RESUMO

Objective To investigate the clinical features and viral load of persons infected with HCV and the risk factors for severe outcomes.Methods Medical testing and questionnaire survey were conducted on 465 cases who were infected with HCV,20-30 years back.HCV RNA,alanine transaminase (ALT),aspartate transaminase (AST),albumin,globulin and bilirubin were tested for these subjects.Factors as demography,tobacco and alcohol consumption,SNP of rs7453920 and rs2856718 on HLA-DQ gene of subjects with HCV RNA,were analyzed by multiple logistic regression method to explore the risk factors for severe outcomes among the patients.Result Totally,465 subjects had symptoms as hypodynamic (15.70%,73/465),digestive system (17.63%,82/465),and arthrodynia (10.32%,48/465).HCV RNA was positive in 68.60% (319/465) of the subjects with median viral load as 76.01 × 104 copies/ml (min-max:592 copies/m1-1.08 × 1010 copies/ml).Totally,11.83% (55/465) of the cases appeared having liver inflammation by routine ultrasound exams.ALT and AST was seen higher than 80 (IU/L) in 12.70% (59/465) and 11.18% (52/465) of the subjects,separately.Factors as being male (OR=2.298,95%CI:1.247-4.238),GA genotype compared with AA type in rs2856718 (OR=1.716,95%CI:1.070-2.752),alcohol intake ≥7 times per-week (OR=2.966,95% CI:0.979-8.988) etc.,were independently related to HCV RNA sustained positivity.Factors as:being male (OR=1.694,95%CI:0.975-2.942),in 50-59 years age group (OR=2.414,95% CI:1.156-5.042),having other liver diseases (OR=2.592,95%CI:1.105-6.079) and carrying positive HCV RNA (OR=3.479,95%CI:1.648-7.343) etc.were independent risk factors for abnormal liver function.Conclusion High rates of carrying sustained positive HCV RNA and abnormal liver function appeared in subjects who got the HCV infection 20-30 years ago.Factors as being male,in old age,being frequent alcohol taker,GA genotype in rs2856718 and with other liver diseases etc.were related to higher risk for developing severe outcomes.

<p><b>OBJECTIVE</b>To investigate the hepatitis C virus (HCV) infection rate among general population in Jiangsu province and to analyze the main risk factors for HCV infection.</p><p><b>METHODS</b>Subjects in 3 counties were surveyed by stratified cluster random sampling in the National Major Science and Technology Projects demonstration area in Jiangsu province. Interview was carried out with uniformly- designed questionnaires. Blood samples were collected and anti-HCV tested. Data were analysed under SPSS 13.0. Case-control study was conducted on two groups with subjects under 1:4 matching by sex, age, village of residence and status of HCV infection. Cox's proportional hazards regression analysis was then performed to analyze the risk factors for HCV infection.</p><p><b>RESULTS</b>The positive rate of anti-HCV was higher in females (0.965%, 95%CI:0.899%-1.035%) than in males (0.572%, 95%CI:0.517%-0.632%). Significant differences were noticed among the positive rates of anti-HCV in the three counties (0.131%, 95%CI:0.103%-0.164%;0.316%, 95%CI: 0.268%-0.370%; 2.173% , 95% CI:2.039%-2.315% , respectively). Peak prevalence of anti-HCV (1.577%, 95%CI:1.425%-1.740%) was observed among persons at 50-59 years of age. Bottom rate (0.161%, 95%CI:0.125%-0.204%) was observed below 30 years of age. High anti-HCV positive rate was detected in people with lower education background or belonged to 'floating population'. Factors as having histories of hospitalization (OR = 3.049, 95% CI:1.322-7.036), blood transfusion (OR = 14.319, 95%CI:2.318-88.459) or sharing razor (OR = 3.604, 95%CI:1.096-11.851) were risk factors of HCV infection in the area with the lowest anti-HCV positive rate. In another county, factor as having histories of 'blood products transfusion' (OR = 7.202, 95% CI:1.170-44.310) appeared important while in the third county, having history of 'blood donation' (OR = 7.496, 95%CI:6.121- 9.180); 'blood transfusion' (OR = 2.305, 95%CI:1.578-3.369) and 'invasive physical examination' (OR = 1.258, 95% CI:1.021-1.549) appeared to be important. Age seemed a risk factor for HCV infection. "Sharing razor" was a specifically important risk factor among the 30- year-olds. "Having received acupuncture" was noticed a risk factor only among people at 50-59 years of age while "Having received invasive physical examination" was important in the 50-59 and 60-69 year-olds.</p><p><b>CONCLUSION</b>High anti-HCV positive rate was observed in people at middle of aged population, as well as in those with poor education or under 'floating'. Unsafe blood donation or having received blood-product transfusion might be the risk factors in the last decades. Factors as sharing razor, having received acupuncture or invasive examination might be specifically risky in different populations.</p>

