RESUMO

A large majority of primary hepatobiliary tumors are hepatocellular carcinomas (HCC), with the remainer being cholangiocarcinoma. While surgical resection remains the gold standard treatment for hepatobiliary tumors, relatively few patients are operative candidates, and systemic treatments have limited effectiveness. Locoregional therapies offer significant promise in the management of HCC. Ablation and radioembolization may offer similar outcomes to surgery for early-stage hepatocellular carcinoma while radioembolization and chemoembolization are valuable in the management of advanced disease. There is significantly less evidence for the role of locoregional therapy in the treatment of cholangiocarcinoma, although it appears to be well tolerated.

Concrete is the most used human-made material in the world, and it is responsible for around 8% of the total greenhouse gas emissions worldwide. Hence, efficient concrete construction methods are one of the main foci of research in architecture, civil engineering, and material science. One recent development that promises to achieve this goal is the use of digital fabrication for building components. Most investigations focus on direct extrusion 3D printing with concrete, which has already been covered in several review articles. Conversely, this article reviews a different approach, which focuses on the indirect digital fabrication of concrete through 3D printed formworks. This approach is under investigation for structural and nonstructural, as well as for on- and off-site applications, with a number of projects having already been built, but a comprehensive review of 3D printed formworks has not yet been compiled to synthesize the findings. This article provides a comprehensive map of the state-of-the-art of five different 3D printing technologies used for the fabrication of formworks so far. The aim is to serve as a fundamental reference for future research, provide a basis for consistent language in this field, and support the development of construction standards. The article further discusses the new geometric possibilities with 3D printed formworks and their potential for making concrete construction more sustainable. In addition, the opportunities and challenges of 3D printed formworks are evaluated in the context of other traditional and digital fabrication tools. A synthetic classification in five functional typologies is proposed and illustrated with 30 representative case studies. Finally, the article concludes with a brief reflection on the role of 3D printing in the broader context of formwork innovation and a possible outlook for this technology.

Although a wide variety of pathologies can occur in the limited anatomic space within and surrounding the sella turcica only a few are common. This review aims to briefly summarize pituitary and parasellar anatomy and provide a focused description of the imaging features of both common and rare pituitary pathologies. Diagnoses of imaging findings with important implications for clinical management are highlighted. MR is the primary diagnostic modality for evaluation of this anatomic region. CT supplements MR in the evaluation of pathologies involving the bony sella turcica while angiography or nuclear medicine plays a limited clinical role. Despite the wide array of pathologies, imaging and basic clinical history will frequently yield a specific diagnosis or narrow differential. In certain pathologies such as hypophysitis or pituitary hyperplasia, proper imaging interpretation may obviate the need for surgical biopsy or resection. The two key elements to diagnosis in the pituitary region are localization of the abnormality and recognition of characteristic imaging features for different pathologies. Localization is particularly important in separating parasellar masses such as meningiomas, skull base tumors, carotid aneurysms, craniopharyngiomas, or sphenoid sinus tumors from pituitary masses. Imaging features are often variable and in some cases such as craniopharyngioma or epidermoid, can be almost pathognomonic. In cases of neoplastic pathology, imaging both provides diagnostic information and guides planning of surgical biopsy or resection. In most cases, biopsy or resection is performed though a trans-sphenoidal endoscopic route, and identifying invasion or the suprasellar cistern, skull base, or cavernous sinuses is critical.

A 61-year-old man presented for lung cancer screening with low dose CT. A spiculated right apical nodule suspicious for primary lung malignancy and an indeterminate small basilar consolidation were identified. PET/CT was notable for increased FDG uptake in the basilar consolidation. Transthoracic needle biopsy of both lesions was performed which lead to pathologic diagnoses of pulmonary amyloid nodule for the apical nodule and pulmonary extramarginal zone lymphoma of the mucosa associate lymphoid tissue for the basilar consolidation. While incidental findings are common in lung cancer screening CT, exceedingly rare diagnoses or combinations or diagnoses may also be encountered. This case also underscores the value of pathologic diagnosis in cases of indeterminate lung nodules.
