RESUMO

OBJECTIVES: To investigate the effects of a structured moderate-intensity resistance exercise program on blood glucose levels and other health-related indicators in patients with GDM. METHODS: A total of 99 patients with GDM in a tertiary class A general hospital were randomly divided into an experimental group and a control group. GDM patients in the control group received routine prenatal care, online education, and a personalized diabetes diet intervention. The experimental group was treated in the same way as the control group with the addition of a moderate intensity resistance exercise program. RESULTS: The blood glucose levels in both groups were lower after the intervention compared with before intervention (P < 0.05). After intervention, the average fasting blood glucose, the 2 h postprandial blood glucose, the utilization rate of insulin, the amount of insulin, gestational weight gain and blood pressure (P < 0.05) in the experimental group were lower than the control group. In addition, there was no statistical significance in the incidence of adverse pregnancy outcomes between the two groups after intervention (P > 0.05). CONCLUSIONS: Moderate intensity resistance exercise was helpful for improvement of blood glucose control and insulin use, gestational weight gain and blood pressure in patients with GDM. In the future, long-term follow-up of both maternal and newborn infants should be performed to assess the long-term effects of exercise intervention on maternal and child health. The impact on the risk of obesity and diabetes may need to be further clarified. INTRODUCTION: To investigate the effects of structured moderate-intensity aerobic exercise on blood glucose, insulin, and pregnancy outcomes in patients with gestational diabetes mellitus (GDM). METHODS: One hundred one patients with GDM were randomly divided into a control group (50 cases) and an experimental group (51 cases) in a class 3 first-level general hospital. GDM patients in the control group received a personalized diabetes diet intervention, online education, and routine prenatal care. The experimental group added 6 weeks of moderate-intensity aerobic exercise in addition to the identical conditions given to the control group. The differences of fasting and 2-h postprandial blood glucose, insulin use, and adverse pregnancy outcomes were evaluated between the experimental and control group after intervention. RESULTS: Outcomes were available from 89 participants. Compared with before intervention, there were statistically significant differences in fasting blood glucose and 2-h blood glucose after three meals in both groups (P < 0.05). There were statistically significant differences in the average fasting blood glucose, the average 2-h postprandial blood glucose, the insulin dosage, and the utilization rate between the experimental and control group after the intervention (P < 0.05). Parameters in the experimental group were all lower than in the control group. Compared with the control group, the incidence of adverse pregnancy outcomes in the experimental group after intervention was not statistically significant (P > 0.05). CONCLUSION: Moderate-intensity aerobic exercise can help improve blood glucose control and insulin use in patients with GDM. In the future, long-term follow-up can be conducted for maternal and neonatal infants to evaluate the impact of exercise intervention on the risk of type 2 diabetes. PROBLEM: There are different conclusions about the effects of resistance training on blood sugar levels and pregnancy outcomes in gestational diabetes mellitus (GDM) patients. BACKGROUND: Resistance training is recommended as an easier and more practical method to exercise for women with GDM due to their growing belly. Although some researchers have explored this notion, there are no consistent conclusions about its effects. AIM: To explore whether resistance training has an effect on blood sugar levels and pregnancy outcomes in patients with GDM in randomized controlled trials. METHOD: Pubmed, Cochrane Library, CINAHL, Embase, Scopus, Web of Science, Clinical Trials, CKNI, Wanfang Database, VIP Database, and Chinese Biomedical Literature Database were systematically searched since their establishment to April 2019. Relevant meta-analyses, reviews, and eligible literature were also searched. The quality of the included literature was evaluated according to the Cochrane Assessment Manual 5.1.0. Meta-analysis was performed using Revman 5.3 software. FINDINGS: A total of four studies (n = 242 patients) were included. Compared to the control group, there were statistical differences in fasting blood glucose level [MD=-0.37, 95%CI=(-0.65, -0.09), Z = 2.62, P = 0.009], average 2-h post-meal blood glucose level [MD=-0.96, 95%CI=(-1.80, -0.12), Z = 2.25, P = 0.02], insulin dosage [MD=-0.58, 95%CI=(-0.99, -0.17), Z = 2.75, P = 0.006], rate of insulin injection [RR=0.52, 95%CI=(0.31, 0.86), Z = 2.54, P = 0.01], and incidence of macrosomia [RR=0.15, 95%CI=(0.04,0.66), Z = 2.53, P = 0.01] in the intervention group consisting of GDM patients. There were no statistical differences in preterm delivery outcomes [RR=0.44, 95%CI=(0.09, 2.16), Z = 1.01, P = 0.31]. CONCLUSION: Resistance training can improve blood sugar levels, insulin usage, and some adverse pregnancy outcomes in patients with GDM and is therefore worthy of clinical promotion.

