RESUMO

The conjunctival wound leakage that occurs often afterglaucoma filtering surgery may predispose to the development of hypotony, infection, and bleb failure, so primary resuturing is tried to prevent them usually. Recently, because of the friable conjunctiva due to the use of mitomycin C(MMC) during the glaucoma filtering surgery, re-leakage is frequent after resuturing. We thought of the method that compress the leaking site without direct injury to leaking conjunctival wound and tried to the 4 patients, 6 eyes with the conjunctival wound leakage who had received the MMC trabeculectomy with fornix based conjunctival flap. We performed the suture posterior to the leaking sithe across the conjunctival flap with the nonabsorbing suture material after local anesthesia for the block of leakage due to the compression. Postoperatively, the sutures were removed at the time of no leakage and complete healing at conjunctival wound. And we had a good results in all 6 eyes in recent 2 years. The method of compression using this suture is one adequate method of management in conjunctival wound leakage after glaucoma filtering surgery.

RESUMO

Pseudomonas scleral abscess is characterized by acute onset and rapid progression to scleral perforation, and has a subsequent risk of endophthalmitis. After control of infection with appropriate antibiotics, graft surgery should be performed for reinforcing the defected sclera. We performed a scleral homograft on the eye with large necrotic sclera caused by Pseudomonas aeruginosa infection after pterygium excisIon. And the results were remarkably good.

RESUMO

Vitelliform macular dystrophy, adult type, is a type of pattern dystrophies of the pigment epithelium characterized by autosomal daminant inheritance, mid-life onset and small, round or oval, yellow deposits located at the level of the pigment epithelium. The authors report a case of this dystrophy which showed small yellow round lesion measuring 1/4-1/3 D.D. within the macula, normal electroretinogram, subnormal electrooculogram light-peak/dark-traugh ratio. typical irregular ring-like transmitted fluorescence surrounding the central non-fluorescent Iesion and leakage from perifoveal capillaries on fluorescent angiogram.

RESUMO

We analysed clinically 354 glaucomatous patients from 1983 to 1987 who visited the clinic of the Department of Ophthalmology of National Medical Center. The results were as follows: 1) The prevalence of glaucoma was 1.56% among the out-patients. 2) The primary open-angle glaucoma(POAG) was the most frequent type. 3) The mean age of the patients with POAG was 47.8 +/- 14.2 years and 60.8 +/- 8.9 years was that of the patients with primary angle-closure glaucoma(PACG). 4) There was no sexual preference in various types of glacoma, except PACG and secondary glaucoma. 5) There was no significant difference in laterality between right and left eyes.

RESUMO

Malignant lymphoma is mainly found in lymphnodes but extralymphatic organs may be rarely involved. Malignant lymphoma of the orbit is relatively rare representing less than 1% of all non-Hodgkin's lymphoma. All lymphoid tumors of the orbit are radiosensitive and are mainly treated wiht curative intent by irradiation. We report a case of orbital malignant lymphoma in a 41-year-old male patient. He received radiation therapy and subsequent enucleation and till now is clinically free of the disease.

RESUMO

Candida albicans keratitis was established in rabbits by intrastromal injection of a suspension containing 1.2 X 10(4) spores. After a 48-hour incubation period, the animals were treated with oral ketoconazole(10mg/kg daily) alone and in combination with topical ketoconazole(2% solution ten times a day) for 2 weeks. Clinical scores were significantly lower in both treated groups than in the control group. But there was no significant difference between the two treated groups.

RESUMO

Complicated microphthalmos can be diagnosed when a reduction in global dimension is combined with other ocular anomalies. These include corneal opacities or staphylomas, cataract, aniridia, corectopia, persistance of pupillary membrane or the tunica vasculosa lentis and hyaloid vessel, thickening or ossification of the choroid and various retinal anomalies. Such a deformity is frequently bereditary, and the transmission may be either as an autosomal dominant or recessive or as a sex linked recessive character. A 10-day-old female infant was found to have unilateral microphthalmos with microcornea, corneal opacity, and aphakia.

RESUMO

Ocular toxocariasis in human is a rare disease, which occurs by ingesting infective ova accidentally. Clinically, patients with ocular toxocariasis are divided into 3 groups: group 1 consisted of those with diffuse destructive endophthalmitis, group 2 included those focal posterior pole granuloma, group 3 was made up of those with peripheral inflammatory masses in a relatively quiet eye. We experienced a 17 year-old female whose left eye exhibited a dense white elevated inflammatory mass over the macular area and connective-tissue strands extending from the mass to vitreous and surrounding retina. We report this case with a review of literature, but we couldn't get the aid of the ELISA test for antibody of Toxocara canis.