<p><b>OBJECTIVE</b>To study the clinical features, pathologic findings and prognosis of patients with dysplastic nodules of liver (DN) and early hepatocellular carcinomas (eHCC).</p><p><b>METHODS</b>One hundred and forty-five archival cases previously diagnosed as DN or eHCC or well-differentiated HCC during the period from 2000 to 2009 were retrieved and reevaluated with the new diagnostic criteria by two experienced pathologists, according to International Consensus Group for Hepatocellular Neoplasia (ICGHN) 2008. Immunohistochemical study (EnVision method) for CD34, HSP70, glutamine synthetase, glypican 3 and Ki-67 was carried out. The original diagnosis and diagnosis after review were compared and correlated with the survival data of the patients, with statistical analysis.</p><p><b>RESULTS</b>With the new criteria, 16 cases were diagnosed as low-grade DN, 50 cases as high-grade DN, 72 cases as DN with microinvasion, 7 cases as advanced HCC. Slide review showed no diagnostic discrepancy in 112 cases (77.2%). Amongst the 33 (22.8%) underdiagnosed cases, there were 7 cases of advanced HCC initially diagnosed as DN or DN with microinvasion and 26 cases of eHCC initially diagnosed as high-grade DN. Kaplan-Meier analysis showed that the diagnosis of high-grade DN or early HCC carried no statistically significant difference in overall survival (P = 0.778, 0.677) or disease-free survival (P = 0.949, 0.700) in all patients and in patients with no history of HCC. The co-existence of advanced HCC in patients with DN or eHCC significantly correlated with overall survival (P = 0.004) but not with disease-free survival (P = 0.079).</p><p><b>CONCLUSIONS</b>The new diagnostic criteria by ICGHN 2008 are useful in delineating high-grade DN and eHCC. The overall survival and disease-free survival of patients with eHCC or high-grade DN undergoing hepatectomy show no statistically significant difference. Patients with DN or eHCC have better prognosis than patients with advanced HCC, though there is still a high risk of tumor recurrence.</p>

Objective To explore the diagnostic value of virtual touch tissue quantification (VTQ) for the assessment of liver fibrosis in patients with chronic hepatitis B.Methods VTQ was prospectively performed in 181 patients with chronic hepatitis B and a shear wave velocity (Vs) indirectly reflecting liver stiffness was obtained for each patient.With the gold standards of fibrotic stages and inflammation grades,the relationship between Vs,fibrotic stages and inflammation grades was evaluated.Results A relatively high correlation existed between Vs and liver fibrotic stages (r = 0.812,P < 0.001).Significant differences existed among groups with different fibrotic stages except for groups SO vs.S1 and S1 vs.S2 (P < 0.05).The areas under the ROC curve were 0.971,0.96l,0.969 and 0.908 for Vs predicting S≥S1,S≥S2,S≥S3 and S =S4 fibrosis respectively.Moderate correlation existed between Vs and inflammation grades (r =0.476,P =0.000).Conclusion Capable of staging liver fibrosis and preliminarily grading inflammation,VTQ is helpful for the early diagnosis of liver fibrosis and assessment of therapeutic follow-up and prognosis in patients with chronic hepatitis B.

