Splenic contusion was revealed in 12% of the sufferers with splenic trauma. The main methods for diagnosis of contusion are computerized tomography and angiography. The treatment is conservative. The operation is indicated in development of complications: splenic abscess, posttraumatic thrombophlebitic splenomegaly, intrasplenic hematoma.

Posttraumatic splenolithiasis was diagnosed in 33 patients, late posttraumatic hematoma in 4, posttraumatic splenosis in 4, and posttraumatic thrombophlebitic splenomegaly in 8 patients. The diagnosis of the sequelae of injury to the spleen is based on the case history and results of ultrasonic examination, computerized tomography, scintigraphy, laparoscopy, and angiography. Complicated splenomegaly and hypersplenism are indications for operative treatment of the sequelae of trauma of the spleen. Splenectomy is the operation of choice. The results of puncture methods of treatment and organ-preserving operations are to be studied.

Splenectomy still remains the method of choice in treatment of patients with hairy cell leukemia (HCL) due to high efficiency and low percentage of complications. Five-year survival of patients with HCL after splenectomy was 74%. Because of the low percent of hairy cells in the patient's blood the diagnosis of HCL should widely include trephine biopsy of the bone marrow as well as immunological studies of the leukemic cells. In order to detect pathological processes in the spleen and estimate the topographo-anatomical alterations in the left quadrant of the abdomen before operation, it is expedient to use computed tomography, ultrasound examination, scintigraphy.

Seven patients with pseudocysts of the spleen were treated in the clinic of the 2nd chair of surgical diseases of the Riga Medical Institute. The cysts in all of them lacked characteristic signs and were recognized late, when they had grown to a large size. Ultrasonic examination and computerized tomography played the main role in establishing the diagnosis. The pseudocysts of the spleen were treated by surgery. In large cysts splenectomy was the operation of choice, though organ-preserving operations (enucleation of the cyst, resection of the spleen) were suggested.

Eight patients with abscesses of the spleen were observed. The clinical picture had no special characteristics. Splenomegaly was not a pathognomonic symptom of the spleen abscess. The ultrasonic investigation and computed tomography were the main methods of diagnostics. The timely treatment of the spleen abscesses gives satisfactory results. The operation of choice is thought to be splenectomy.

The results of 68 combined operations for lung cancer and sarcoma are presented. Postoperative complications occurred in 42.7% and lethality rate was 16.2%. Total survival was 41% at 1 year, 20%--3 years and 9.9% at 5 years. The results of combined surgery proved worse than those of surgical treatment of lung cancer. However, tracheobronchial combined surgery produced better results than vascular-atrial or parietal-diaphragmatic types.

Effect of vitamin A on some patterns of specific and unspecific immunity was studied in patients with chronic pneumonia. The vitamin was administered into the patients during 7 days at a daily dose of 500,000 IU. Administration of vitamin A was found to stimulate several immunological patterns: content of T- and B lymphocytes in peripheral circulation as well as IgG and IgM blood serum were increased. After the vitamin A treatment the cellular immunity was also stimulated, involving the activation of lymphocyte blast transformation with PHA and the increase in exhibition of skin reaction towards PHA. In response to vitamin A administration activation was observed of unspecific bactericidal, and complement activities in blood serum as well as of the myeloperoxidase activity in leukocytes of peripheral circulation. Administration of vitamin A into the control patients within a week at a daily dose of 10,000 IU did not alter the immunological patterns studied.
