RESUMO

OBJECTIVE: To demonstrate that a T2 periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) technique using deep learning reconstruction (DLR) will provide better image quality and decrease image noise. METHODS: From December 2020 to March 2021, 35 patients examined cervical spine MRI were included in this study. Four sets of images including fast spin echo (FSE), original PROPELLER, PROPELLER DLR50%, and DLR75% were quantitatively and qualitatively reviewed. We calculated the signal-to-noise ratio (SNR) of the spinal cord and sternocleidomastoid (SCM) muscle and the contrast-to-noise ratio (CNR) of the spinal cord by applying region-of-interest at the spinal cord, SCM muscle, and background air. We evaluated image noise with regard to the spinal cord, SCM, and back muscles at each level from C2-3 to C6-7 in the 4 sets. RESULTS: At all disc levels, the mean SNR values for the spinal cord and SCM muscles were significantly higher in PROPELLER DLR50% and DLR75% compared to FSE and original PROPELLER images (P < .0083). The mean CNR values of the spinal cord were significantly higher in PROPELLER DLR50% and DLR75% compared to FSE at the C3-4 and 4-5 levels and PROPELLER DLR75% compared to FSE at the C6-7 level (P < .0083). Qualitative analysis of image noise on the spinal cord, SCM, and back muscles showed that PROPELLER DLR50% and PROPELLER DLR75% images showed a significant denoising effect compared to the FSE and original PROPELLER images. CONCLUSION: The combination of PROPELLER and DLR improved image quality with a high SNR and CNR and reduced noise. ADVANCES IN KNOWLEDGE: Motion-insensitive imaging technique (PROPELLER) increased the image quality compared to conventional FSE images. PROPELLER technique with a DLR reduced image noise and improved image quality.

RESUMO

Autoimmune neuroinflammatory diseases are a group of disorders resulting from abnormal immune responses in the nervous system, causing inflammation and tissue damage. The interleukin (IL) family of cytokines, especially IL-1, IL-6, and IL-17, plays a critical role in the pathogenesis of these diseases. IL-1 is involved in the activation of immune cells, production of pro-inflammatory cytokines, and promotion of blood-brain barrier breakdown. IL-6 is essential for the differentiation of T cells into Th17 cells and has been implicated in the initiation and progression of neuroinflammation. IL-17 is a potent pro-inflammatory cytokine produced by Th17 cells that plays a crucial role in recruiting immune cells to sites of inflammation. This review summarizes the current understanding of the roles of different interleukins in autoimmune neuroinflammatory diseases, including multiple sclerosis, amyotrophic lateral sclerosis, Alzheimer's disease, neuromyelitis optica, and autoimmune encephalitis, and discusses the potential of targeting ILs as a therapeutic strategy against these diseases. We also highlight the need for further research to better understand the roles of ILs in autoimmune neuroinflammatory diseases and to identify new targets for treating these debilitating diseases.

OBJECTIVE: This study aimed to explore the myelin volume change in patients with mild traumatic brain injury (mTBI) with post-concussion syndrome (PCS) using a multidynamic multiecho (MDME) sequence and automatic whole-brain segmentation. MATERIALS AND METHODS: Forty-one consecutive mTBI patients with PCS and 29 controls, who had undergone MRI including the MDME sequence between October 2016 and April 2018, were included. Myelin volume fraction (MVF) maps were derived from the MDME sequence. After three dimensional T1-based brain segmentation, the average MVF was analyzed at the bilateral cerebral white matter (WM), bilateral cerebral gray matter (GM), corpus callosum, and brainstem. The Mann-Whitney U-test was performed to compare MVF and myelin volume between patients with mTBI and controls. Myelin volume was correlated with neuropsychological test scores using the Spearman rank correlation test. RESULTS: The average MVF at the bilateral cerebral WM was lower in mTBI patients with PCS (median [interquartile range], 25.2% [22.6%-26.4%]) than that in controls (26.8% [25.6%-27.8%]) (p = 0.004). The region-of-interest myelin volume was lower in mTBI patients with PCS than that in controls at the corpus callosum (1.87 cm3 [1.70-2.05 cm3] vs. 2.21 cm3 [1.86-3.46 cm3]; p = 0.003) and brainstem (9.98 cm3 [9.45-11.00 cm3] vs. 11.05 cm3 [10.10-11.53 cm3]; p = 0.015). The total myelin volume was lower in mTBI patients with PCS than that in controls at the corpus callosum (0.45 cm3 [0.39-0.48 cm3] vs. 0.48 cm3 [0.45-0.54 cm3]; p = 0.004) and brainstem (1.45 cm3 [1.28-1.59 cm3] vs. 1.54 cm3 [1.42-1.67 cm3]; p = 0.042). No significant correlation was observed between myelin volume parameters and neuropsychological test scores, except for the total myelin volume at the bilateral cerebral WM and verbal learning test (delayed recall) (r = 0.425; p = 0.048). CONCLUSION: MVF quantified from the MDME sequence was decreased at the bilateral cerebral WM in mTBI patients with PCS. The total myelin volumes at the corpus callosum and brainstem were decreased in mTBI patients with PCS due to atrophic changes.

