehensive and systematic rehabilitation care was not available either because lack of time or due to the absence of a department within their reach which could ensure adequate care. The authors discuss the reasons why they prefer after accurate establishment of the diagnosis a primary surgical approach and elimination of the cause of further mechanical damage of the neurovascular bundle with necessary and obvious continuation with long-term intensive rehabilitation. This is a reverse procedure than that recommended by some others concerned with this problem.

[Uncommon case of a giant diaphragmatic hernia in a pregnant patient]. / Neobvyklý prípad obrovské bránicní kýly u tehotné pacientky.

The authors submit an uncommon case of a true leftsided posterolateral (Bochdalek) hernia with the stomach, transverse colon and spleen in the hernial sac in a 25-year-old patient. They assume that the large hernation developed as a result of rupture of an already congenitally preformed hernial opening as a result of intraabdominally increased pressure during repeated pregnancies. The authors describe effective surgical solution of the case by laparoscopy from a transabdominal approach.

[The compartment syndrome--still a threat in traumatology]. / Kompartment syndrom--stálá hrozba v traumatologii.

Paper presents current opinions on the definition, causes, classification, diagnosis and treatment of the compartment syndrome (CS). The development of CS after the long-bone fractures of the lower extremity and also after the limb operations or trauma without fracture is discussed. The importance of early and correct diagnostics and treatment is also revealed. Authors present several case reports and they stress the importance of early fasciotomy when necessary.

In the report a new method of tension-free repair of inguinal hernia using a prolene-mash PHS is described.

[Differential diagnosis of pain in the right lower abdominal quadrant--case reports]. / Diferenciální diagnostika bolestí v pravém dolním brisním kvadrantu--kazuistiky.

In the submitted case-records the authors present interesting peroperative findings--a coprolith obturing the lumen of the appendix, the torsion of an ovarian cyst and torsion of the omentum during diagnosis of acute appendicitis. The authors discuss the reliability of preoperative sonography for the diagnosis of pathological conditions in the appendical region which despite its falsely negative results are considered by the authors a useful method in the differential diagnosis of pain in the right iliac fossa.
