RESUMO

PURPOSE: To evaluate the efficacy of in vitroretrieval of epididymal sperm for the induction of pregnancy in patients with post-testicular causes of infertility, and assess the sperm distribution within the epididymis of these patients. MATERIALS AND METHODS: Analyses of the sperm count and motility of sperm retrieved from the detached epididymis, and of the resulting fertilization and pregnancy outcomes were conducted. A total 62 infertile couples, due to post-testicular male causes, had sperm retrieved from the epididymis. The sperm count and motility, as well as the fertilization and pregnancy rates, were measured. RESULTS: Epididymal sperm was retrieved from 60 males with post-testicular infertility. The average sperm concentration and motility of the sperm after sperm preparation were 43.0x10(6)/ml and 25%, respectively. The clinical pregnancy rate per cycle was 57.9%(11/19) with in vitro fertilization(IVF), 39.0%(16/41) with intracytoplasmic sperm injection(ICSI) using fresh epididymal sperm, and 42.9%(27/63) with ICSI using frozen epididymal sperm. CONCLUSIONS: The in vitro retrieval of epididymal sperm is recommended as a simple and effective method to obtain viable sperm from infertile patients with obstructive azoospermia or sperm excretion disorders.

RESUMO

For patients with a spinal cord injury, assisted ejaculation procedures are recommended as the treatment of choice. If assisted ejaculation procedures fail or yield spermatozoa insufficient to assisted reproductive techniques, surgical sperm retrieval is indicated. A total of 7 patients with a spinal cord injury, who still failed to obtain pregnancy after assisted ejaculation procedures, were treated in our clinic with the in vitro retrieval of epididymal sperm. The efficacy of in vitro retrieval of epididymal sperm was evaluated.

RESUMO

Spontaneous umbilical cord hematoma is a rare cause of fetal distress and intrauterine fetal death. The perinatal mortality rate approaches 50%. Various causes of the cord hematoma have been suggested as congenital abnormalities, short cord, trauma, cord around neck, torsion, syphilis, or postmature pregnancy. We have experienced one case of the fetal distress from umbilical cord hematoma, which is presented with a brief review of the literatures.

RESUMO

Selective termination can be used to preserve normal fetus from the other fetus with chromosomal, structural abnormality in multifetal pregnancy. This case is about twin pregnant patient who had undergone in vitro fertilization and embryo transfer. In 18 weeks of gestation, we found that one of the fetus had cranical encephalocele and was in concern for the termination by using ultrasound guided intrcardiac injection of 2.5 ml of potassium chloride solution. After performing this procedure the other healthy female newborn was naturally delivered at 38 weeks gestation with weight of 2.8 kg.

RESUMO

Anemia is the one of the most common complications among pregnant women, but sideroblastic anemia is very rare condition. The sideroblastic anemias have diverse etiologies but have in common an impaired biosynthesis of heme in the erythroid cells of the marrow. The ringed sideroblasts in the bone marrow aspirate is diagnostic hallmark of sideroblastic anemia. We report here a prenatal care and delivery in a pregnant woman complicated by hereditary sideroblastic anemia. This patient was treated with 200mg of pyridoxine per day during entire pregnancy period and further more, 4mg of oral folate per day was supplemented because concomitant folate deficiency is frequent in case of erythroid hyperplasia. Intermittently, the transfusions of packed red blood cells were required to maintain the hemoglobin level in the 9 to 10gm/dl range. We have experienced healthy maternal and perinatal outcome.

RESUMO

The incidence of cervical pregnancy and the number of combined intrauterine pregnancy and ectopic pregnancy seems to be increasing. So the possibility of heterotopic pregnancy should always be considered by every gynecologist, especially those who treat infertility problem. We experience a case of a heterotopic pregnancy coexisting of an intrauterine pregnancy and a cervical pregnancy after in-vitro fertilization and embryo transfer, and was successfully managed by transcervical evacuation and resulted in a normal intrauterine pregnancy. Here we present the case with review of literatures.

RESUMO

A complete hydatidiform mole coexisting with a fetus is a rare condition, particularly when diagnosed after IVF-ET. In spite of the wide spread use of assisted reproductive technology, there have been, to our knowledge, only few reported cases of molar pregnancy after IVF-ET. At present, there are limited data to guide the antenatal management of a complete hydatidiform mole coexisting with fetuses. A complete mole can coexist with a normal, healthy fetus who can be carried to term, with good outcome. But, patients who desire to continue the pregnancy after such a diagnosis must be cautioned about the potential for severe medical complications and developing persistent gestational trophoblastic tumor. We report here a case of complete hydatidiform mole in a triplet pregnancy coexistent with two live fetuses following in IVF-ET.
