RESUMO

UNLABELLED: Operations were performed on 750 children with cystic hypoplasia and dysontogenetic and acquired bronchiectases. The operation volume: pulmonectomy, lobectomy, combined resections, resection of pulmonary tissue with extirpation of bronchi, extirpation of bronchi. COMPLICATIONS: atelectasis, pneumonia, intrapleural hemorrhage were encountered in 87 (11.6%) children. Six (0.8%) children died. Recurrent or residual bronchiectases were found in 27 children, 18 underwent a second operation. Recurrences were encountered mainly in children with dysontogenetic bronchiectases.

RESUMO

In the period between 1970 and 1983 the authors had 242 patients (aged from 19 to 67) with expiratory stenosis of the trachea and main bronchi under observation. Complex bronchological examination was conducted with study of the mechanics of respiration at different levels of the tracheobronchial tree before and after treatment. The object of treatment was to arrest the inflammatory process in the tracheobronchial tree, restore the tonus of the membranous part, and reduce intrathoracic pressure. Respiration was controlled in 28 patients by means of a special tube with dosed resistance during inspiration. The indications and contraindications for surgical management of expiratory stenosis were determined. The choice of the graft for stenosis correction is guided by the severity of expiratory stenosis.

RESUMO

The number of operated on patients who are referred to a commission for expert medical assessment of working capacity is growing at present. Correct expert assessment of the working capacity allows the results of surgery to be reliably consolidated and promotes restoration of working ability. The duration and severity of working ability. The duration and severity of the pulmonary abscess, the method of surgical treatment, the lapse of time after the operation, the course of the restorative processes, complications and concomitant diseases, the degree or respiratory and circulatory insufficiency, the patients' age, profession, and the conditions and character of work are taken into account during examination. Timely and complete treatment and rational choice of occupation are important factors in the prevention of invalidism and enable most patients (86.8%) to resume work in full volume or with limitations as specified by the commission.

RESUMO

The immediate results of conservative (116 subjects) and surgical (37 subjects) treatment of patients with lung abscesses were studied and the remote outcomes were followed up for periods of 1 to 8 years in 104 therapeutically treated and 33 surgically treated patients. 13.7 and 40.4 per cent of the patients, respectively, were discharged from the hospital after complete and clinical recovery. In 41.6 per cent of the patients, chronic abscesses were stated. 4.3 per cent of the patients died. After the surgical treatment postoperative complications were observed in 51.3 per cent of the patients, fatal outcomes were stated in 10.8 per cent and 21.7 per cent of the patients were qualified as invalids. In the remote periods complete and clinical recovery was stated in 26.8 and 52.8 per cent of the patients respectively, 4 (4.1 per cent) patients died and 7.4 per cent of the patients previously engaged in manual work and 3.0 per cent of the patients previously engaged in mental work were qualified as permanent invalids.

Nineteen patients suffering from congenital heart diseases were examined by means of the apparatus Pneumoscreen manufactured by Eger Company (FRG) and the mechanocardiograph H-106 designed by N. I. Savitsky, which synchronously recorded the curves of the volumetric speed of the flow and intrathoracic pressure. Analysis was made of the relationship of the magnitude of the instant maximal velocity characteristics of forced expiration in all lung capacities and their time-course of changes to the total pulmonary resistance. It is revealed that the decrease of the maximal velocity characteristics of forced expiration in large pulmonary capacities was directly proportional to the degree of pulmonary tissue rigidity in patients with congenital heart diseases and corresponded to the degree of hemodynamic disorders in pulmonary circulation. The fall of the instant velocity of forced expiration in small lung capacities in patients with congenital heart diseases evidences not only the presence of bronchial obstruction but also high total pulmonary resistance.
