RESUMO

No depending on the reason the large defects of the scalp, denuded from periost calvaria or without calvaria are difficult problem for reconstruction. There are several methods for reconstruction: local, distant pedicled and free flaps. Local fastiocutan flaps should be consider as a first step when planning scalp reconstruction. They are stable coverage and prepared by single technique with minimal functional loss and rapid patien recovery. In this paper we present 14 cases of this technique and discuss the advantage of the local fastiocutaneus scalp flaps.

RESUMO

Eight women with deliveries after undergone two and more cesarean section are described. Delivery occurred vaginally in 6 of them, but cesarean section was performed in two of them, whose indications were diagnosed after full dilatation of the cervical canal because of nonadvancement of delivery. A review of literature on this question is presented. Taking into consideration the literary data and their own observations an opinion is expressed that there is basis for a change in the present management of pregnant women with two and more undergone cesarean sections and is proposed to accept the concept for "trial of labor"--evaluation of the cases for vaginal delivery and those for elective cesarean section, taking into consideration criteria for delivery after cesarean section.

RESUMO

The short-lived antibiotic prophylaxis finds wider and wider usage in the present obstetric practice. The authors aim to examine clinically and to use in the obstetric practice short-lived antibiotic prophylaxis with doxycycline after interruption of pregnancy and residual abrasion due to incomplete abortion. The treatment of women from the experimental group was 500 mg of doxycycline per os at once, but 100 mg twice for 6 days in women from the control group in accordance with the common usage. Attention was paid to 3 basic symptoms: temperature over 37 degrees C, pains at the lower part of the abdomen and need of additional treatment. An inference i made that inflammatory complications after the two types of antibiotic prophylaxis does not differ in frequency (p greater than 0.05). The advantages of short-lived usage are evaluated as well.


PIP: Therapeutic efficacy of premedication with a single dose (500 mg) of broad-spectrum antibiotic doxycycline (vibramycin) was studied in 90 women undergoing abrasion residuorum following incomplete abortion. A group of 72 women subjected abortion on demand by means of vacuum aspiration and treated with conventional dose of doxycycline (100 mg, 2 times a day, for 6 days) was used as controls. The test and control groups were comparable with respect to age and duration of pregnancy. Complications included fever (5.6%, compared with 8.3% in control group) and pain in the lower abdomen (19.0%, compared with 20% in control group); 3.3% of the patients required additional treatment with antibiotics, compared with 4.1% in control group. These findings indicated that short-term administration of doxycycline was as effective in preventing inflammatory complications of abortion as conventional 6- day course.

RESUMO

The authors described the pregnancy and delivery of a woman with unstable angina pectoris (ECG data for ischemic disease of the heart). Delivery occurred through the vagina without complications for the mother and fetus. The literary references treat the question about course of pregnancy, way of delivery and analgesia in women with myocardial infarction during pregnancy.