Aim To study the effects of HiHi and HiLo on the lipid metabolism in rats through blood lipids and fatty acid oxidation in gastrocnemius of rats. Methods Thirty male SD rats were selected by means of adaptive training, and then divided into 3 groups randomly: living low-training low group (LoLo), living high-training high group (HiHi), and living high-training low group (HiLo). The rats ran on a treadmill 60 min a day at the speed of 35 m/min under normoxic condition or 30 m/min under hypoxic condition (13.6% of ambient FIO_2), 6 days a week for 4 weeks. The samples from blood and gastrocnemius were removed 24 hours after the last training by the end of 4-week experiment. TC, TG, HDL and LDL were tested with a full-automatic biochemical analyzer. LPL, leptin and AD were measured using ELISA. Real-time quantitative PCR was adopted to test the expression of PPARa and CPT-1 mRNA in rats' gastrocnemius. Results Compared to the LoLo, TC and TG decreased significantly (P0.05) and LPL and AD increased significantly (P<0.01) in HiHi.' PPARa mRNA and CPT-1 mRNA expressions in gastrocnemius were significantly higher in HiHi(P<0.05) than in LoLo, and significant descent of HDL(P<0.05) and CPT-1 mRNA expression(P<0.01) revealed in HiLo, as compared to the LoLo. Compared to the HiLo, HDL(P<0.05), and LPL, AD and CPT-1 mRNA expression(P<0.01) enhanced in HiHi. Conclusion (l) HiHi was more superior in regulation of blood lipids than LoLo due to increasing of LPL, and could facilitate fatty acid oxidation because of enhancement of CPT-1 mRNA, AD and PPARa mRNA. (2) Comparing to the normoxic training, HiLo had no beneficial effect on the blood lipid metabolism and decreased the fatty acid oxidation in gastrocnemius.(3 )HiHi was more superior in the effeet of HDL thah HiLo due to increasing of LPL, and could facilitate fatty acid oxidation because of enhancement of AD and CPT-1 mRNA.

Objective To find out more optimal surgical approach for glioblastoma located in sensofimotor cortex and language area. Methods A respective review of 27 cases of glioblastomas located in sensorimotor and language area were operated, via the transsylvian-opercular approach, ineluding outcomes of microneurosurgery between January 2005 to December 2007. Results Among of 27 cases, tumors in 21 cases were covered by opereular cotex and were totally resected, tumors protruded into sylvian and wrapped around middle cerebral artery in 6 cases. 3 of them were totally removed, others were subtotally. Neurofunction of 26 cases were preserved of one were injuried. Conclusion Transsylvian-opercnlar approach for resection of glioblastomas located in subcortex of sensorimotor and language area is more optimal surgical approach, which not only ensure tumor resection with great degree, but preserve neurofunction well.

Background and purpose:Angiofollicular lymphoid hyperplasia(Castleman's disease,CD) is a rare kind of abnormal disease with lymphoid hyperplasia. We analyzed the clinical data of 21 patients with pathologically-confirmed CD. Methods:Twenty-one cases,including 10 males and 11 females with a mean age of 38.6 years,with CD were collected from January 1971 to December 2007 in this study. The disease was divided into localized type(n=19) and disseminated type(n=2) . The clinical data,CT images and pathological and surgical results were restrospectively analyzed. Results:All cases were categorized as hyaline vascular type. In 19 cases with localized type of CD,the lesion presented as a solitary soft-tissue mass with a mean diameter of 5.2 cm,located at the left and right hilus of the lung(n=3 and 2 respectively) or in the interior lung(n=3) ,in the thoracic wall(n=2) or in the mediastinum(n=9) . Two cases of disseminated type were characterized by many groups of thoracic lymphadenopathy with a mean diameter of 2.3 cm,without other organ involvement. On plain CT scan,the mean CT value was 42 Hu. After contrast administration,obvious homogeneous enhancement was observed in arterial phase with CT value of 112 Hu. The preoperative misdiagnosis by CT was 90.5%. Conclusion:CD have no specifi c CT characteristic,its verifi cation is mainly based on pathologic examination. Surgical resection for localized type of CD is curative,but for disseminated CD,multidisciplinary therapies are recommended.

