RESUMO

Interpretation of topographical maps used to detect corneal ectasias requires a high level of expertise. Several artificial intelligence (AI) technologies have attempted to interpret topographic maps. The purpose of this study is to provide a review of AI algorithms in corneal topography from the perspectives of an eye care professional, a biomedical engineer, and a data scientist. A systematic literature review using Web of Science, Pubmed, and Google Scholar was performed from 2010 to 2020 on themes regarding imaging modalities, their parameters, purpose, and conclusions and their samples and performance related to AI in corneal topography. We provide a comprehensive summary of advances in corneal imaging and its applications in AI. Combined metrics from the Dual Scheimpflug and Placido device could be a good starting point to try AI models in corneal imaging systems. The range of area under the receiving operating curve for AI in keratoconus detection and classification was from 0.87 to 1, sensitivity was from 0.89 to 1, and specificity was from 0.82 to 1. A combination of different types of AI applications to corneal ectasia diagnosis is recommended.

PURPOSE: The purpose of this study was to report the adverse effects of the COVID-19 pandemic on the clinical outcomes of infectious keratitis in South India. METHODS: Two hundred fifty-eight patients presented with infectious keratitis at 6 tertiary care centers when strict travel restrictions were in place from March 24 to May 31, 2020. Case records were collected retrospectively and analyzed for demographics, type of initial treatment, length of delay in presentation, microbiological diagnosis, clinical course, and final treatment outcome. RESULTS: The mean age of the patients was 49.2 years, with most of them (61.2%) being male. Forty-one patients (15.9%) did not receive any treatment for at least a week after the onset of symptoms. One hundred eight patients (41.9%) presented with severe ulcer (32 had a frank perforation). Resolution of the infection was achieved only in 103 patients (45.6%). A total of 90 patients (39.8%) with ulcers had a perforation, yet only 29 patients (32.2%) could receive keratoplasty because of the unavailability of donor tissues. At the end of 6-month follow-up, 47 patients had anatomical failure (loss of globe) and 12 additional patients had functional failure (total permanent vision loss). CONCLUSIONS: Delay in presentation and acute shortage of donor corneal tissues for emergency keratoplasty because of the COVID-19 pandemic had a grave impact, resulting in irreversible blindness in a significant number of patients.

Purpose: There is limited information in literature about the scope and usability of Smartphone Applications (Apps) in ophthalmology. Eye care professionals are therefore skeptical about the benefits of smartphone Apps and are reluctant to adopt it in their day to day practice. The purpose of this study was to provide an updated overview of all Apps exclusive to ophthalmology. Methods: This study was a quantitative analysis of Smartphone Apps exclusively designed for ophthalmic care. The Apple iPhone and Google play store were searched for eye care themed Apps. Any App related to eye treatment and management such as visual acuity screening, eye education, calculators, eBooks, Low Vision Aids were included in the study. Data on the purpose of the Apps, target end-users, validation, App usage, user ratings, and App developer qualifications were documented. Results: As of March 2020, a total of 131 Apps exclusively dedicated for eye care were identified. 53% (69/131) were available in iPhones, 44% (58/131) in Android smartphones, and 3% in both. 32% (41/131) Apps were designed for visual acuity (VA) screening, 13% (17/131) for eye relaxation exercises, 12% (15/131) for professional education, and the remaining for Apps detecting color blindness, low vision aids, and assistance and patient education. Among the 131 Apps, 6 (4.5%) Apps are claimed by the developers as validated. Conclusion: This study unveils a wide range of smartphone Apps exclusive to ophthalmology and suggests guidelines to choose an appropriate App. The study also highlights the importance of interdisciplinary collaboration in the design, development, and validation of such Apps.

Purpose: With increasing environmental pollution, the incidence of allergic conjunctivitis is increasing. Newer anti-allergic medications with combined anti-histaminic and mast cell stabilization action can help reducing the use of topical steroids for milder form of disease. There is no study directly comparing olopatadine (0.1%), bepotastine (1.5%), and alcaftadine (0.25%) for mild to moderate allergic conjunctivitis cases. Hence, we decided to methodically study the efficacy of three topical medications. Methods: Prospective, observer-masked clinical trial enrolled 45 patients with 15 patients in each of the three groups. Patients with mild to moderate allergic conjunctivitis were sequentially assigned to respective groups, and relief of symptoms and signs were noted upto 1-month follow-up. Results: All three topical medications faired almost equally in resolving symptoms of the patients with mild to moderate allergic conjunctivitis, and most of them reported complete relief after 1 week of use of medication. Few cases with limbal or palpebral papillae reported symptomatic relief after use of medication, but the resolution of these signs was not noted in all three groups. Conclusion: We concluded similar efficacy of three medications in relieving symptoms and inefficacy in regressing palpebral and limbal papillae in cases of allergic conjunctivitis.
