RESUMO

Rheumatoid arthritis (RA) patients have substantial functional disability which poses a significant impact on the quality of life (QoL). So, this study aims to assess the QoL of RA patients by the Short Form Health Survey-36 (SF-36) questionnaire. Total sixty-two (62) rheumatoid arthritis patients were included in this cross-sectional observational study and were performed in the Department of Physical Medicine and Rehabilitation, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from August 2018 to September 2019 according to selection criteria. A Bengali validated version of Medical Outcomes Study-36: Item Short Form Health Survey (SF-36) questionnaire was used to assess QoL. Among the sixty-two (62) patients, mean age was 44.18±12.14 years with female preponderance (77.4%). About one-fifth of the patients (19.4%, n=12) were illiterate and the majority (71.0%, n=44) were housewives. Amongst all, 53.2% (n=33) had monthly family income >20,000 taka. Forty percent (n=25) had a disease duration of 1 to 5 years. According to Disease Activity Score Sheet 28 (DAS-28), 36(58.1%) patients had moderate disease activity and 26(41.9%) patients had high disease activity. Among the 8 domains, the mental health domain had the highest score (42.77±13.78) and the role physical domain had the lowest (35.89±15.43). Male, literate patients with higher income had relatively better QoL. Patients with prolonged disease, high disease activity and irregular taking of Disease Modifying Anti-Rheumatic Drugs (DMARDs) had poor quality of life. A significant negative correlation was present between SF-36 scores and the DAS-28 scores (r = -0.803, p<0.001). Rheumatoid arthritis had a relatively poor quality of life regarding both physical and mental health components, but the physical health component was predominantly affected.

AIM: The aim of this in vitro study was to evaluate the effect of accelerated artificial aging (AAA) and fatigue on the biaxial flexural strength (BFS) of three types of monolithic high-translucency zirconia restorations compared with conventional low-translucency zirconia restorations. MATERIALS AND METHODS: Four groups of 20 disc-shaped specimens (10 × 1.2 mm) were made from the following computer-aided design and computer-aided manufacturing (CAD/CAM) zirconia blocks: Low-translucency zirconia (Ceramill ZI-LT) as a control, and three brands of high-translucent zirconia (Lava Plus, Ceramill Zolid White, and Copran Monolithic HT). Ten discs from each group were subjected to the BFS test using the universal testing machine. The other 10 discs from each group were subjected to AAA (thermocycling, 3,500 cycles) and fatigue (250,000 cycles) before the fracture test. The definitive fracture load was recorded, and the BFS was calculated in accordance with International Organization for Standardization (ISO) 6872. The data were analyzed with one-way analysis of variance (ANOVA), Scheffe post hoc, and Mann-Whitney U test. Data analyses were evaluated at a significance level of p < 0.05. RESULTS: Significant differences were detected in the BFS among the four groups before AAA and fatigue. The mean BFS was highest with Ceramill ZI (935.3 ± 47.1 MPa), and least in Ceramill Zolid White (685.7 ± 32.6 MPa). After AAA and fatigue, significant differences were reported where the mean of BFS was highest with Copran Zr-i Monolithic HT (777.5 ± 21.2 MPa), and least in Ceramill Zolid White (576.0 ± 36.3 MPa). Furthermore, Mann-Whitney U test showed that AAA and fatigue significantly affect the BFS of each material individually. CONCLUSION: The AAA and fatigue significantly affected the BFS of the monolithic high-translucency zirconia restorations. CLINICAL SIGNIFICANCE: Although monolithic high-translucency zirconia had significantly lower BFS than conventional zirconia tested in this study, they still have sufficient strength for clinical use.

Impurity Fe3+ ion electron spin resonance (ESR) spectroscopy using multiple dielectric modes in a SrTiO3 dielectric resonator has been performed with a tunable DC magnetic field of up to 1.6 T. The Ti[Formula: see text] ion is substituted by Fe3+ ion forming FeO6 octahedral complex with an iron-oxygen-vacancy (Fe[Formula: see text]). In such a metal-ligand complex, a giant g-factor of [Formula: see text] was observed in the ferroelectric phase at 20 mK. The change of Fe3+ ion center-symmetry in the FeO6 complex as a soft-mode characteristics of ferroelectric phase transition and the influences of iron-oxygen-vacancy (Fe[Formula: see text]), are interactively sensitive to asymmetry in the octahedral rotational parameter Φ in SrTiO3.

