BACKGROUND: It has been suggested digital mucous cysts (DMCs) are associated with osteoarthritis and osteophytes in the elderly, and usually have a communicating pedicle with the joint. Surgical excision is a standard therapy with a high cure rate. OBJECTIVE: The purpose of this prospective study is to evaluate the features of DMCs via clinical, radiological and pathological examination and the efficacy of surgical excision of DMCs. METHODS: Between 2010 and 2014, 24 Korean patients were treated with the resection of the cyst and the pedicle. Preoperative X-ray and ultrasonography were performed to detect the presence of the osteophyte and the connection to the joint space. Postoperative patients' satisfaction score was assessed by the visual analogue scale (0~10). RESULTS: The osteophytes were found in 15.8%. In ultrasonographic findings, there were prominent flow signals between the cyst and the joint space in 13.6%. There were no serious postoperative complications, and recurrences were observed in 16.7%. Mean postoperative satisfaction score was 8.3. CONCLUSION: It seems that preoperative X-ray for osteophytes and ultrasonographic study for connection are not always helpful for the treatment of DMCs, and that the surgical excision with a pedicle ligation and electrocoagulation is an effective treatment modality.

RESUMO

Eccrine squamous syringometaplasia (ESS) is a histologically distinctive skin eruption occurring predominantly in acral or intertriginous areas presenting as erythematous macules, papules or patches. The etiology of ESS remains unclear, but it is usually reported in patients receiving chemotherapy for various malignant neoplasms. To date, only two cases of ESS associated with non-steroidal anti-inflammatory drugs (NSAIDs), which has distinctive clinical features and pathogenesis, have been reported in the literature. Herein, we report a rare and interesting case of ESS associated with pelubiprofen, a recently developed NSAID, which appeared after pelubiprofen therapy and resolved spontaneously after discontinuing the medication.

RESUMO

Segmental neurofibromatosis is a rare form of neurofibromatosis that is characterized by neurofibromas and/or café au lait macules, limited to one region of the body. The neurofibromas of segmental neurofibromatosis are most commonly occupied by either a cervical or a thoracic dermatome. Segmental neurofibromatosis on the face is extremely rare, and only 10 cases have been described so far. Herein, we report a case of segmental neurofibromatosis on the V1 dermatome for its rarity and unusual location.

Terbinafine is a highly effective oral antifungal agent that is used to treat dermatophyte infections of the skin and nails. Although generally well tolerated, common side effects of terbinafine include nausea, vomiting, abdominal pain, urticarial or erythematous eruptions, and pruritus. Taste disturbance is a rare side effect of terbinafine, occurring in 0.6%approximately2.8% patients taking the drug orally. Here, we report the case of a 67-year-old man who complained of taste disturbance and loss of appetite 1 month after starting terbinafine treatment for onychomycosis. Low body mass index and advanced age are considered predisposing factors for the development of terbinafine-induced taste disturbance. As the proportion of elderly population is increasing, the frequency in use of the prescription of drugs such as terbinafine for the elderly is also increasing. Therefore, clinicians should be aware of this unfamiliar and uncommon adverse effect associated with the antifungal agent, terbinafine and should perform careful follow-up of elderly patients with low BMI taking this medication, as they may become depressed or alter their food intake in response to a decrease or loss of taste sensation.

RESUMO

Eczematid-like purpura of Doucas and Kapetanakis is a type of pigmented purpuric dermatoses. It is clinically characterized by pruritic seasonal eruptions occurring in the spring and summer, and histopathologically characterized by spongiosis and parakeratosis in the epidermis and by the lymphocyte-mediated leakage of erythrocytes from capillaries in the papillary dermis. We report a case of eczematid-like purpura of Doucas and Kapetanakis that showed clinical improvement with narrowband UVB (NB-UVB). The patient was a 66-year-old man with pruritic, well-demarcated scaly purpuric patches on his feet that appeared 2 months prior. A histopathological study showed mild superficial perivascular lymphocytic infiltration, focal spongiosis with exocytosis, and erythrocyte leakage. We initiated treatment with an oral antihistamine, ascorbic acid, and a topical steroid, but the lesion was aggravated. We therefore treated the patient with NB-UVB for 6 months, and the lesions regressed progressively with residual postinflammatory hyperpigmentation.

RESUMO

Kaposi sarcoma (KS) is a multifocal proliferative vascular tumor often caused by human herpes virus 8. Among the four subtypes of KS, classic KS (CKS) is usually chronic, persisting over many years, but is not life threatening. Because the natural history of KS varies, many kinds of therapeutic option for CKS are available and their assessment may be difficult. We experienced three cases of CKS showing HHV-8 positive in biopsy, no history of immunosuppression and HIV infection, normal chest X-ray, no metastasis, and successfully treated with intralesional injection of Pegylated Interferon-alpha2a (Peg-IFN-alpha2a). After an intralesional Peg-IFN-alpha2a injection of 180 mcg/0.5 ml was administered once a week for 4 months, the size and induration of the lesions reduced greatly and the colors faded away. Histopathological examination of the resolved lesions did not show any evidence of KS with negative HHV-8 results. To the best of our knowledge, this is the first report in the literature for the treatment of CKS with intralesional Peg-IFN-alpha2a injection.