OBJECTIVE:To investigate the effects of Qigong pills decoction combined with chemical drugs on hormone levels of polycystic ovarian syndrome(PCOS)patients complicated with infertility. METHODS:A total of 98 PCOS patients complicated with infertility selected from gynaecology department of our hospital during Jul. 2015-Jun. 2016 were divided into observation group and control group according to random number table,with 49 cases in each group. Control group was given Clomiphene ci-trate tablet 50 mg orally,qd,for consecutive 5 d;B-ultrasonography for follicle development was monitored at the 11th day of menstruation,and then they were given Chorionic Gonadotropin for injection 2000 U intramuscularly,qod,if there was not domi-nant follicle >10 mm till the diameter of follicle reached 18 mm. Observation group was additionally given one dose of Qigong pills decoction,every day 400 mL after decocted with water,each 200 mL for morning and night,on the basis of control group. After 3 months of treatment,the levels of FSH,LH,PRL,E2,T,DHEA-S and SHBG,the endometrial thickness,the number of mature follicle and pregnancy rate were compared between 2 groups. RESULTS:Before treatment,there was no statistical signifi-cance in the levels of FSH,LH,PRL,E2,T,DHEA-S or SHBG between 2 groups(P>0.05). 3 months after treatment,the lev-els of PRL,T and DHEA-S in 2 groups were decreased and E2,SHBG were increased significantly;the levels of FSH,PRL,T and DHEA-S in observation group were significantly lower than control group,while the levels of E2 and SHBG were significantly higher than control group,with statistical significance (P<0.05). Endometrial thickness of observation group were significantly higher than that of control group;pregnancy rate (18.37%) was significantly higher than control group (4.08%),with statistical significance (P<0.05). CONCLUSIONS:Qigong pills decoction combined with chemical drugs can help to improve pregnancy rate and regulate serum hormone level in the treatment of PCOS complicated with infertility.

RESUMO

Objective To investigate depression disorders and its influencing factors among senile patients with type 2 diabetes mellitus(T2DM) in rural-urban fringe in order to supply reference for commu-nity nursing intervention for it. Methods This study was conducted among 323 residents above 60 years old in Gushan town, Fuzhou City. CES-D (center for epidemiology survey-depression scale) and ADL (ac-tivities of daily living scale) were used to evaluate the depression condition and related factors in these pa-tients and the results underwent analysis. Results 25.7% of people with diabetes had various degree of depression, among which 13.3% might had depression and 12.4% had depression definitely. Patients with loss of spouse, living along,reduction of self- care ability and low BMI showed higher incidence of depres-sion. Variables went into the unconditional multivariate logistic regression equation included marriage status and BMI. Conclusions T2DM patients with loss of spouse, living alone,reduction in self-care ability of daily living and low BMI are high risk population of depression. It was proposed that psychological nursing for the aged should be the important content of community nursing.

Objective To describe the status of activities of daily living (ADL) and explore the influential factors among the elderly in Gushan town of Fuzhou. Methods A census was conducted by using questionnaires among people aged 60 years and above at this area. Results 17. 3% of 4237 old people investigated decreased their ADL The main influential factors were natural attribute, social economic features, living habits, depressive symptoms, psychomotility, and number of diseases. Conclusions Most of the old people in Gushan town of Fuzhou could take care of their life. Many factors showed great influence on ADL Applying to all-around measures and improving the aged's functions could help to implement successful aging.

Objective To investigate the relation of physical,mental,and social health status with family functioning in elderlv outpatients with chronic diseases to provide theoretic guidance for their community-based health care.Methods Cluster sampling methods were used to assess 568 elderly patients with at least one chronic disease at Gushan Town of Fuzhou City by Activity of Daily Living Scale(ADL),Center for Epidemiologic Studies Depression Scale(CES-D),self made daily social activity questionnaire,and family APGAR index questionnaire.Results The daily activity dependence rate was 23.8%.The prevalence of depression symptoms was 22.9%.Those with normal functions.and modest or severe dysfunction accounted for 56.9%,36.4%,and 6.7%,respectively.The daily activity dependence rate,the prevalence of depression symptoms,and daily social activity were found to be significantly associated with family functioning (P<0.05).Conclusion The health situations of elderly outpatients with chronic diseases may be correlated with family functioning.The community health care providers should concern both individuals and households to improve overall physical and memal health.

Objective To know the current status and its main influential factors of overweight and obesity among residents in Gushan county, Fuzhou city. Method The population aged above 20 years old was sampled by multistage sampling. Back- ground information was collected by face-to-face interview. Data on weight and height was obtained through physical check-ups. Results There were 2 412 objects were interviewed and physically measured. The overall prevalence was 26.0% for overweight and 7.1% for obesity. OBJECTIVE To explore the interventions for ventilator-associated pneumonia(VAP) caused by Acinetobacter baumannii by making investigation to the ICU patients on ventilators.METHODS Bacteria culture of bronchial discharge of patients on ventilators was conducted and followed by causation analysis of VAP caused by A.baumannii.RESULTS The prevalence of VAP caused by A.baumannii was 8.7%.The main reasons for VAP caused by A.baumannii were inappropriate use of antibiotics,long duration of mechanical assisted ventilation,contaminated ventilator and its accessories,unable to perform proper aseptic technique,and severity of the underlying illness.CONCLUSIONS For patients on ventilator,effective nursing interventions and sterilization and isolation techniques can reduce the infection and promote the treatment effects.
