RESUMO

Since the performance appraisal of national tertiary public hospitals was carried out, higher requirements have been put forward for the operation and management of hospitals. Under the premise of ensuring the quality of medical service and medical safety, how to save hospital operating costs and improve the efficiency is an urgent problem for hospital managers. Supported by information upgrading, a tertiary hospital in Guangzhou reformed the treatment process and carried out pre-hospitalization in surgical departments. Data showed that pre-hospitalization can significantly shorten the length of stay, reduce hospitalization costs, and improve the operation efficiency of the hospital.

RESUMO

Objective To explore the effects of 1,25-dihydroxyvitamin D3 on renal expression of TGFβ1,Smad3 and Smad7.Methods 40 Sprague-Dawley mts were randomized into 4 groups:normal control rats (group A),diabetic nephropathy group (group B),small dose 1,25-dihydroxyvitamin D treatment group (0.5 μg · kg-1 · d-1,group C) and large dose 1,25-dihydroxyvitamin D treatment group (1 μg · kg-1 · d-1,group D),each group had 10 rats.After 12 weeks,the renal function,blood glucose,glycosylated hemoglobin and the urine trace albumin content of each rats were tested.Results The biochemical indicators in group B were higher than those in group A,(t =-16.566,P <0.05;t =-16.949,P <0.05;t =-11.844,P <0.05;t =-19.778,P <0.05;t =-14.013,P < 0.05).Compared with group B,the biochemical indicators and the expression of TGFβ1,Smad3 mRNA reduced in group C and group D,and the expression of Smad7 mRNA increased (F =37.892,P < 0.05;F =70.068,P < 0.05;F =21.95,P <0.05;F =77.619,P <0.05;F =37.670,P <0.05;F =1062.562,P <0.05;F =2463.789,P <0.05;F =81.745,P < 0.05).There were no significant differences between group C and group D (t =0.538,P>0.05;t =1.737,P>0.05;t =0.671,P>0.05;t =1.763,P >0.05;t =0.997,P >0.05;t =1.653,P >0.05;t=1.543,P>0.05;t =-1.313,P >0.05).Conclusion 1,25-dihydroxyvitamnin D3 has protective effect on diabetic nephropathy rats model,the mechanism may be associated with inhibiting the expression of TGF β1 and Smad3,increasing the expression of Smad7.

RESUMO

BACKGROUND: Wounds that have been closed under excessive tension, and skin defects that cannot be closed primarily, pose a daily challenge for the reconstructive surgeon. OBJECTIVE: To evaluate a new tension relief system (TRS) device for skin stretching and secure wound closure. METHODS: From September 2013 to March 2014, a consecutive series of 41 Chinese patients with 43 wounds were enrolled for application of 50 cycles of TRS therapy. TRS was used for two main clinical applications: closure of a variety of surgical/traumatic wounds; and securing wound closure after high-tension suture closure. Basic information and details regarding this therapy and its complications were recorded. Follow-up visits were conducted three to six months after wound closure. RESULTS: Mean residual wound width decreased approximately 20% every two days during cycles of TRS therapy. Infection was the most common complication (five cases). Other complications included dehiscence (two cases) and pressure ulcer (one case). At the six-month follow-up visit, (21 wounds in 20 patients), both the extent of healing and the scar were acceptable. DISCUSSION: There are no absolute contraindications to TRS therapy. The authors have formulated instructions for the prevention and treatment of the most common complications. CONCLUSIONS: The results demonstrate that TRS therapy is a simple, effective method for primary closure of difficult wounds, and large skin and soft-tissue defects. Larger randomized studies are required to further evaluate of the effectiveness, indications, complications and cost effectiveness of this innovative TRS therapy.

AIM:To investigate the effect of tanshinone IIA on the apoptosis of human umbilical vein endothe -lial cells (HUVECs) after high glucose treatment.METHODS:The cell viability was determined by MTT assay. The cell apoptotic rate was examined by flow cytometry with Annexin V-FITC/PI double staining .The expression of Bcl-2 and Bax, and the release of mitochondrial cytochrome C ( Cyt C) were analyzed by Western blotting .RESULTS: Tanshinone IIA significantly inhibited high glucose-induced decrease in cell viability and increased the cell apoptosis .Additionally, after tanshinone IIA treatment , Bax expression and the release of mitochondrial Cyt C were significantly inhibited , while Bcl-2 expression was increased .CONCLUSION: Tanshinone IIA prevents high glucose-induced endothelial cell apoptosis via mitochondria-dependent pathway .

RESUMO

A multiresidue method is described for the analysis of 106 multiclass pesticides (organochlorine, organophosphate, carbamate, pyrethroid, and triazine classes) in apple juice in a single injection. The determination procedure was based on matrix solid-phase dispersion of juice on diatomaceous earth in a glass column and subsequent extraction with a mixture of hexane-dichloromethane (1 + 1) at a flow rate of 5 mL/min. The analytes were determined by capillary gas chromatography with mass spectrometric detection and confirmed by their retention times and ion ratios. The coefficients of variation for analysis of samples fortified over the range of 0.01-0.2 mg/kg were 1.62 to 18.4%, and the recoveries for all analytes were between 70 and 110%.
