RESUMO

The hardened single line of sight camera has been recently characterized in preparation for its deployment on the National Ignition Facility. The latest creation based on the pulse-dilation technology leads to many new features and improvements over the previous-generation cameras to provide better quality measurements of inertial confinement fusion experiments, including during high neutron yield implosions. Here, we present the characterization data that illustrate the main performance features of this instrument, such as extended dynamic range and adjustable internal magnification, leading to improved spatial resolution.

RESUMO

Viscoelastic technics are used for 10 years and include the ROTEM® and the TEG®. These devices that exploit the viscoelastic properties of the clotting blood, allow a rapid and global analysis of the haemostatic process taking into account all the process interfering with haemostasis such as inflammation. It has been shown that the use of these technics in clinical situations such as trauma, postpartum haemorrhage, gastrointestinal bleeding or cardiac surgery may reduce the need for blood product, the cost and perhaps may improve the outcome of the patients. Thanks to a rapid identification of the underlying coagulation deficit, these technics facilitate decision-making during acute care and help to guide the treatment, particularly with coagulation factor's concentrate.

RESUMO

We demonstrate the first successful growth of large-area (200 × 200 µm(2)) bilayer, Bernal stacked, epitaxial graphene (EG) on atomically flat, 4H-SiC (0001) step-free mesas (SFMs) . The use of SFMs for the growth of graphene resulted in the complete elimination of surface step-bunching typically found after EG growth on conventional nominally on-axis SiC (0001) substrates. As a result heights of EG surface features are reduced by at least a factor of 50 from the heights found on conventional substrates. Evaluation of the EG across the SFM using the Raman 2D mode indicates Bernal stacking with low and uniform compressive lattice strain of only 0.05%. The uniformity of this strain is significantly improved, which is about 13-fold decrease of strain found for EG grown on conventional nominally on-axis substrates. The magnitude of the strain approaches values for stress-free exfoliated graphene flakes. Hall transport measurements on large area bilayer samples taken as a function of temperature from 4.3 to 300 K revealed an n-type carrier mobility that increased from 1170 to 1730 cm(2) V(-1) s(-1), and a corresponding sheet carrier density that decreased from 5.0 × 10(12) cm(-2) to 3.26 × 10(12) cm(-2). The transport is believed to occur predominantly through the top EG layer with the bottom layer screening the top layer from the substrate. These results demonstrate that EG synthesized on large area, perfectly flat on-axis mesa surfaces can be used to produce Bernal-stacked bilayer EG having excellent uniformity and reduced strain and provides the perfect opportunity for significant advancement of epitaxial graphene electronics technology.

RESUMO

The aim of this study was presentation of a whole-body MRI technique with a moving table as a screening tool for bone metastases in patients with breast cancer. Twenty-two patients with breast carcinoma underwent both a planar whole-body bone scintigraphy and whole-body MRI at 1.5 T. The MRI images were acquired with a moving table at six different anatomical positions within a measurement time of 20 min. Coronal images were acquired using a short-tau inversion recovery sequence, accomplished by an axial T2-weighted turbo-spin-echo sequence through the head, and a T1-weighted opposed-phase sagittal 2D fast low-angle shot sequence covering the whole spine. The MRI findings indicating bone metastases were compared with findings from bone scintigraphy. Metastatic lesions were confirmed by follow-up examinations over 1 year. Twelve patients showed bone metastases. Whole-body MRI was superior to bone scintigraphy in predicting lesion origin with a sensitivity of 92% (bone scintigraphy 83%), a specificity of 90% (scintigraphy 80%) and an accuracy of 91% (scintigraphy 82%). The MRI showed additional findings such as metastases of the lung and liver. Whole-body MRI with moving table technique may be an effective method of total body screening for bone in selected patients with breast carcinoma and a high risk of distant metastases, although with the higher costs of MRI bone scintigraphy must still be considered as the first method for screening patients with breast cancer.

RESUMO

This article describes to what extent the expected rise in the educational level of the Dutch population can counterbalance the increases in the prevalence of ill-health and health care utilization based on the aging of the population for the period 1996-2020. Logistic regression models are used to estimate current differences in health (care utilization) by age, sex, and educational level, using data from the Netherlands Health Interview Survey. The current differences in health (care utilization) are applied to national projections of the composition of the population by age, sex, and educational level. Also, scenarios have been made in which the health differences by educational level are assumed to converge and diverge. The rise in the educational level counteracts the expected increases in ill-health based on population aging to a substantial degree (10-100%). We therefore recommend that in projections of ill-health also changes in educational level are taken into account.

RESUMO

PIP: The use of microsimulation methods for making population projections is examined. "A particular problem in microsimulation results from the fact that the projections are subject to random variation. Various sources of random variations are examined but the most important is the one we refer to as specification randomness: the more explanatory variables are included in the model, the greater the degree of random variation affecting the output of the model. After a brief survey of the microsimulation models which exist in demography, a number of the essential characteristics of microsimulation are illustrated using the KINSIM model for projecting the future size and structure of kinship networks." RESUMO

