RESUMO

OBJECTIVE: To examine the influence of Saponin I from Shuitianqi (Rhizoma Schizocapasae Plantagineae) (SSPH I) on hepatocellular carcinoma (HCC) metastasis, and to elucidate the underlying mechanism. METHODS: The intrahepatic metastasis Bagg's Albino/c (BALB/c) mouse model was established with human hepatocellular carcinomas (HepG2) cells, then treated with normal saline (once per day), cisplatin (2 mg/kg, once every 2 d), and SSPH Ⅰ (25, 50, and 75 mg/kg, once per day). Then, we assessed alterations in the hepatic pathology and target protein expressions in the intrahepatic metastasis BALB/c mouse model using a series of molecular biology techniques. RESULTS: Based on our analysis, SSPH Ⅰ significantly alleviated hepatocyte necrosis and tumor cells infiltration. Moreover, SSPH Ⅰ suppressed extracellular matrix (ECM) degradation and angiogenesis viaa decrease in matrix etalloproteinase-2 (MMP-2), MMP-9, CD31, CD34, and vascular endothelial growth factor (VEGF) levels. Furthermore, SSPH Ⅰ repressed invasion and meta-stasis by suppressing the transforming growth factor-ß1 (TGF-ß1)/Smad7 axis and epithelial-mesenchymal transition (EMT), as evidenced by the scarce TGF-ß1, N-cadherin, and Vimentin expressions, and elevated Smad7 and E-cadherin expressions. CONCLUSION: The SSPH Ⅰ-mediated negative regulation of the TGF-ß1/Smad7 axis and EMT are critical for the inhibition of HCC invasion and metastasis.

Objective:To investigate the changes of serum catecholamines (CST) and oxidative stress index in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) with hypertension and the correlation with blood pressure.Methods:Total of 100 OSAHS patients admitted to Jingmen No.1 People′s Hospital from February 2018 to August 2020 were selected. According to whether patients combined with hypertension, they were divided into OSAHS group (54 cases) and OSAHS combined with hypertension group (46 cases). The changes of serum CST and oxidative stress index in the two groups were compared and analyzed the correlation of blood pressure and CST and oxidative stress index.Results:The level of myeloperoxidase(MPO) in the OSAHS combined with hypertension group was higher than that in the OSAHS group: (41.67 ± 3.02) U/L vs. (39.11 ± 2.65) U/L; the levels of catalase (CAT) and CST in the OSAHS combined with hypertension group were higher than those in the OSAHS group: (11.47 ± 2.54) kU/L vs. (15.88 ± 2.31) kU/L, (23.63 ± 4.38) ng/L vs. (28.61 ± 5.42) ng/L, there were statistically differences ( P<0.05). The levels of respiratory disturbance index, lowest oxygen saturation, triacylglycerol, total cholesterol, low density lipoprotein cholesterin, high density lipoproteincholesterol, epinephrine, noradrenaline in the two groups had no significant differences ( P>0.05). Spearman correlation analysis showed that the systolic pressure and diastolic pressure had positive correlation with MPO ( P<0.05), and had negative correlation with CAT and CST ( P<0.05). Conclusions:The serum CST and CAT are down-regulated in patients with OSAHS with hypertension, and serum MPO is up-regulated. Oxidative stress may be related to OSAHS with hypertension.

Objective: Analysis on the financial influences of for Medicaid expenditure expansion. Methods: Using Economics analysis to study the relation between the method of Federal Medicaid grants and scope of States Medicaid. Results and Conclusion:The design of transfer payment motivated the states government to pursue maximizing Federal grants , which led to the medical expenditure expansion. The Policy aims on establishing unify Medicaid policy in the nation and all grants raised from the Federal government.

Objective To investigate the effect of rice water on prevention of diarrhea in patients with traumatic head injury undergoing enternal nutrition. Methods 110 patients with traumatic head injury in neurosurgery department were selected and randomly divided into the observation group and the control group.The observation group was fed with rice water in the early two days of entemal nutrition and then begun with nutrient solution on the third day, as to the control group,patients were fed with nutrient solution at the beginning of enternal nutrition. Incidence of diarrhea in these two groups were viewed and underwent χ2 test. Results The number of samples with diarrhea in the observation group(3 cases) was significantly less than that of the control group (12 cases). Conclusions Feeding with rice water can clean the intestine, regulate the function of gastrointestinal tract and reduce the incidence of diarrhea obviously in patients with traumatic head injury during the early period of enternal nutrition.