Four-dimensional (4D) flow magnetic resonance imaging (MRI) allows three-dimensional velocity encoding to measure blood flow in a single scan, regardless of the intracranial artery direction. We compared blood flow velocity quantification by non-contrast 4D flow MRI and by transcranial Doppler ultrasound (TCD), the most widely used modality for measuring velocity. Twenty-two patients underwent both TCD and non-contrast 4D flow MRI. The mean time interval between TCD and non-contrast 4D flow MRI was 0.7 days. Subsegmental velocities were measured bilaterally in the middle cerebral and basilar arteries using TCD and non-contrast 4D flow MRI. Intracranial velocity measurements using TCD and non-contrast 4D flow MRI demonstrated a strong correlation in the bilateral M1, especially at the proximal segment (right r = 0.74, left r = 0.78; all p < 0.001). Mean velocities acquired with 4D flow MRI were approximately 8 to 10% lower than those acquired with TCD according to the location of M1. Intracranial arterial flow measurements estimated using non-contrast 4D flow MRI and TCD showed strong correlation. 4D flow MRI enables simultaneous assessment of vascular morphology and quantitative hemodynamic measurement, providing three-dimensional blood flow visualization. 4D flow MRI is a clinically useful sequence with a promising role in cerebrovascular disease.

Black ginseng (BG), which is produced by repeated steaming and drying of fresh ginseng, has various pharmacological and therapeutic properties. This study investigated the anti-hyperglycemic and hypolipidemic effects of BG ethanolic extract in type 2 diabetic db/db mice. The levels of fasting blood glucose, HbA1c, insulin levels and thiobarbituric acid reactive substances values were decreased in the groups fed BG extract (BG) (100 and 900 mg/kg BW/day), compared to the control group (CON). In the BG compared with the CON, hepatic steatosis in the liver and the size of adipocytes in muscle tissue were improved. The administration of BG regulated the glucose transporter type (GLUT) 4 and 2, and peroxisome proliferator-activated receptor (PPAR) α and γ in muscle and liver. Moreover, ginsenosides (Rh4, Rg5, and Rk1), which profiled by HPLC, regulated the markers for lipid metabolism and glucose metabolism; PPARs and GLUTs in muscle and C2C12 rather than liver cells and tissue. These findings suggested that ginsenosides (Rh4, Rg5, and Rk1) from BG extract can ameliorate type 2 diabetes through their anti-hyperglycemic and hypolipidemic effects.

Polycyclic aromatic hydrocarbons (PAHs) are primarily produced during the incomplete combustion of organic matter. PAHs are suspected endocrine disruptors and possible carcinogenic materials. The major sources of human exposure to PAHs are inhaled fumes and food. The aim of this study was to provide an alternative drying method to mitigate PAH formation in dried red peppers. We prepared dried red pepper samples using air-drying and heat pump-assisted drying methods, and measured the concentrations of four PAHs (PAH4), benzo[a] anthracene (B[a]A), chrysene (CHR), benzo[b]fluoranthene (B[b]F), and benzo[a]pyrene (B[a]P), in the resulting pepper samples. The PAH concentrations ranged from 3.61 to 18.0 µg/kg and from 2.22 to 8.35 µg/kg in the air-dried and heat pump-dried pepper samples, respectively. Overall, the results have shown that dried peppers contain PAH4, that the drying conditions for these contaminants should be optimized for mitigating the PAH formation in dried red peppers.

