RESUMO

A 32-year-old man, complaining of an abdominal mass, was admitted to hospital where, on undergoing examination, the mass, which had spread widely through the abdominal cavity and the retroperitoneal space, was diagnosed as a malignant schwannoma with a liver metastasis. Thus, a multidisciplinary treatment was initiated and a remission was achieved. Later, however, he developed a multiple, local recurrence and died forty months after onset of his clinical symptoms. Since the prognosis of patients with a malignant schwannoma that do not undergo radical surgery is extremely poor, the authors emphasize the necessity of research for an effective adjuvant therapy.

By means of pancreatic and biliary duct drainage in two pancreato-duodenectomized patients, excretion of CDDP into pancreatic and hepatic bile was studied. CDDP was infused at a dose of 50 mg/m2 for 40-60 min. Platinum was determined by the flameless atomic absorption spectrometric method. CDDP was excreted into pancreatic juice in the form of filterable platinum over a short duration and in a small amount. The excretion gradient into pancreatic juice was around 20%. Total output of CDDP in pancreatic juice was estimated to be 0.1% of administered platinum. Although no new observations on the urinary and biliary excretion of CDDP were obtained, it was noticed that platinum was excreted in filterable form in both urine and bile, and that the excretion gradients in each were approximately 300% and 35%, respectively, in the present study.

A 57-year-old woman was referred to our hospital because of epigastric tumor on Jan. 16, 1980. Two advanced gastric carcinomas were recognized in the antrum and the corpus by X-ray and endoscopic examinations. The biopsy diagnosis was poorly differentiated adenocarcinoma. She was treated with FT-207 suppositories and subcutaneous injection of OK-432 without operation because of a suspicion of ascites and Virchow's metastasis. After five months, the tumor of the corpus disappeared, and, after nine months, that of the antrum changed to a scarlike lesion. On July 30, 1981, total gastrectomy with extended nodal dissection was performed. Histological examination revealed complete absence of carcinoma cells in the stomach and regional lymph nodes.

A previous study by the authors has made it clear that regional arterial infusion of Adriamycin (ADM) increases the drug concentration in the gastric tissue and the regional lymph nodes. In order to obtain further augmentation of the ADM concentration, simultaneous arterial infusion of ADM and Urokinase (UK), as a lysosome labilizer, was tried in the present study. ADM and UK were infused in three different ways through the left gastric artery on laparotomy, that is, a mixture of both in the first group, ADM followed by UK in the second group and ADM after UK in the third group. No augmentation of tissue ADM was obtained by any of these three methods and ADM concentration in gastric tumors, gastric wall and regional lymph nodes was rather lower than that using ADM only. A similar tendency was observed in the adjacent organs; liver, pancreas and spleen. Furthermore, the three-year survival rates of these groups were not statistically significant against the control group treated with ADM only.

A total of thirteen patients with histologically-proved adenocarcinoma of stomach took part in this study at Saitama Cancer Center between July 1982 and September 1983; nine patients were considered evaluable. There were 7 male and 2 female patients with a median age of 52 yrs (range 42-75) and a median performance status of 3 (range 2-4). Patients were treated with a two drug combination of cis-diammine dichloroplatinum (II) and fluoropyrimidine derivatives (tegafur, cormobur ). Three out of 9 evaluable patients (33.3%) had a partial response which lasted 10 months. 6+ months, and 5+ months, respectively. The major toxic effect was a gastrointestinal symptom which was generally manageable. Myelosuppression was mild.

As a fundamental investigation to prevent from the postoperative recurrence of gastric cancer, the concentrations of adriamycin(ADM) in the gastric tumor, gastric wall and regional nodes were measured after an arterial infusion of 5 or 10 mg of ADM. The drug was infused through either left or right gastric artery or the right gastroepiploic artery at the beginning of operation. The ADM concentration in gastric tumor and gastric wall were moderately elevated after the infusion, especially in the tissue at the area of arterial supply used for the infusion. No significant correlation was found among ADM concentration in the tumor, its histological classification and its amount of interstitial connective tissue. However, the marked elevation of ADM level was confirmed in the perigastric nodes along the drug-infused artery. The moderate elevation was also observed in the 2nd or 3rd group of the regional lymph nodes. As for the drug concentration in the adjacent organs, ADM was detected in slight high level in the liver, less in the spleen and pancreas. From the results obtained, a possibility of prevention from the recurrence of gastric cancer was discussed.
