RESUMO
A emergência pré-hospitalar é uma temática atual e bastante debatida. Também a presença do enfermeiro na prestação de cuidados pré-hospitalares é considerada indispensável. Os enfermeiros dos Cuidados de Saúde Primários enfrentam por vezes situações de emergência nos seus serviços ou durante os domicílios que vão realizar. É sobre estes que nos vamos focar e procurar entender a importância que para eles reveste a emergência pré-hospitalar. É de extrema importância que estes enfermeiros tenham conhecimento atualizado da urgência/emergência e experiência prática nas diversas situações, para que se sintam à vontade nessas ocorrências e para que haja um apoio mútuo com a equipa multidisciplinar com que trabalham. Iremos estudar os enfermeiros pertencentes ao ACES Douro Sul, um ACES da zona Norte do país, que se encontram afastados dos grandes centros, uma vez que são estes profissionais os que mais necessitam de formação para poderem auxiliar os utentes sempre que estes se encontram em alguma situação de urgência/emergência. Para se ser enfermeiro pré-hospitalar é necessário um conjunto de requisitos que deverão ser tidos em conta antes de um profissional decidir optar por essa área pois é necessário um perfil profissional adequado. Com o presente Trabalho de Projeto pretende-se perceber como é que as equipas dos Cuidados de Saúde Primários se sentiam em relação à enfermagem pré-hospitalar e, se após doze anos do início da rede de requalificação das urgências se se consideram mais seguros e mais preparados para atender utentes urgentes. Para tal, foi feita uma pesquisa bibliográfica para a componente teórica que servirá de suporte para posterior realização de uma investigação.
Prehospital emergency is a current and widely discussed topic. Also, the presence of nurses in the provision of prehospital care is considered essential. Primary Health Care nurses sometimes face emergency situations in their services or during the home care support they provide. It is on these that we will focus and seek to understand the importance that for them the prehospital emergency has. It is extremely important that these nurses have urgency/emergency up-to-date knowledge and practical experience in various situations, so that they feel comfortable in these occurrences and so that there is mutual support with the multidisciplinary team with whom they work. We will study nurses who work for ACES Douro Sul, an ACES located in the northern part of the country, who are far from big cities, as these professionals are the ones who most need training to be able to assist patients whenever they are in a urgency/emergency situation. To be a prehospital nurse a set of requirements is necessary which must be taken into account before a professional decides to choose this area, as an appropriate professional profile is required. The aim of this Project Work is to understand how Primary Health Care teams felt about prehospital nursing and, if, 12 years after the beginning of the emergency redefinition network, they consider themselves more confident and well-prepared to attend urgent patients. To this end, a bibliographical research was carried out for the theoretical component which will support further investigation.

The coronavirus disease (COVID-19) caused by SARS-CoV-2 is creating tremendous health problems and economical challenges for mankind. To date, no effective drug is available to directly treat the disease and prevent virus spreading. In a search for a drug against COVID-19, we have performed a massive X-ray crystallographic screen of two repurposing drug libraries against the SARS-CoV-2 main protease (Mpro), which is essential for the virus replication and, thus, a potent drug target. In contrast to commonly applied X-ray fragment screening experiments with molecules of low complexity, our screen tested already approved drugs and drugs in clinical trials. From the three-dimensional protein structures, we identified 37 compounds binding to Mpro. In subsequent cell-based viral reduction assays, one peptidomimetic and five non-peptidic compounds showed antiviral activity at non-toxic concentrations. We identified two allosteric binding sites representing attractive targets for drug development against SARS-CoV-2.

Objetivo: avaliar instituição hospitalar quanto à implantação das práticas seguras. Método: estudo descritivo e observacional, realizado em 2019, em hospital público do nordeste do Brasil. Utilizou-se o questionário Avaliação da Gestão de Riscos Assistenciais em Serviços de Saúde (AGRASS), com 40 itens, e roteiro para a observação não participante. Amostra foi constituída por um gestor de risco, três farmacêuticos, quatro enfermeiros, nove técnicos de enfermagem e dois de farmácia. Resultados: na avaliação do grau de implantação da gestão de riscos, foi obtido um total de 22 itens com respostas positivas, indicando a completa adesão desta prática em mais de 50%. Na observação de campo, encontraram-se fragilidades na maioria dos achados, destacando-se a higienização das mãos e prescrição, uso e administração de medicamentos. Conclusão: as potencialidades e fragilidades referentes às práticas observadas exigem ações educativas contínuas e melhoria da gestão de riscos para alcance pleno da cultura de segurança.
Objective: to evaluate a hospital institution regarding the implementation of safe practices. Method: descriptive and observational study, conducted in 2019, in a public hospital in northeastern Brazil. A 40-item Health Care Risk Management Assessment (AGRASS) questionnaire was used and a script for non-participant observation. Sample consisted of one risk manager, three pharmacists, four nurses, nine nursing technicians and two pharmacy technicians. Results: in the assessment of the degree of implementation of risk management, a total of 22 items with positive answers were obtained, indicating the complete adherence of this practice by more than 50%. In the field observation, weaknesses were found in most findings, especially hand hygiene and prescription, use and administration of medications. Conclusion: the potentialities and weaknesses regarding the observed practices require continuous educational actions and improvement of risk management to fully reach the safety culture.
Objetivo: evaluar una institución hospitalaria con respecto a la implementación de prácticas seguras. Método: estudio descriptivo y observacional, realizado en 2019, en un hospital público del noreste de Brasil. Se utilizó un cuestionario de evaluación de gestión de riesgos de atención médica (AGRASS) de 40 ítems y un guión para la observación no participante. La muestra consistió en un gerente de riesgos, tres farmacéuticos, cuatro enfermeras, nueve técnicos de enfermería y dos técnicos de farmacia. Resultados: en la evaluación del grado de implementación de la gestión de riesgos, se obtuvieron un total de 22 ítems con respuestas positivas, lo que indica la adherencia completa de esta práctica en más del 50%. En la observación de campo, se encontraron debilidades en la mayoría de los hallazgos, especialmente la higiene de manos y la prescripción, el uso y la administración de medicamentos. Conclusión: las potencialidades y debilidades con respecto a las prácticas observadas requieren acciones educativas continuas y la mejora de la gestión de riesgos para alcanzar plenamente la cultura de seguridad.

A previously healthy 13-year-old girl presented with a 9-day history of acute onset severe neck pain associated with limited range of movement. Medical evaluation at day 2 was suggestive of muscle contracture, and she was discharged home with diazepam, antiinflammatory agents, and rest; however, she returned because of progressive clinical worsening with left arm distal paresthesia and paralysis since day 3. There was no history of trauma or other systemic complaints, and her familial medical history was unremarkable.Physical examination revealed left cervical and paravertebral tenderness on palpation with severe limitation of cervical and trunk movements; neurologic examination revealed left forearm and hand weakness and paralysis (grade II/V) with thenarhypothenar atrophy. Laboratory studies including coagulation profile were normal. Magnetic resonance imaging revealed an epidural hematoma from C4-T1 without underlying cause apparent on magnetic resonance angiography. On day 12, she underwent C3-7 laminotomy with laminoplasty and complete drainage of the hematoma. After 5 months of follow-up, she displays no neurological deficits. The spontaneous spinal epidural hematoma is a rare neurosurgical emergency in children. It usually presents acutely with neurologic deficits, but the initial presentation may be atypical or insidious, delaying diagnosis and intervention. Definitive diagnosis is made by magnetic resonance imaging and implies a high index of suspicion. Surgical drainage of the hematoma is the mainstay of treatment with favorable prognosis even in cases with a delayed diagnosis.