The impurity paramagnetic ion, [Formula: see text] substitutes Al in the [Formula: see text] single crystal lattice, this results in a [Formula: see text] elongated octahedron, and the resulting measured g-factors satisfy four-fold axes variation condition. The aggregate frequency width of the electron spin resonance with the required minimum level of impurity concentration has been evaluated in this single crystal [Formula: see text] at 20 millikelvin. Measured parallel hyperfine constants, [Formula: see text], were determined to be [Formula: see text] and [Formula: see text] at [Formula: see text] for the nuclear magnetic quantum number [Formula: see text], and [Formula: see text] respectively. The anisotropy of the hyperfine structure reveals the characteristics of the static Jahn-Teller effect. The second-order-anisotropy term, [Formula: see text], is significant and cannot be disregarded, with the local strain dominating over the observed Zeeman-anisotropy-energy difference. The Bohr electron magneton, [Formula: see text], (within [Formula: see text] so-called experimental error) has been found using the measured spin-Hamiltonian parameters. Measured nuclear dipolar hyperfine structure parameter [Formula: see text] shows that the mean inverse third power of the electron distance from the nucleus is [Formula: see text] a.u. for [Formula: see text] ion in the substituted [Formula: see text] ion site assuming nuclear electric quadruple moment [Formula: see text] barn.

A cylindrical single crystal SrLaAlO4 Whispering Gallery mode dielectric resonator was cooled to millikelvin temperature using a dilution refrigerator. By controlling a DC-magnetic field, impurity ions' spins were coupled to a variety of modes allowing the measurement of hybrid spin-photon systems. This Electron Spin Resonance mapping technique allowed us to detect Cu2+,Fe3+ and Mn4+ impurity ions (at the level of parts per million (ppm) to parts per billion (ppb)), verified by the measurement of the spin parameters along with their site symmetry. Whispering Gallery modes exhibited Q-factors ⩾105 at a temperature less than 20mK, allowing sensitive spectroscopy with high precision. Measured hyperfine line constants of the Cu2+ ion shows different parallel g-factors, g‖Cu, of 2.526,2.375,2.246 and 2.142. The spin-orbit coupling constant of the Cu2+ ion was determined to be λ≃-635cm-1. The low-spin state Fe3+ ion's measured parallel g-factor, g‖Fe, of 2.028 reveals tetragonal anisotropy. The Mn4+ ion is identified in the lattice, producing hyperfine structure with high-valued g-factors,g‖Mn, of 7.789,7.745,7.688,7.613,7.5304 and 7.446. The hyperfine structures of the Cu2+ and Mn4+ ions show broadening of about 79G between 9.072GHz and 10.631GHz, and 24.5G broadening between 9.072GHz and 14.871GHz, respectively.

AIM: The aim of this study was to evaluate the effect of surface treatments on shear bond strength (SBS) of Turkom-Cera (Turkom-Ceramic (M) Sdn. Bhd., Puchong, Malaysia) all-ceramic material cemented with resin cement Panavia-F (Kuraray Medical Inc., Okayama, Japan). MATERIALS AND METHODS: Forty Turkom-Cera ceramic disks (10 mm × 3 mm) were prepared and randomly divided into four groups. The disks were wet ground to 1000-grit and subjected to four surface treatments: (1) No treatment (Control), (2) sandblasting, (3) silane application, and (4) sandblasting + silane. The four groups of 10 specimens each were bonded with Panavia-F resin cement according to manufacturer's recommendations. The SBS was determined using the universal testing machine (Instron) at 0.5 mm/min crosshead speed. Failure modes were recorded and a qualitative micromorphologic examination of different surface treatments was performed. The data were analyzed using the one-way analysis of variance (ANOVA) and Tukey honestly significant difference (HSD) tests. RESULTS: The SBS of the control, sandblasting, silane, and sandblasting + silane groups were: 10.8 ± 1.5, 16.4 ± 3.4, 16.2 ± 2.5, and 19.1 ± 2.4 MPa respectively. According to the Tukey HSD test, only the mean SBS of the control group was significantly different from the other three groups. There was no significant difference between sandblasting, silane, and sandblasting + silane groups. CONCLUSION: In this study, the three surface treatments used improved the bond strength of resin cement to Turkom-Cera disks. CLINICAL SIGNIFICANCE: The surface treatments used in this study appeared to be suitable methods for the cementation of glass infiltrated all-ceramic restorations.

RESUMO

Design of a rectangular spiral planar inverted-F antenna (PIFA) at 915 MHz for wireless power transmission applications is proposed. The antenna and rectifying circuitry form a rectenna, which can produce dc power from a distant radio frequency energy transmitter. The generated dc power is used to operate a low-power deep brain stimulation pulse generator. The proposed antenna has the dimensions of 10 mm [Formula: see text]12.5 mm [Formula: see text]1.5 mm and resonance frequency of 915 MHz with a measured bandwidth of 15 MHz at return loss of [Formula: see text]. A dielectric substrate of FR-4 of [Formula: see text] and [Formula: see text] with thickness of 1.5 mm is used for both antenna and rectifier circuit simulation and fabrication because of its availability and low cost. An L-section impedance matching circuit is used between the PIFA and voltage doubler rectifier. The impedance matching circuit also works as a low-pass filter for elimination of higher order harmonics. Maximum dc voltage at the rectenna output is 7.5 V in free space and this rectenna can drive a deep brain stimulation pulse generator at a distance of 30 cm from a radio frequency energy transmitter, which transmits power of 26.77 dBm.

