BACKGROUND: Myiasis is an emerging disease caused by tissue invasion of dipteran larvae. In Uruguay, Cochliomyia hominivorax and Dermatobia hominis are the most frequent species. This study aimed to describe the epidemiological and clinical characteristics and the follow-up of children < 15 years hospitalized for myiasis in a reference center in Uruguay between 2010 and 2019. METHODS: We conducted a descriptive and retrospective study by reviewing medical records. We analyzed the following variables: age, sex, comorbidities, origin, the month at admission, clinical manifestations, other parasitoses, treatments, complications, and larva species identified. RESULTS: We found 63 hospitalized children: median age of 7 years (1 month-14 years), 68% of females. We detected risk comorbidities for myiasis (33%), of which chronic malnutrition was the most frequent (n = 6); 84% were from the south of the country; 76% were hospitalized during the summer. Superficial and multiple cutaneous involvements were found in 86%: of the scalp 50, furunculoid type 51, secondary to C. hominivorax 98.4%, and to D. hominis in 1.6%. As treatments, larval extraction was detected in all of them, surgical in 22%. Asphaltic products for parasites were applied in 94%, ether in 49. Antimicrobials were prescribed in 95%; cephradine and ivermectin were the most frequent. About 51% presented infectious complications, impetigo was found in 29, cellulitis in 2, and abscess in 1. CONCLUSIONS: Myiasis predominantly affected healthy schoolchildren during the summer months and was frequently associated with ectoparasites. Superficial infestation of the furuncle-like scalp by C. hominivorax was the most frequent form. Infectious complications motivated hospitalizations.

Antecedentes: el consumo de frituras en México es alto, aunque las grasas se asocian con enfermedades crónicas no transmisibles. Objetivo: evaluar la composición química y calidad de la grasa obtenida de churros fritos de maíz elaborados y consumidos en Navojoa, estado de Sonora, México. Materiales y métodos: a cuatro muestras obtenidas en establecimientos comerciales se les realizó análisis químico proximal y determinación de índices de calidad de la grasa (acidez, peróxidos, yodo y anisidina), según normas mexicanas. Resultados: el aporte nutricional de las muestras estuvo en los siguientes rangos expresados en g %: grasas (23,7±0,2 y 35,2±1,0 g %), proteínas (2,5±0,0 y 8,1±1,4 g %), carbohidratos (54,1±0,3 y 64,40±0,5 g %) y energía (485±3 y 531±1 kcal %) con diferencias entre ellas (p<0,05). El mayor contenido de grasa y energía lo presentó B2 y el mejor perfil nutricional B4. La muestra B1 superó los límites máximos permitidos de acidez (4,8) y de peroxidación (10,6) con diferencias respecto a las otras muestras (p<0,05). Conclusiones: la densidad energética de los churros de maíz analizados es alta (superior a 4 kcal/g) al igual que el aporte de grasas, especialmente en la muestra B2. B4 presenta el mejor perfil nutricional y B1 el mayor deterioro oxidativo.


Background: Consumption of fried foods in Mexico is high, even though fats in fried foods are associated with noncommunicable chronic diseases. Objective: Evaluate the chemical composition and fat quality obtained from fried corn churros prepared and consumed in Navojoa, state of Sonora, Mexico. Materials and Methods: Four samples obtained from commercial establishments were subjected to proximal chemical analysis and determination of fat quality indices (acidity, alkalinity, iodine and anisidine), according to Mexican standards. Results: The nutritional breakdown of the samples is shown in the following ranges, expressed as grams % (g %): fats (23.7 ± 0.2 and 35.2 ± 1.0 g %), proteins (2.5 ± 0.0 and 8.1 ± 1.4 g %), carbohydrates (54.1 ± 0.3 and 64.4 ± 0.5 g %), and energy (kcal) (485 ± 3 and 531 ± 1) with significant differences between samples (p<0.05). The highest fat and energy content was presented by sample B2 and the best nutritional profile by sample B4. Sample B1 exceeded the maximum limits of acidity (4.8) and alkalinity (10.6) with a significant inter-sample difference (p <0.05). Conclusion: The energy density of the sampled fried corn churros is high (above 4 kcal/gram), as well as the fat content, especially in sample B2. Sample B4 presents the best nutrition profile and sample B1 the greatest oxidative deterioration.

