RESUMO

The article presents the results of endovascular treatment of 35 patients with Budd-Chiari syndrome. An analysis of efficacy of modern interventions was made. There were shown the. possibilities of balloon dilatation of stenosis of renal veins, the balloon dilatation and stenting with metal self-expansible stents of stenosis an occlusion of intrahepatic section of the postcava and results of transhepatic portsystemic bypass surgery.

RESUMO

Demonstration of possibilities of an effective application of endovascular techniques in the diagnosis and treatment of pancreatic inflammatory diseases. Group N1 - 41 patients with severe acute pancreatitis, whose complex treatment was complemented with catheterization of the celiac trunk and subsequent regional intra-arterial drug perfusion of the pancreas and spiral celiac angiography. Group N2 - 18 patients with acute and chronic pancreatitis in whom endovascular surgery was performed with the aim of diagnosis and arresting of arrosive haemorrhage. Regional intra-arterial perfusion of the pancreas is effective as a method of prevention of septic complications, progression of pancreatic necrosis in patients with aseptic forms of severe pancreatitis, in case of infected necrotizing pancreatitis significant advantage of this method over the standard multimodality treatment was observed. Effectiveness of endovascular surgery in the diagnosis and haemostasis of arrosive arterial haemorrhage is showed. The results of this study suggest that the effective use of endovascular surgery as possible in patients with severe acute pancreatitis and late postnecrotic complications.

RESUMO

The authors suggest an original method of endobiliary stenting with laparoscopic assistance for reduction of obstructive jaundice in patients with non-operative tumors of hepatopancreaduodenal zone in the case of impossibility of performance of percutaneous or endoscopic decompression of the bile ducts. A recanalization of tumor stricture was carried out with the aid of laparoscopic assistance. A self-expandable metal stent with polymeric covering was implanted in the bile ducts. The proposed method was applied in 5 cases. An adequate decompression of the bile ducts was noted in all cases. The patient's condition was good after surgery. There weren't any cases of serious complications or hospital lethality.

RESUMO

The article presents the results of surgical treatment of 58 patients with pancreatic cysts. An analysis of efficiency of various current miniinvasive surgical methods of treatment is given. Criteria of differential approach to the choice of the kind of miniinvasive intervention in pancreatic and parapancreatic cysts were formulated.

RESUMO

An analysis of results of magnetic resonance tomography (MRT) of the stomach of 80 patients has shown that a complex examination with morphological verification had diagnosed gastric cancer in 60 of them. The technique of fulfilling MRT of the stomach was optimized, the main symptoms of gastric cancer were specified. The sensitivity of preoperative assessment of spread of gastric cancer on MRT was 82%, specificity -78%. The sensitivity of detection of the lymph nodes infiltrated with the tumor in MRT was 82%, specificity--84%. The general diagnostic informative value of MRT in gastric cancer was as follows: sensitivity--93%, specificity--86.6%, accuracy--92%.

RESUMO

An analysis of 24 cases with tumors of the small intestine of both benign and malignant origin has been made. Their pathomorphological characteristics are given. Acute intestinal obstruction, hemorrhage and perifocal inflammation are found to be most frequent complications. The topical diagnostics of these tumors before operation is difficult, they manifest themselves by their complications. The operative methods are dependent on intraoperative findings.