PIP: This study examined the budgetary consequences of alternative policies on aging in the public sector in the Netherlands. Budgetary consequences include salaried employee costs, the benefit costs of the inactive population aged under 65 years, and the implicit costs of not replacing retired workers. Data were obtained from the largest pension fund (ABP). Public sector includes all sectors covered by the ABP and includes all civil servants, educational workers, and noncommercial services, excluding health care. The authors construct a flow-model of public sector workers in 1993, with varying age specific rates of change in inflow, outflow, and position. Scenarios answer questions about the impact of aging on labor productivity and demand for new workers, and the effects of redistributing labor, changing early retirement schemes (ERS), and retiring part-time. It is assumed that labor productivity per hour is the same regardless of age. Salary increases with age, but scenarios assume a constant age-salary profile. Participation in senior worker schemes and/or use of inactivity schemes by restricting eligibility criteria will not generate substantial savings in public spending. ERS cutbacks will pass expenses on to other agencies and not reduce total public spending. Increased part-time retirement produces limited budgetary benefits. Costs increase with age and are reflected in ERS and unemployment schemes. Alternatives will reflect disability costs, salary costs of active workers, or unemployment benefits to young people who cannot find jobs because older workers are retiring too slowly. Aging societies are expensive.^ieng

RESUMO

"A full multiregional projection model requires migration data that are simultaneously classified by age and gender and region of origin and region of destination. Except for a very small number of regions, these data requirements are so high that aggregation of the data (which is equivalent to simplification of the model) is called for. This paper investigates the extent to which the full internal migration matrix can be simplified without seriously affecting the performance of the resulting multiregional model. Using IPF (iterative proportional fitting) methods, a log-linear analysis of alternative model specifications is made, using data for Italy, the Netherlands and the UK....A reasonable balance between goodness-of-fit and parsimony is found for the model in which time interacts with the main effects only (i.e. with age/sex, with origin and with destination)."

RESUMO

"This paper provides a geometric-mean solution to the consistency problem of multidimensional demographic projection models, based on the constrained minimization of an entropy function. A comparison with the existing harmonic-mean solution yields many similarities and almost no differences....However, one major advantage of the geometric mean is that its corresponding distance function is firmly based on (information) theory." RESUMO

"This paper studies the impact of demographic change and international migration on economic development and the education sector. We employ a simple simulation model for tracing the impact of international migration on the educational and economic system, under alternative assumptions on the education background and adaptation costs of migrants. An application to the case of the Netherlands shows that international migration of whatever (realistic) level will not be able to prevent strong population aging during the period 2010-2035. Given the current below-average educational and productive profile of the immigrant population in the Netherlands, increased migration will only make matters worse."

RESUMO

"This paper presents the multidimensional demographic projection on the basis of an exponential specification, which avoids the problems associated with the widely used specification on the basis of the linear integration hypothesis. It generalizes earlier work by Gill (1986) on Markov models for closed populations to include the case of open populations. A numerical illustration is also provided." (SUMMARY IN FRE)

RESUMO

PIP: The significance of deviations from exponential population growth to economic development requires the construction of growth models that explicitly recognize demographic forces as a potential source of nonstationarities. This paper uses an overlapping generations approach to analyze the impact of population aging on technological innovations in the production process. Specifically, it is assumed that a newly invented technology can be used only if the production unit engages human labor that has been trained for this task (labor-embodied technical change). In a model of overlapping generations 1-sector optimal growth, there are 3 basic components: population, education and labor, and production and investment. An individual who has been in school continuously from the time of birth embodies, according to this model, the most up-to-date form of human capital. When the population growth rate is high, technological innovations are achieved largely through the constant influx of recently educated young people into the labor force. However, when population declines lead to a slowing down of this influx, increased adult education is necessary to stimulate the continued introduction of innovations into the productive process. The form of the optimality conditions, the comparative statistics properties, and the form of the nonstationary optimal economic growth path in this model are similar to those in the Van Imhoff model of homogeneous human capital.^ieng

RESUMO

It is obvious that the surfaces of the boxes of sterile packed disposable instruments and infusion bottles are not sterile. The disposable surgical masks and surgical caps used for sterile clothing are delivered by the producers not sterile, either. To quantify these gaps and to judge their risks in the aseptic region the surfaces of 117 sterile packed disposable instruments and the inner sides of their boxes were examined bacteriologically. The surfaces of these objects proved to be not sterile by 21% and 4% were heavily contaminated with saprophytic germs. 3% of the examined articles showed pathogenic germs like Clostridium perfringens, Staphylococcus aureus, and Acinetobacter spec. 5-15% of the surfaces (glass- respectively plastic and labels) of the 331 infusion bottles proved to be heavily bacteriologically contaminated; 4-6% of them even showing pathogenic germs like Clostridium perfringens, Staphylococcus aureus, and Achromobacter spec. The surfaces of 25% of the examined disposable surgical masks and caps were considerably contaminated with saprophytic germs. Although pathogenic germs could not be detected, it means that these not sterile objects represent a considerable deficiency in surgical asepsis.

RESUMO

The hemodynamic effects of intravenously administered trinitroglycerin (TNT) and isosorbide dinitrate (ISDN) in stechiometric equivalent doses were studied in 30 patients with acute myocardial infarction. Both drugs were given over 30 min in every patient, and the stability of the condition of the patient was checked by administering the initial drug again. Measurements were made by right heart catheterization using a balloon tip thermodilution catheter and a catheter in the radial artery. TNT and ISDN have different hemodynamic effects. By multivariate analysis it could be shown that TNT is a venous dilator, lowering mainly pulmonary capillary pressure, while ISDN acts more as a mixed vasodilator, diminishing mainly peripheral vascular resistance and increasing cardiac output. These drugs have, therefore, a different spectrum of indications in patients with acute myocardial infarction.