<p><b>BACKGROUND</b>To investigate the relationship between cytokine level and cancer cachexia on murine model, and to observe the effects of regular dose of indomethacin on cancer cachexia and survival of mice.</p><p><b>METHODS</b>C57 mice bearing Lewis lung carcinoma were used to establish murine cancer cachexia model. The serum cytokine levels (IL-1, IL-6, TNF-α and IFN-γ) and body weight of the mice were measured at different time points before and after treatment with daily intraperioneal injection of either indomethacin (1 mg/kg) or saline.</p><p><b>RESULTS</b>The mice in cancer cachexia group had significantly higher serum levels of IL-1, IL-6 and TNF-α ( P < 0.05) and lower body weight ( P < 0.05) than those in healthy control group. Compared to saline treatment, indomethacin intervention apparently down regulated the levels of IL-1, IL-6 and TNF-α ( P < 0.05 ), and remarkably prolonged the survival of mice ( P < 0.05). No significant difference in IFN-γ level was observed between cancer cachexia and healthy control groups ( P > 0.05), as well as between indomethacin and saline groups ( P > 0.05).</p><p><b>CONCLUSIONS</b>The results suggest that serum IL-1, IL-6 and TNF-α are possibly associated with cancer cachexia, however, IFN-γ seems to be irrelevant. Indomethacin may potentially ameliorate cancer cachexia through down-regulating levels of cytokines.</p>

Purpose:To evaluate the role of COX 2 in the carcinogenesis and progression of human colorectal neoplasms. Methods:The expression level of COX 2 in 122 colorectal neoplasm tissues(including 35 colorectal adenomas, 67 colorectal carcinoma and 23 colorectal cancer with synchoronous hepatic metastasis) was assayed by immunohistochemical methods. All specimens was analyzed by Conformation quantitative assay system, their stain strength was calculated.Results:Conformation quantitative assay showed the mean stain strength is 704.5 131.8 in colorectal adenoma, 1197.2 204.3 in colorectal cancer without hepatic metastasis and 1901.2 324.8 in colorectal cancer with synchoronous hepatic metastasis, there are significant differences among them statistically. According to the Dukes stages, mean stain strength is 1145.3 187.0 in B stage,1237.0?298.7 in C stage,1901.2 in D stage. There is statistic difference between Dukes stage D and B, C. Also our study indicated there was no relationship between age, sex, tumor location, tumor differentiation or tumor size with the level of COX 2 protein expression.Conclusions:In our test,higher COX 2 expression is seen in colorectal cancer than colorectal adenomas, colorectal cancer with synchoronous hepatic metastasis than colorectal cancer without hepatic metastasis,and Dukes D stage than the B?C. Thus COX 2 responses is important in the carcinogenesis and progression of colorectal cancer. NSAIDs or the others may play a role in the chemoprevention strategies of colorectal neoplasm as the COX 2 inhibitor.

Background and purpose:The skeleton is one of the most common sites of metastasis in patients with lung cancer.Bone metastases often cause poor prognosis.It is important to early detect bone metastases.Therefore,we planned a retrospective study to investigate the role of radionuclide bone scanning with 99mTc-MDP in detecting bone metastases in lung cancer patients.Methods:It was a retrospective analysis.400 patients with lung cancer diagnosed in our hospital from 2005.5 to 2007.5 were recruited.Using 99mTc-MDP bone scanning,we analyzed the symptoms,sites,numbers,pathological types and TNM stages of bone metastases.Results:Ostealgia was the most frequent symptom in lung cancer patients with bone metastases.The sites of bone metastases were:thoracic bones(41.52%),vertebras(32.31%),pelvic bones(15.89%),extremities(8.68%),skull(1.60%).126 patients had multiple bone metastases(80.25%).There was a statistical significance of bone metastases in adenocarcinoma patients(48.09%)compared with in squamous cell carcinoma patients(33.33%)(P