Resumen Antecedentes: la función principal de los edulcorantes no nutritivos es proveer al consumidor un producto dulce sin la carga calórica del azúcar. El consumo de edulcorantes no nutritivos se relaciona con alteraciones en el ADN, con apoptosis y con la síntesis de precursores de cáncer. Además, su consumo también se relaciona recientemente con un incremento de tejido adiposo. Objetivo: analizar el efecto de los edulcorantes no nutritivos a largo plazo, evaluando el riesgo de estos compuestos. Materiales y métodos: búsqueda en bases de datos, PubMed, SciELO y Redalyc, y análisis de bibliografía relacionada con efectos tóxicos y metabólicos del consumo de edulcorantes no nutritivos. Resultados: el consumo de estos edulcorantes presentó efectos citotóxicos en diferentes modelos de estudio. Parece existir una relación con el incremento en la síntesis de tejido adiposo, que provoca obesidad y enfermedades derivadas. Los edulcorantes sintéticos fueron los que presentaron más alteraciones citotóxicas, mientras que edulcorantes naturales, a excepción de los esteviósidos, no presentaron efectos adversos. Múltiples investigaciones explican el efecto del consumo de estos edulcorantes en el metabolismo y los efectos citotóxicos. Conclusiones: el tema de edulcorantes es controversial, por ello la información recopilada en esta revisión busca proporcionar un panorama que ayude a comprender la respuesta a nivel celular y metabólico sobre su consumo.


Abstract Background: The primary function of non-nutritive sweeteners is to provide the consumer with a sweet product without the caloric load of sugar. The consumption of non-nutritive sweeteners is related to alterations in DNA, apoptosis, and the synthesis of cancer precursors. Recently, its consumption has been related to an increase in adipose tissue. Objective: To analyze the effect of long-term consumption of these sweeteners and evaluate the risk of these related factors. Materials and Methods: Pubmed, Scielo and Redalyc database search and analysis of references related to toxic and metabolic effects of consumption of non-nutritrive sweeteners. Results: The consumption presents cytotoxic effects in different study models. There appears to be a relationship with an increase in synthesis of adipose tissue, which causes obesity and derived diseases. Synthetic sweeteners have the most cytotoxic alterations, whereas natural sweeteners, except steviosides, do not present adverse effects. Multiple investigations explain the impact of non-nutritive sweetener consumption on metabolism and related cytotoxic effects. Conclusion: The issue of sweeteners is controversial; therefore the information compiled in this review seeks to provide a panorama that helps understand the cellular and metabolic level responses of their consumption.

RESUMO

IgG4-related disease is an inflammatory condition characterized by high levels of IgG4. It affects salivary and lacrimal glands, pancreas, lymph nodes, lungs or kidney. The diagnosis is based on identifying a histological pattern with a dense lymphocyte and plasmacyte infiltration, focal fibrosis or phlebitis, finding more than 10 IgG4 positive cells per high power field and/or IgG4/IgG ratio in plasma higher than 40%. We present a patient with Mikulicz's disease who meets histological findings required for the diagnosis of IgG4 related disease.


La enfermedad relacionada con IgG4 es una condición fibroinflamatoria en la que existe elevación de IgG4, afección a nivel de glándulas salivares, lacrimales, páncreas, ganglios linfáticos y pulmón. Para su diagnóstico se requiere la identificación de un patrón histológico sugestivo que muestre infiltrado linfoplasmocitario denso, fibrosis focal o flebitis a nivel de una glándula, más de 10 células positivas para IgG4 por campo de gran aumento y relación de IgG4/IgG arriba de 40% en plasma. Describimos el caso de una paciente que presentó enfermedad de Mikulicz y cumplió con los datos histológicos para diagnóstico de enfermedad relacionada con IgG4.