RESUMO

Angioid streaks are easily recognized although relatively rare. They are typically described as irregularly radiating lines extending from the peripapillary area peripherally. They are usually bilateral, and are characteristically found in the posterior pole. Angioid streaks may occur alone or in association with various systemic diseases. Pseudoxanthoma elasticum is a relatively uncommon disorder of elastic tissue involving dermal, ocular, gastrointestinal, and cardiovascular tissues. We examined a case of angioid streaks associated with pseudoxanthoma elasticum found in a 31 year-old female and presented a brief review of the relating literatures.

RESUMO

Typical coloboma are due to some interference with the normal closure of the embryonic cleft, the aberration lying primarily in the epiblast of the optic cup. They occur between the time of invagination of the optic vesicle and closure of the cleft(between 7 and 14 mm stage of development), and include a whole series of anomalies varying from a complete defect involving the optic disc, the retina and choroid, the ciliary body, the iris, and even the lens to a minimal localized and partial failure of closure. The authors have experienced 2 cases of the typical coloboma of the iris, the zonule, the, ciliary body, the choroid and retina, the optic nerve head, and the lens, so briefly reviewed the litera.tures relating to typical coloboma.

RESUMO

Terrien's marginal degeneration was defined by Terrien in 1900 as a rare bilateral condition involving a peripheral cornea with degeneration and ectasia. About 75 percent of the reported cases has been in male between the age of 10 and 70. The majority of cases are first diagnosed before age 40. The condition usually arises in the superior portion of the cornea as a peripheral corneal opacity with gradual development of a peripheral furrow and progresses in a circumferential fashion. During evolution of the disease, the eye usually remains quiet and asymptomatic except for visual problems due to astigmatism. A 54 year-old Korean male has been found to have bilateral Terrien's marginal degeneration of the cornea and perforation on the right eye following trauma has been developed.

RESUMO

Primary orbital meningioma is rare. We have experienced a case of orbital meningioma arising from the orbital portion of the optic nerve sheath, which was attached to the optic sheath and extended through the optic canal to the intracranial cavity. Ten-year-old boy had suffered from visual disturbance in both eyes for seven months pre viously, his visual acuity was totally blind in the left eye and finger count/3m in the right eye. In the ophthalmoscopic finding of the left eye the optic disc was poorly demarcated and atrophic. Postoperatively his visual acuity was improved to 0.3 in the right eye.

RESUMO

We applied the cryosurgical technique to treat 10 selected patients with trichiasis. AmoHs retinal probe (CO2) was applied on the lids with temperature of -40 degrees C - -50 degrees C for 30 seconds using the double, rapid-freeze, slow-thaw technique. We observed recurrent trichiasis in a case, but retreatment by double freeze-thaw cycle was successful.

RESUMO

The resuIts of cryotherapy of the ciliary ganglion in absolute glaucoma are presented. The air cases had been suffered from severe ocular pain, headache without vision. The tip of the cryoapplicator (2.5mm in diameter, retinal probe, Amoils Cryo Unit) at a temperature between -60 degrees C to -70 degrees C was applied to the ciliary ganglion site for 30 seconds four to five times at one time. Edema of lids and conjunctiva occurred in all three cases except a case without lid edema and subsided in 5 days. All cases could spend life without pills for pain even though the tension was not decreased to normal leveI. The observation course was between 14 and 90 days.

RESUMO

In 1910 Scholler introduced the use of low temperatures in the field of ophthalmology. The report of cryoextraction of cataract by Krwawicz in 1961 acted as a catalyst; ophthalmologist all over the world saw the value of the cryosurgery in reducing the number of unintentional capsular ruptures. There are several types of cryogenic instruments. The most commonly used cryogens in ocular surgery are solid carbon dioxide, halogenated hydrocarbons (Freons) and liquid nitrogen. The major advantage of cryoextraction over the forceps and erisophake technics is the reduction of unintentional breakage of the lens capsule with the resulting decrease in the formation of secondary membranous cataracts and in the onset of postoperative inflammations. The applications of cryoextraction on the intumescent cataract, lens witb torn capsule, luxated lens and planned extracapsular method are briefly described.

RESUMO

The results of 8 cases of herpes simplex keratitis and one case of corneo-lenticular adhesion caused by explosion wound treated by cryogenic instrument (Amoils cryo unit) are presented. The cryoapplicator (2mm in diameter) were applied on the lesions 7-15 seconds. (Temperature: -50 degrees C ~ -70 degrees C). One of herpes simplex keratitis patients was suffered from redness of the lids following cryotherapy for 3 days.

189-192 Six male patients with typical symptoms of Behcet's syndrome were subjected to a clinical examination. One of these was a Chinese patient. The age when the disease started was between 20 to 39 years. All of them became blind but no one was dead.

RESUMO

The results of 234 cataract extractions by freezing method are presented. An important advantage of cryoextraction is the reduced incidence of accidental capsular rupture. Good results can be achieved even by the begginner. Cryoextraction of cataract has proved well suited as a routine method.
