RESUMO

The efficacy of allapinin, ethacizine, ritmilen, kinilentin, obsidan and finoptin given alone and in different combinations was studied in 44 patients with paroxysmal atrial fibrillation provoked by transesophageal pacing. The use of the combinations of the drugs belonging to the first group as well of those of the first group combined with the drugs belonging to the 2nd-4th group permitted one to significantly enhance the treatment efficacy, to reduce drug doses in a lot of patients and, therefore, to minimize the rate of side effects. The data obtained promoted the choosing of the therapy algorithm to prevent paroxysms of atrial fibrillation. While designing the algorithm, account was taken of the rate of paroxysms and of the initial sinus rhythm as was of the results of estimating the comparative efficacy of the drugs under study.

RESUMO

Safety and the effectiveness of combined therapy of arrhythmia was studied in 27 patients in prolonged (up to 4 months) administration of a combination of medicinal agents specially chosen during pharmacodynamic investigation. The obtained anti-arrhythmic effect was stable and there were no new side effects in prolonged administration of combinations of ethmozine with chinidin, ritmilen, obsidan, cordaron (amiodaron) and of allapinin with chinidin, ritmilen (disopyramide) and obsidan (propronalol). Tolerance to drugs was adequate and the ECG parameters remained unaffected.

RESUMO

Eight versions of ECG alterations were identified in 84 patients with hypertrophic cardiomyopathy (HCMP). As for the most prevalent version (60.7 percent of the cases), the pathological Q waves or QS were recorded in the presence of the voltage signs of ventricular hypertrophy or without such signs. The use of ultrasound cardiography and roentgenocardiography made it possible to distinguish 3 groups of patients with different myocardial hypertrophy. The first group included 20 patients with isolated hypertrophy of the interventricular septum (IVS), the second group 40 patients with IVS and left ventricle hypertrophy, and the third group included 24 cases of IVS hypertrophy and combined hypertrophy of the ventricles. The main features of ECG alterations were determined in accordance with the types of hypertrophy under consideration. In the first group, Q waves were mainly recorded (in 71.4 percent of the cases). In the second group, the signs of left ventricle hypertrophy were recorded in 50 percent of the cases while Q/QS in 62.5 percent of the cases. In the third group, the voltage signs of ventricular hypertrophy were observed in 62.5 percent of the cases, whereas the Q and QS were recordable less commonly (in 41.7 percent of the cases).

RESUMO

New diagnostic parameters, improving the specificity and sensitivity of electrocardiographic diagnosis of right-ventricular hypertrophy, have been identified on the basis of an automated analysis of precordial maps in 56 patients with isolated right-ventricular hypertrophy and 71 normal subjects.

RESUMO

On the basis of verified material (136 patients with combined myocardial hypertrophy--CMH, 39 with left ventricular hyperthrophy, 57 with right ventricular hyperthrophy and 71 normal subjects) a principally new approach to computerized quantitative analysis of the EKG data has been worked out. Results of study of the informative value of 35 praecordial EKG in the diagnosis of CMH are presented. The authors propose to use the amplitude and area of S and R waves as well as their sum calculated with the use of non-traditional leads. This method makes it possible, when CMH differs from the norm, to increase the sensitivity of the electrocardiography up to 86.8% and its specificity up to 91.5%, which is more effective than the traditional Sokolov-Lyon criteria (sensitivity of 64% and specificity of 87.3%). Nevertheless, these characteristics are ineffective in differential diagnosis because increase in these values can be observed also by isolated hyperthrophy.
