RESUMO

Objetivos: La evolución de los procedimientos laparoscópicos hace que sea necesario en ocasiones, la realización de cirugías combinadas con otras especialidades. Actualmente, algunas de estas intervenciones se pueden realizar mediante cirugía por puerto único umbilical. Material y métodos: Presentamos el caso de un paciente con tumoración renal de 4,5 cm y colelitiasis al que realizamos colecistectomía y nefrectomía radical derecha por puerto único umbilical de manera concomitante. Este es el primer caso publicado en España en el que se realiza esta intervención combinada mediante cirugía de puerto único umbilical. Resultados: Se realizó la cirugía sin complicaciones intraoperatorias ni postoperatorias, con un correcto control de la patología tumoral y un excelente resultado estético. Conclusión: En las situaciones que sean necesarias realizar un abordaje multidisciplinar con otras especialidades quirúrgicas involucradas, estas se pueden realizar mediante cirugía por puerto único umbilical, con las ventajas de una menor morbilidad postoperatoria y un mejor resultado estético

Material and methods: We report a patient with renal tumor of 4.5 cm and cholelithiasis who undergone to trans-umbilical single port-right radical nephrectomy with concomitant cholecystectomy. This is the first case reported in Spain that this combined procedure is performed using umbilical single port surgery. Results: No complications (intra or postoperative) have been described in this case, achieving proper control of tumor pathology and an excellent cosmetic outcome. Conclusion: In those cases in which multidisciplinary approach is required, surgery can be performed by trans-umbilical single-port laparoscopy as consequence of its reduced postoperative morbidity and better cosmetic results

RESUMO

El diagnóstico diferencial del síndrome confusional agudo abarca un gran número de procesos entre los que hay que considerar firmemente las enfermedades metabólicas. El diagnóstico del hiperparatiroidismo como causa del mismo debe apoyarse en parámetros biológicos, y su tratamiento tiene un enfoque inicial destinado a corregir el trastorno neuropsiquiátrico y otro definitivo basado en la cirugía para revertir la desmineralización ósea y la mortalidad ulterior relacionada con la hipercalcemia. El abordaje quirúrgico debe efectuarse por cirujanos experimentados, cuyo objetivo principal conlleva la extirpación de la glándula enferma mediante técnicas lo menos agresivas posibles. Los estudios de imagen preoperatorios pueden contribuir a un abordaje selectivo que evite una exploración cervical bilateral. La determinación intraoperatoria de hormona paratiroidea intacta (PTHi), o el empleo de sonda isotópica, contribuyen a asegurar una solución quirúrgica efectiva


The differential diagnosis of acute confusional state encompasses a large number of processes including metabolic diseases. The diagnosis of hyperparathyroidism as a cause of acute confusional state should rest on biological parameters; treatment should initially be focused on correcting the neuropsychological disorder and then definitive surgical treatment should be undertaken to reverse bone demineralization and ulterior mortality related to hypercalcemia. The surgical approach should be carried out by experienced surgeons with the principal aim of extirpating the diseased gland using the least aggressive techniques possible. Preoperative imaging studies can help to ensure a selective approach that obviates bilateral cervical exploration. Intraoperative determination of intact parathyroid hormone (PTHi) levels or the use of a gamma probe help to ensure an effective surgical solution

RESUMO

Cells that produce somatostatin are widely distributed throughout the digestive tube. They are found in the stomach, small bowel, large bowel and pancreas. The authors used 54 Wistar rats, with an approximate weight of 300 gr, to evaluate the possible variations of the D cell population in isolated and functional intestinal segments, using jejuno-ileal bypass as a model. Rats were divided into three groups, a control group and two groups in which simple derivation techniques were performed following the techniques of Payne and DeWind, and Scott. Rats were sacrificed after 7, 30 and 90 days in subgroups of six animals. Macroscopic, microscopic and ultrastructural studies were carried out. Cells were specifically stained using immunocytochemical techniques (PAP). The corresponding values of the mucous areas were obtained using a computerized image analyzer (Quantimet 800) and then the number of D cells per mm2 mucosa was calculated. The results show a decrease in the number of D cells per mm2 of mucosa in the functional intestinal segments and degranulation of these cells, coinciding with the existence of large areas of rough endoplasmic reticulum (sign of cellular hyperactivity).
