RESUMO

The study group were persons with risk factors of colonization by pathogenic strains and included smokers, patients suffering from paradontosis, and patients with visibly neglected oral cavity and teeth. We isolated and classified to the species or genera 488 microorganisms. Of posterior pharyngeal wall flora, 61% of were Gram-negative bacteria, represented predominantly by Haemophilus and anaerobic rods and aerobic cocci belonging to the Neisseria and Moraxella (Branhamella) genera. In the group of Gram-positive cocci (34% of the total number of microorganisms), oral streptococci, Stomatococcus mucilaginosus and Streptococcus pneumoniae were isolated most frequently. The affected by neoplastic lesions larynx, was colonized by similar bacterial groups. However, the incidence of Gram-positive cocci was higher. The main etiologic factor of purulent post-operative wound inflammations were methicillin-resistant Staphylococcus aureus strains (MRSA), which had been absent among the bacteria isolated from patients on admission.

The bacterial flora of the posterior laryngeal wall, larynx and surgical wounds of the patients treated surgically for carcinoma of the larynx was investigated. A similarity of some bacterial strains isolated from various habitats was demonstrated by comparing the biochemical characteristics, drug resistance patterns, and susceptibility to bacteriocins and bacteriophages. Bacteriophagotyping confirmed the similarity of rods belonging to the genera Enterobacter and Pseudomonas. Thus, colonization of surgical wounds by bacteria present in the pharynx and the larynx, but not inducing healing complications, has been confirmed. The detected strains demonstrated high susceptibility to antibiotics, including those administered within the framework of post-operative preventive treatment.

The aim of the study was objective assessment of the results of betahistyne treatment in patients with chronic insufficiency of the vertebrobasilar artery system by means of elektronystagmography (ENG). The patients had mostly vertigo and/or dizziness, headache, ear buzzing and disequilibrium sensations. In 46 patients clinical, audiological and ENG examinations were carried out. All symptoms decreased in frequency after the treatment. Vertigo and/or dizziness improvement was noted in 58.5% patients of cases, headache decreased in 56.5% of cases, ear buzzing decreased in 42.8% and hearing improved in 22.2%. Before treatment 76% of patients had pathologic changes in ENG, after treatment improvement of ENG was noted in all tests, particularly in 17 cases (48.6%). This was most frequent in spontaneous nystagmus (83%), positional test (85%), caloric test (83%), and optokinetic test (71%). Comparison of complaints and ENG findings before and after treatment showed improvement both in symptoms and signs. Betaserc was effective in the treatment of patients with vertebrobasilar system insufficiency both in subjective opinions of patients and in objective assessment by ENG.

Angiogenesis as multistage process, which is required for forming of new vessels and is essential for the growth of solid tumors and making metastasis. Sixty patients with laryngeal cancer were analyzed. They were treated in ENT Department of Medical University in Lodz and follow-up period was not shorter than 5 years. Intensity of neovascularisation was examined by means of immunohistochemical techniques carried out on paraffin sections using selected antibody against antigens of vessel cells. The value of the prognostic factors of angiogenesis intensity were evaluated against: dissected nodal metastasis, tumor recurrences and metastasis observed in prospective follow-up and after cancer treatment live time. Morphological properities of tumor or hospital treatment of the patients with laryngeal cancer were analyzed as well. Taking into consideration the results, we can make the conclusion that investigation of angiogenesis can be forthcoming prognostic factor for the laryngeal cancer.

Fifty-eight patients with laryngeal cancer from ENT Department in Lodz operated because of neoplastic recurrences after radio-therapy or associated therapy (surgery + radio-therapy) were analyzed. Postradiation oedema of the larynx was the most common cause of failure and delay in making diagnosis of neoplastic recurrences after primary treatment. The most common pooperative complication were cutaneous-pharyngeal and cutaneous-oesophageal fistules (24 patients--41%). In 41 patients (70.1%) long healing after operation was observed. In 8 patients (13%) recurrences in oesophagus, in pooperative scar and in stomal region were observed. Five year follow-up was observed in 54% and 3 year follow-up in 39%.

RESUMO

Finding of the a etiologic factors and participation of bacteria flora in wound healing in laryngeal cancer treatment was the purpose of our study. Investigations were performed in 27 patients. Swabs were taken from the postoperative wounds. Detailed identifications of the bacteria flora and antibiotic susceptibility of bacteria were performed. Wound healing was estimated according to extension of the carcinoma, applied antibiotics, state of the oral cavity, the kind of bacteriological flora isolated after surgical treatment from postoperative wounds. It was found that wound healing depended on the extension of carcinoma, as well as, type of isolated bacteria and antibiotic therapy used. The proper healing of postoperative wounds was not dependent on the state of the oral cavity and the dentition. The main cause of postoperative complication of wounds was methicillin-resistant Staphylococcus aureus (MRSA).

All over the world an increase in the number of subjects addicted to various chemical substances has been observed. The studies of their toxic effect on the hearing have been so far most frequently limited to acute intoxication or direct effect: there are no data concerning distant and persistent effects. The aim of the study was to determine the type of hearing loss in drug addicts and individuals drinking non-consumable alcohols, examined during the period of abstinence. A group of 210 subjects addicted to various substances was examined using acumetry test and tuning forks, as well as threshold, suprathreshold and speech audiometry. Taking into consideration the threshold results for particular subjects, the percentage of hearing loss for both ears was calculated using CPT index. Most frequent complaints connected with the organ of hearing, such as tinnitus and hearing impairment were reported by alcoholics and opioid addicts. In the age adjusted threshold audiometry, perceptive hearing loss was diagnosed in nearly 50% of the subjects. The percentage of hearing loss calculated for both ears, using CPT index, demonstrated the highest values in the group of subjects drinking non-consumable alcohols and addicted to volatile organic substances.

RESUMO

The study was carried out on 12 surgically treated patients with carcinoma laryngis. Isolation and identification of oral cavity, pharynx, larynx and postoperative neck wound microflora were performed. Failures in neck wounds healing resulting from bacterial flora were compared. Sources of wound infection were found due to biochemical features and antibiotics sensibility.

RESUMO

We presented the case report of a postoperative maxillary cyst, which developed 26 years after the radical Caldwel-Luc procedure in the maxillary sinus. This is a rare case in nonoriental population but it has been described well in Japanese literature. Considering the size and damage found in clinical and radiological examination, we suspected a malignance lesion.

In this method perpendicular scores of the skin are performed and special external sutures are made which stretch tracheostomy foramen to order to obtain wide inside measurement of wind-pipe. 78% operated patients may be without tracheotomy tube.

RESUMO

In clinical practice there are no sufficient histopathological and immunohistochemical parameters to predicting progression of laryngeal cancer. We applied the new quantitative Anertoha, Blatsakisa and Luny cancer classification and expression of PCNA (proliferating cell nuclear antigen) to clinical findings to appreciate prognosis in laryngeal cancer.

The study was carried out on 223 alcoholics, 96 drug addicts and 50 healthy persons as a control group. Clinical otolaryngological examination, ENG examination, and audiometry diagnosis were made. The results showed lesions of the central part of the vestibular system in alcohol and drug addicts which are much more evident on ENG examination than in clinical otolaryngological findings. We have observed that trauma capitis does not cause increased vestibular disturbances in addicted persons. Audiologic findings indicate significant hearing loss in both groups.