PURPOSE: The purpose of our study was to investigate the effect of different slice thicknesses and/or interslice gaps on longitudinal and transverse relaxation times (T1 and T2) measured by a multi-dynamic, multi-echo (MDME) sequence. MATERIALS AND METHODS: This retrospective study included nine healthy subjects who underwent MDME sequence (at 3T) with four different combinations of slice thicknesses and/or interslice gaps: slice thickness of 4 mm and interslice gap of 0 mm (TH4/G0), TH4/G1, TH5/G0, and TH5/G1. T1 and T2 were measured in various brain regions by a qualified neuroradiologist with 8 years of clinical experience: the frontal white matter (WM), occipital WM, genu, splenium, frontal cortex, thalamus, putamen, caudate head, and cerebrospinal fluid (CSF). The paired samples t-test was used to investigate the effect of different slice thicknesses and interslice gaps (TH4/G0 versus TH4/G1 and TH5/G0 versus TH5/G1). P < 0.013 was considered statistically significant. RESULTS: T2 in all brain regions and T1 in the frontal WM, putamen, and CSF did not significantly change for different slice thicknesses and/or gaps (Ps > 0.013). In addition, T1 in all brain regions of interest did not significantly change between TH4/G0, TH4/G1, TH5/G0 and TH5/G1. However, T1 in some of the brain regions was higher with TH4/G0 than with TH5/G0 (occipital WM, frontal cortex, and caudate head) and with TH4/G1 than with TH5/G1 (occipital WM, genu, splenium and thalamus, all Ps < 0.013). CONCLUSION: T2 estimated using the MDME sequence was stable regardless of slice thickness or gap. Although the sequence seems to provide stable relaxation values, identical slice thicknesses need to be used for follow-up to prevent potential T1 changes.

Objective: The purpose of this study was to investigate the time-dependent effects of contrast medium on multi-dynamic, multi-echo (MDME) sequence in patients with brain metastases. Materials and Methods: This study included 7 patients with 15 brain metastases who underwent magnetic resonance (MR) examination which included MDME sequences at 1 minute, 10 minutes and 20 minutes after contrast injection. Two volumes of interests, covering an entire tumor (whole tumor) and the enhancing portion of the tumor, were derived from post-contrast synthetic T1-weighted images. Statistical comparisons were performed for three different time delays for histogram parameters of the longitudinal relaxation rate (R1) and the transverse relaxation rate (R2), and lesion volumes. Results: The mean and the median of R1 and the mean of R2 in both the whole tumor and the inner enhancing portion were larger on the 10 minutes delayed images than on the 1 minute or 20 minutes delayed images (mean of R1 in the whole tumor on the 1 minute, 10 minutes, and 20 minutes delayed images: 1.26 ms, 1.39 ms, and 1.37 ms; mean of R1 in the inner enhancing portion: 1.43 ms, 1.53 ms and 1.44 ms; all p < 0.017). The volumes of the whole tumor and the inner enhancing portion were significantly larger in the 10 minutes and 20 minutes delayed images than on the 1 minute delayed images (all p < 0.017). Conclusion: Magnetic resonance relaxation times and the volumes of the whole tumor and the inner enhancing portion were measured larger on the 10 minutes or 20 minutes delayed images than on the 1 minute delayed images. The MDME sequence immediately after contrast injection cannot fully reflect the effects of gadolinium-based contrast agent leakage in the tissue.

PURPOSE: This study aimed to evaluate the usefulness of transit time corrected cerebral blood flow (CBF) maps based on multi-phase arterial spin labeling MR perfusion imaging (ASL-MRP). METHODS: The Institutional Review Board of our hospital approved this retrospective study. Written informed consent was waived. Conventional and multi-phase ASL-MRPs and dynamic susceptibility contrast MR perfusion imaging (DSC-MRP) were acquired for 108 consecutive patients. Vascular territory-based volumes of interest were applied to CBF and time to peak (TTP) maps obtained from DSC-MRP and CBF maps obtained from conventional and multi-phase ASL-MRPs. The concordances between normalized CBF (nCBF) from DSC-MRP and nCBF from conventional and transition time corrected CBF maps from multi-phase ASL-MRP were evaluated using Bland-Altman analysis. In addition, the dependence of difference between nCBF (ΔnCBF) values obtained from DSC-MRP and conventional ASL-MRP (or multi-phase ASL-MRP) on TTP obtained from DSC-MRP was also analyzed using regression analysis. RESULTS: The values of nCBFs from conventional and multi-phase ASL-MRPs had lower values than nCBF based on DSC-MRP (mean differences, 0.08 and 0.07, respectively). The values of ΔnCBF were dependent on TTP values from conventional ASL-MRP technique (F = 5.5679, P = 0.0384). No dependency of ΔnCBF on TTP values from multi-phase ASL-MRP technique was revealed (F = 0.1433, P > 0.05). CONCLUSION: The use of transit time corrected CBF maps based on multi-phase ASL-MRP technique can overcome the effect of delayed transit time on perfusion maps based on conventional ASL-MRP.