The transfusion of whole blood derived platelets (WBDPs) or apheresis platelets (APs) is standard support for cancer patients. However, disputes remain about which type of platelets are ideal in terms of efficacy, cost, and the risk of adverse reactions. This cross sectional study included 141 cancer patients who underwent chemotherapy or hematopoietic progenitor cell transplantation and received platelet transfusions at The University of Texas M.D. Anderson Cancer Center between 2002 and 2003 were retrospectively evaluated. A total of 141 patients who did not differ significantly in terms of age or sex had a reaction to transfusions (WBDPs, n=123; APs, n=18), for a frequency of 0.66% in patients who received WBDPs and 0.45% in patients who received APs, but this difference was not statistically significant (p=0.13). More WBDP-related reactions occurred in patients transfused with older platelets (>2 days old) than in patients transfused with fresh platelets, but the difference compared with AP-associated reactions was not statistically significant. However, the rate of reactions to WBDP may increase if WBDPs are stored for a prolonged time (>2 days). Until evidence becomes available that clearly refutes this; the more fresh platelets as possible may be used.

Deep brain stimulation has emerged as an effective medical procedure that has therapeutic efficacy in a number of neuropsychiatric disorders. Preclinical research involving laboratory animals is being conducted to study the principles, mechanisms, and therapeutic effects of deep brain stimulation. A bottleneck is, however, the lack of deep brain stimulation devices that enable long term brain stimulation in freely moving laboratory animals. Most of the existing devices employ complex circuitry, and are thus bulky. These devices are usually connected to the electrode that is implanted into the animal brain using long fixed wires. In long term behavioral trials, however, laboratory animals often need to continuously receive brain stimulation for days without interruption, which is difficult with existing technology. This paper presents a low power and lightweight portable microdeep brain stimulation device for laboratory animals. Three different configurations of the device are presented as follows: 1) single piece head mountable; 2) single piece back mountable; and 3) two piece back mountable. The device can be easily carried by the animal during the course of a clinical trial, and that it can produce non-stop stimulation current pulses of desired characteristics for over 12 days on a single battery. It employs passive charge balancing to minimize undesirable effects on the target tissue. The results of bench, in-vitro, and in-vivo tests to evaluate the performance of the device are presented.

LaCoste linkage vibration isolators have shown excellent performance for ultra-low frequency vertical vibration isolation. However, such isolators depend on the use of conventional pre-stressed coil springs, which suffer from creep. Here, we show that compressional Euler springs can be configured to create a stable tension unit for use in a LaCoste structure. In a proof of concept experiment, we demonstrate a vertical resonance frequency of 0.15 Hz in an Euler-LaCoste configuration with 200 mm height. The system enables the use of very low creep maraging steel as spring elements to eliminate the creep while minimising spring mass and reducing the effect of parasitic resonances. Larger scale systems with optimized Euler spring boundary conditions should achieve performance suitable for applications on third generation gravitational wave detectors such as the proposed Einstein telescope.

This community-based study examines health care seeking strategies with respect to types of practitioners consulted by disabled persons in rural Bangladesh. A primary health care specialist collected the data through household surveys. The study found that 81% of the disabled people had sought some forms of care from various health practitioners. Unqualified practitioners were found to be strongly involved (96%) in providing health care in this area. Persons with learning difficulties, speech difficulties, fits and strange behavior were more likely to seek treatment from unqualified practitioners. Mean delay and cost of treatment were significantly higher among the qualified practitioners than the unqualified practitioners. Visits to universally free public or government health care facilities were characterized as frustrating, inconvenient, time-consuming and less rewarding for disabilities by 34% of the disabled people. Further examination of the plurality of providers and practitioners in rural Bangladesh is warranted to see how best they can be used or re-trained to respond to the health care needs of disabled persons.

Oral cancer is the second most common cancer in women and the third most common in men in Pakistan. Tobacco is smoked and chewed extensively in Pakistan. Paan is a quid of piper betel leaf that contains areca nut, lime, condiment, sweeteners, and sometimes tobacco, which is also used extensively. We did this study to clarify the independent association of paan and oral cancer. Between July 1996 and March 1998, we recruited biopsy-proven, primary cases of oral squamous-cell carcinoma, from 3 tertiary teaching centers in Karachi, Pakistan, and controls pair-matched for age, gender, hospital and time of occurrence, excluding persons with a past or present history of any malignancy. There were 79 cases and 149 controls. Approximately 68% of the cases were men, 49 years old on average, the youngest being 22 years old and the eldest 80. People with oral submucous fibrosis were 19.1 times more likely to develop oral cancer than those without it, after adjusting for other risk factors. People using paan without tobacco were 9.9 times, those using paan with tobacco 8.4 times, more likely to develop oral cancer as compared with non-users, after adjustment for other covariates. This study identifies an independent effect of paan without tobacco in the causation of oral cancer. Its findings may be of significance in South Asian communities where paan is used, and among health-care providers who treat persons from South Asia.
