In an outbreak of plasmid-free Salmonella enteritidis phage type 4 (PT4) food poisoning at a hospital for mentally handicapped people in July 1990, 101 residents and 8 staff were affected and a cohort study implicated beef rissoles cooked by deep-fat frying as the vehicle of infection (relative risk 2.92, 95% confidence interval 1.73-4.93, P << 0.001). Replication of the cooking process demonstrated that the rissoles achieved core temperatures of only 48-60 degrees C despite external temperatures of 91-95 degrees C and an oil temperature of 142-154 degrees C. No residual food was available for microbiological testing but plasmid-containing S. enteritidis PT 4 was isolated in shell eggs from the hospital kitchen.

An outbreak of Salmonella typhimurium DT9 food poisoning due to contaminated chicken pieces affected at least 196 delegates at a medical conference. Twelve per cent of the cases reported suffering parasthaesiae. Nine reported serious complications of infection which included rectal prolapse and perianal abscess. Antibiotic therapy did not measurably reduce the duration or severity of diarrhoea. Over 1,600 doctor-days were lost to the NHS. Most doctors sought expert infection control advice before returning to work but the advice given differed between hospitals.

A total of 100 tropical aquaria from six pet shops were sampled for the presence of Salmonella, Campylobacter and Aeromonas species. Eight fish tanks contained unusual serotypes of salmonella. None had been used to house terrapins. One hundred and three isolates of Aeromonas species were obtained from 98 tanks. One tank contained Plesiomonas shigelloides. No Campylobacter species were isolated. It is suggested that a tropical aquarium may constitute an infection hazard.

A ward outbreak of diarrhoea associated with Shigella sonnei on a long stay psychogeriatric ward is described. Nine patients and three staff had positive stool cultures for S. sonnei. The source of infection of the index case was not discovered. Environmental factors at ward level which predispose long stay hospitals to such outbreaks are discussed.
