BACKGROUND: Negative symptoms of schizophrenia show limited response to both typical and atypical antipsychotics. Repetitive transcranial magnetic stimulation applied over the prefrontal cortex (PFC) has been proposed as an adjuvant to pharmacological treatment of negative symptoms in schizophrenia, but whether the improvements obtained are specific to negative symptoms or attributable to antidepressant effects is still unclear. OBJECTIVE: The aim of the present study is to determine to which extent the improvements in negative symptoms of schizophrenia obtained after high-frequency stimulation of the bilateral PFC using deep TMS (dTMS) are attributable to antidepressant effects. METHODS: Repetitive dTMS was administered to the PFC in a cohort of 16 patients with schizophrenia under successful pharmacological control of positive symptoms and predominant negative symptoms. Patients were treated using high-frequency (18 Hz) bilateral stimulation applied over the lateral PFC bilaterally using Brainsway H-2 coil. The effects of dTMS on negative symptoms were measured using the Scale for the Assessment of Negative Symptoms and the Positive and Negative Syndrome Scales. We then compared the improvements in negative symptoms obtained in patients showing depressive symptoms (≥7 points) with those found in patients without depression (>7 points), as determined by the Calgary Scale for Depression. RESULTS: Repetitive dTMS treatment induced significant improvements in negative symptoms as assessed using both Scale for the Assessment of Negative Symptoms and Positive and Negative Syndrome Scales. Comparison of the improvements obtained in patients with or without depression at the beginning of treatment revealed similar improvements in negative symptoms, irrespective of subjacent depression. CONCLUSIONS: Our data suggest that the beneficial effects of high frequency dTMS of the PFC cannot be attributed solely to its antidepressant effects.

Therapeutic options for psychiatric conditions are limited during pregnancy because many drugs are restricted or contraindicated. Electroconvulsive therapy constitutes an alternative, however there is controversy over its safety. Using the Epistemonikos database, which is maintained by searching multiple databases, we found five systematic reviews, including 81 studies overall describing case series or individual cases. Data were extracted from the identified reviews and summary tables of the results were prepared using the GRADE method. We concluded it is not clear what are the risks associated with electroconvulsive therapy during pregnancy because the certainty of the existing evidence is very low. Likewise, existing systematic reviews and international clinical guidelines differ in their conclusions and recommendations.

BACKGROUND: Delirium is an important problem especially in older medical inpatients. OBJECTIVE: The authors asked whether delirium and its duration are associated with higher mortality in a 3-month follow-up period. METHOD: In this prospective cohort study, inpatients age 65 and older were assessed every 48 hours with the Confusion Assessment Method. RESULTS: Of 542 patients enrolled, 192 (35.4%) developed delirium. After 3 months, mortality in the delirium cohort was 25.9%, and in the nondelirium cohort was 5.8%. Delirium was independently associated with mortality, and increased by 11% for every 48 hours of delirium. CONCLUSION: Delirium and increased delirium durations are significantly associated with higher mortality.

RESUMO

Psychotropic medications have a series of neurobiological effects which may be related to adverse effects. The endocrinological side effects may affect prolactin and thyroid, parathyroid and antidiuretic hormones. They may also influence the appearance of metabolic syndrome, gonadal and sexual problems. There disturbances must be borne in mind to prevent or detect them on time,since they may affect the compliance with the treatment. This revision focuses on the relationship between psychotropic drugs, antidepressants, mood stabilizers and gonadal function. As a general recommendation, patients using these medications should be monitored for menstrual and fertility disturbances, weight change, hirsutism, galactorrhea and changes in libido and sexual life.

El ambiente educacional es un concepto que cada vez cobra mayor relevancia en la educación médica por su impacto en el proceso de enseñanza, aprendizaje y posterior vida laboral. Existen numerosos instrumentos para evaluar el ambiente, según el ciclo de formación o el tipo de rotación. En la Escuela de Medicina de la Pontificia Universidad Católica de Chile se han utilizado las encuestas DREEM y PHEEM, que se complementan con otros instrumentos aplicados a los estudiantes, generados por el Centro de Educación Médica y la American Association of Medical Colleges (AAMC) en el contexto del proceso de la evaluación de la Escuela de Medicina. Todos estos confirman que el ambiente educacional en general es adecuado. Las fortalezas detectadas están relacionadas con el ambiente académico y la calidad de los profesores. Los aspectos deficitarios están relacionados con la retroalimentación, el proceso de evaluación y áreas de desarrollo social. La constante medición del ambiente educacional dentro de la Escuela de Medicina nos permitirá a futuro detectar las fortalezas, mejorar las áreas deficitarias y evaluar el impacto de las modificaciones curriculares.


The Educational Environment has increased its relevance in medical education due to the impact in the process of teaching and learning and later professional work. There are numerous instruments to evaluate the environment, according to the cycle of formation or the kind of rotation. In the Medical School of Pontificia Universidad Católica de Chile it has been used the DREEM and PHEEM questionnaires, which complement with other instruments applied to the students, developed by the Center of Medical Education and the American Association of Medical Colleges (AAMC) in the context of the School of Medicine evaluation. All these instruments confirm that the educational environment in general is good. The strengths are related to the academic atmosphere and the quality of teachers. The weaknesses are related to feedback, evaluation and areas related to social development. The measurement of the educational climate within the Medical School will allow in the future detecting the strengths, in order to improve the deficit areas and to evaluate the impact of curricular changes.
