RESUMO

PURPOSE OF THE STUDY The results of the prospective study evaluating the outcomes of a combined therapeutic approach of non-operative therapy of Groin Pain Syndrome (GPS) in professional football players are presented. MATERIAL AND METHODS The evaluated cohort consisted of a total of 31 football players of a professional football club divided into two groups. The Group 1 included 10 symptomatic patients suffering from GPS for at least 4 weeks, the Control Group 2 included 21 healthy football players not suffering from GPS. The symptomatic patients with GPS who had undergone any surgery or had sustained any major injury to their low back, groins, hip or knee joints were excluded from the study. It was a prospective non-randomized study. The results were evaluated using the HAGOS (Copenhagen Hip and Groin Outcome Score) questionnaire, which in Group 1 compared the results before the initiation of the new combined therapeutic approach of non-operative therapy developed by us and the results after the end of this treatment, i.e. after 6 weeks. Moreover, the HAGOS was used to compare the results of the Group 1 after the treatment and the Control Group 2, i.e. the healthy football players. The HAGOS evaluated six separate areas: Pain, Symptoms, Physical function in daily living, Physical function in sport and recreation, Participation in physical activities, and hip and/or groin-related Quality of Life. The resulting score for each domain ranges from 0 to 100 (100 = no problems, 0 = extreme problems). The results were statistically evaluated by an independent statistician. RESULTS In Group 1, after the end of the treatment a statistically significant improvement was observed in all the parameters assessed by HAGOS. Pain (70 before the treatment, 95 after the treatment), Symptoms (58.9 before/85.7 after), Physical function in daily living (70 before/100 after), Physical function in sport (46.9 before/90.6 after), Participation in physical activities (43.8 before/100 after), Quality of Life (66.3 before/95 after). When comparing the two groups, before the treatment a statistically significant difference was found in all the six parameters assessed by HAGOS, while after the treatment no significant difference between Group 1 and Group 2 was detected. DISCUSSION The most important result of our study is that by applying the new combined therapeutic approach of the GPS nonoperative therapy developed by us better HAGOS score were achieved in our cohort than those achieved by other types of non-operative therapy presented in recent literature. The approach proposed by us is more successful in terms of the speed of return to sport (6 weeks) than the multimodal therapy programme (12 weeks) and Training programme (18 weeks). CONCLUSIONS The results of this prospective study confirm that the Group 1 having undergone the 6-weeek long combined symptomatic non-operative therapy of GPS proposed by us showed a statistically significant improvement in all the parameters assessed by HAGOS and these values showed no statistically significant difference from those of the Control Group of healthy individuals. RESUMO

The authors report the case of a 19-year-old girl with osteolytic bone lesions in whom primary hyperparathyreoidism caused by parathyroid gland adenoma was diagnosed. They emphasize necessity to be aware of a relationship of this rare condition to bone lesions that may be benign in appearance and may show ambiguous histological findings. The patient underwent excision of the adenoma and subsequent substitution therapy, which resulted in the healing of bone lesions.

The susceptibility of sensitive yeast to killer toxins is known to depend on various factors, such as the selected killer toxin, the exposed yeast strain, its growth phase and the state of culture under given experimental conditions. The aim of this paper was to find whether individual cells from one culture are equally susceptible to the impact of the killer toxin. For this purpose the rhodamine B assay in a modified form was used. In order to observe the fate of individual cell the method of fluorescence video microscopy with a digital picture analysis was applied. Four selected groups of specific cells (with no, small, medium, and large bud, respectively) were investigated. Different sensitivity of Saccharomyces cerevisiae cells to the killer toxin K1 was observed in these cell groups. The most susceptible appeared to be the cells which were in S-phase (cells with the small buds); the least susceptible were the M-phase cells with large buds. The enhanced susceptibility in S-phase results probably from coincidence in higher porosity of the cell wall, accumulation of surface receptors, and enlarged growth activity at the surface cell structures.

The secondary structure of Na(+)/K(+)-ATPase after modification of the ATP-binding sites was analyzed. Consistently with recent reports, we found in trypsin-treated Na(+)/K(+)-ATPase additionally to alpha-helix also beta-sheet structures in the transmembrane segments. However, binding of fluorescein 5'-isothiocyanate (FITC), the pseudo-ATP analog, to the ATP-binding site did not affect the secondary structure of undigested Na(+)/K(+)-ATPase. Consequently, fluorescence intensity changes of FITC-labeled Na(+)/K(+)-ATPase commonly used to observe conformational transitions of the enzyme reflect physiological changes of the native structure. The metal complex analogues of ATP, Cr(H(2)O)(4)ATP and Co(NH(3))(4)ATP, on the other hand, affected the secondary structure of Na(+)/K(+)-ATPase. We propose that these changes in the secondary structure are responsible for inhibition of backdoor phosphorylation.

RESUMO

The case history presents a patient with a rare malignant bone tumor - leiomyosarcoma in the region of distal femur. A large resection of the tumor was made with a specially tailored distal femur replacement. The authors emphasize the necessity of a detailed histological examination including immuno-histochemistry which proved the diagnosis of leiomyosarcoma as opposed to the original diagnosis of fibrosarcoma. RESUMO

Two cases of low-grade fibromyxoid sarcoma are presented. In a 31-year-old male the tumour arouse in the scapular soft tissues and local recurrence occurred. In a female of 45, the neoplasm was located in the thigh. Histologically, the neoplasms with low cellularity of deceptively benign-appearing small fibroblastic spindle cells demonstrated alternating dense fibrous and loose myxoid areas, showing a mainly whorled and swirling growth pattern. In myxoid areas a prominent vascular component was present. Mitoses and cellular atypia were absent. Tumor cells showed staining with anti-vimentin and occasionally anti-actin antibodies. As a distinctive soft-tissue sarcoma, low-grade fibromyxoid sarcoma had to be distinguished from variety of benign and malignant soft tissue tumours such as neurofibroma, liposarcoma, myxoid MFH and others.

RESUMO

The authors evaluated the results of resections of musculoskeletal tumours in the area of the shoulder, operated in 1975-1993 at the First Orthopaedic Department in Brno. For evaluation they used Malawer's six grade classification and differentiated indications for different types of resections. The extent of resection depended on the biological nature of the tumour, its size and site. Key words: musculoskeletal tumours, schoulder girdle, resection.