Purpose To determine the association between the administration of the macrocyclic contrast medium gadobutrol and T1 relaxation time in the brains of patients with normal renal function by using multidynamic multiecho (MDME) magnetic resonance (MR) imaging sequences. Materials and Methods The institutional review board approved this retrospective study, and the need to obtain written informed consent was waived. This study included 46 patients (revealed by an electronic medical record search) who had received one or more gadobutrol injections and a maximum of one MR imaging contrast medium injection other than gadobutrol before MDME sequence acquisition. One radiologist performed quantitative analyses of regions of interest on quantitative T1 maps twice to cover the normal-appearing globus pallidus (GP), frontal white matter, frontal cortex, and thalamus. The number of administrations and the cumulative dose of gadobutrol, age, intervals between administrations, sex, and treatment were investigated. Univariable and multivariable linear regression analyses of the T1 values in four brain regions and the GP-to-thalamus signal intensity (SI) ratio were performed. P values of less than the Bonferroni-corrected value of .01 were considered to indicate significant differences. Results Intraobserver reproducibility was good to excellent (intraclass correlation coefficients, 0.62-0.81). Because of high multicollinearity between the number of gadobutrol administrations and accumulated dose (r = 0.96, P < .001), the number of gadobutrol administrations was considered in the regression analyses. T1 shortening in the GP was independently associated with the number of gadobutrol administrations (P = .002). T1 in the other brain regions and the GP-to-thalamus SI ratio were not significantly associated with the number of gadobutrol administrations (P > .01). OBJECTIVE: To investigate the interobserver and test-retest reproducibility of T1ρ and T2 measurements of lumbar intervertebral discs using 3T magnetic resonance imaging (MRI). MATERIALS AND METHODS: This study included a total of 51 volunteers (female, 26; male, 25; mean age, 54 ± 16.3 years) who underwent lumbar spine MRI with a 3.0 T scanner. Amongst these subjects, 40 underwent repeat T1ρ and T2 measurement acquisitions with identical image protocol. Two observers independently performed the region of interest measurements in the nuclei pulposi of the discs from L1-2 through L5-S1 levels. Statistical analysis was performed using intraclass correlation coefficient (ICC) with a two-way random model of absolute agreement. Comparison of the ICC values was done after acquisition of ICC values using Z test. Statistical significance was defined as p value < 0.05. RESULTS: The ICCs of interobserver reproducibility were 0.951 and 0.672 for T1ρ and T2 mapping, respectively. The ICCs of test-retest reproducibility (40 subjects) for T1ρ and T2 measurements were 0.922 and 0.617 for observer A and 0.914 and 0.628 for observer B, respectively. In the comparison of the aforementioned ICCs, ICCs of interobserver and test-retest reproducibility for T1ρ mapping were significantly higher than T2 mapping (p < 0.001). CONCLUSION: The interobserver and test-retest reproducibility of T1ρ mapping were significantly higher than those of T2 mapping for the quantitative assessment of nuclei pulposi of lumbar intervertebral discs.

Diffusion tensor imaging is a new in vivo tool not only for the assessment of white matter structural integrity but also for diagnosis and assessment of disease conditions which disturb tissue structural coherence. In this study, we investigated the integrity of auditory pathway in patients of sensorineural hearing loss by means of fractional anisotropy of water diffusion to see any subtle changes of auditory pathways resulting from sensorineural hearing loss. In addition, this study suggests that the diffusion anisotropy measured by diffusion tensor imaging is highly sensitive to otherwise subtle disease processes not normally seen with conventional magnetic resonance imaging or computed tomography contrast.
