RESUMO

AIM: To analyze management and outcomes in patients with CHF managed by specialists in heart failure (HF) or general cardiologists/physicians in real-life clinical practice. MATERIALS AND METHODS: Survival rate, rehospitalization rate, general health condition, and the administered therapy were evaluated for HF patients with reduced LV ejection fraction at three years of discharge from cardiological hospitals. These patients had been included in a prospective, multicenter, observational study, "The Russian Hospital HF Registry" (RUS-HFR). The first group consisted of patients who were managed at a specialized HF department of the Federal Center and followed up at the outpatient stage by a cardiologist specializing in HF (Group 1, St.­Petersburg; n =74). The other two groups (Group 2 and Group 3) included patients who were managed at other cardiological departments of the Federal Center (n=186) or the Regional Center (n=130) and subsequently followed up at the place of residence. RESULTS: After the discharge from the hospital, 58-95 and 12-19% of RUS-HFR patients were followed up by a cardiologist or a physician, respectively, on an outpatient basis while 5-23% of patients did not visit a doctor at all. In three years, the survival rate of Group 1, 2, and 3 patients was 80 vs. 78 (р>0.05) vs. 52% (р0.05) vs. 100% (p1,2.

Symbiotic nitrogen-fixing bacteria Rhizobium leguminosarum by. viciae VF39 secrete an acidic heteropolysaccharide, the biosynthesis of which involves the stage of polyprenyl diphosphate octasaccharide formation, with its carbohydrate fragment corresponding to the repeating polymer unit. The amino acid analysis of the product of the pssA gene, we have earlier identified, showed its homology to bacterial polyisoprenyl phosphate hexose 1-phosphate transferases catalyzing the formation of phosphodiester bonds between polyprenyl phosphates and hexose 1-phosphates, whose donors are nucleotide sugars. The immunoblotting demonstrated that Rhizobium cells synthesize a protein with a molecular mass of 25 kDa, which implies the translation of the open reading frame occurring from the second initiating codon followed by the protein processing. It was shown that PssA is an integral membrane-bound protein involved in glucose 1-phosphate transfer from UDP-glucose to polyprenyl phosphate to form polyprenyl diphosphate glucose. These results suggest that the pssA gene encodes UDP-glucose:polyprenyl phosphate-glucosyl phosphotransferase.

RESUMO

Effectiveness of some physical therapeutic factors (constant magnetic field, impulse currents) and new hypotensive drugs (tobanum, prinorm, ormidol, minipress, arifon, arilix) was compared in the treatment of essential hypertension stage II. It is suggested that nonpharmaceutical therapy can regulate functions, correct hemodynamic and microcirculatory disorders, produce therapeutic effect without side effects typical for drugs.

RESUMO

The effect of so-called dry immersion on hypertensive patients during hypertensive crisis was investigated. It was demonstrated that after 90-minute immersion blood pressure fell due to beneficial hemodynamic and hormonal changes. This therapeutic method produced a sedative and diuretic effect. The above method of non-drug therapy proves highly effective in the treatment of hypertensive crises of different types, produced no side effects and can be used in a hospital as a cost-effective procedure.

RESUMO

Assessment of the initial data on hemodynamics in patients with stage II essential hypertension made it possible to differentiate indications for the use of some new hypotensive drugs. In the treatment of patients with stage II essential hypertension, preference should be given to arelix and minipress in case the total vascular resistance is initially high, whereas in hypertension of ejection, it is better in long-term treatment of patients with stage II essential hypertension, that the drugs producing an effect on the stroke volume of the heart, namely arifon, and tobanum, may be applied. All the drugs enumerated exert a negligible effect on the microvascular blood flow.

RESUMO

A study was made of the influence of the constant MKM2-1 magnets on patients suffering from essential hypertension. Continuous action of the magnetic field, created by such magnets, on the patients with stage II essential hypertension was noted to result in a decrease of arterial pressure without the occurrence of any side effects and in a simultaneous reduction of the scope of drug administration. Apart from that fact, magnetotherapy was discovered to produce a beneficial effect on the central hemodynamics and microcirculation. The use of the MKM2-1 magnets may be regarded as a feasible method of the treatment of essential hypertension patients at any medical institution.

RESUMO

A new method, "dry" therapeutic immersion, was used to treat hypertonic crisis in 26 patients suffering from stage II essential hypertension. The method turned out effective in removal of crisis both with hyperkinetic and hypo- and eukinetic types of hemodynamics. The use of "dry" therapeutic immersion gave rise to the sedative and diuretic effects. The given non-chemotherapeutic method was found to produce no side effects.

RESUMO

Investigation of the hypotensive action of minipress, a drug belonging to the group of selective blockers of alpha 1-adrenoreceptors, shows its efficacy in 56.3 percent of patients with stage II essential hypertension. The side-effects were recorded in 16.6 percent of the patients. It is noted that application of minipress at a mean daily dose of 3.4 ml for a long time reduces the total peripheral vascular resistance without inducing any substantial changes in the stroke volume of the heart or in the pulse rate. At the same time minipress produces a beneficial effect on the microcirculatory blood flow.

This paper presents a review of reported data about physiological responses of the human body to hypokinesia and discusses physiological, particularly hemodynamic, effects of dry immersion. The paper also describes clinical applications of the method and emphasizes the need for further study of the effects of dry immersion on normal and pathological processes accompanied by hemodynamic changes.
