Artificial intelligence (AI) has rapidly transformed various aspects of life, and the launch of the chatbot "ChatGPT" by OpenAI in November 2022 has garnered significant attention and user appreciation. ChatGPT utilizes natural language processing based on a "generative pre-trained transfer" (GPT) model, specifically the transformer architecture, to generate human-like responses to a wide range of questions and topics. Equipped with approximately 57 billion words and 175 billion parameters from online data, ChatGPT has potential applications in medicine and orthopedics. One of its key strengths is its personalized, easy-to-understand, and adaptive response, which allows it to learn continuously through user interaction. This article discusses how AI, especially ChatGPT, presents numerous opportunities in orthopedics, ranging from preoperative planning and surgical techniques to patient education and medical support. Although ChatGPT's user-friendly responses and adaptive capabilities are laudable, its limitations, including biased responses and ethical concerns, necessitate its cautious and responsible use. Surgeons and healthcare providers should leverage the strengths of the ChatGPT while recognizing its current limitations and verifying critical information through independent research and expert opinions. As AI technology continues to evolve, ChatGPT may become a valuable tool in orthopedic education and patient care, leading to improved outcomes and efficiency in healthcare delivery. The integration of AI into orthopedics offers substantial benefits but requires careful consideration and continuous improvement.

BACKGROUND: Atherosclerosis, as a major cause of stroke, is responsible for a quarter of deaths worldwide. In particular, rupture of late-stage plaques in large vessels such as the carotid artery can lead to serious cardiovascular disease. The aim of our study was to establish a genetic model combined with machining leaning techniques to screen out gene signatures and predict for advanced atherosclerosis plaques. METHODS: The microarray dataset GSE28829 and GSE43292 which were publicly obtained from the Gene Expression Omnibus database were utilized to screen for potential predictive genes. Differentially expressed genes (DEGs) were identified by using the "limma" R package. Gene Ontology (GO) and Kyoto Encyclopedia of Genes Genomes (KEGG) analyses of these DEGs were performed by Metascape. Later, Random Forest (RF) algorithm was applied to further screen out top-30 genes which contribute the most. The expression data of top 30-DEGs were converted into a "Gene Score". Finally, we developed a model based on artificial neural network (ANN) to predict advanced atherosclerotic plaques. The model later was validated in an independent test dataset GSE104140. RESULTS: A total of 176 DEGs were identified in the training datasets. GO and KEGG enrichment analysis revealed that these genes were enriched in leukocyte-mediated immune response, cytokine- cytokine interactions, and immunoinflammatory signaling. Further, top-30 genes (including 25 upregulated and 5 downregulated DEGs) were screened as predictors by RF algorithm. The predictive model was developed with a significantly predictive value (AUC = 0.913) in the training datasets, and was validated with an independent dataset GSE104140 (AUC = 0.827). CONCLUSION: In present study, our prediction model was established and showed satisfactory predictive power in both training and test datasets. In addition, this is the first study adopted bioinformatics methods combined with machine learning techniques (RF and ANN) to explore and predict for the advanced atherosclerotic plaques. However, further investigations were needed to verify the screened DEGs and predictive effectiveness of this model.

The gut microbiota is increasingly considered to play a key role in human immunity and health. The aging process alters the microbiota composition, which is associated with inflammation, reactive oxygen species (ROS), decreased tissue function, and increased susceptibility to age-related diseases. It has been demonstrated that plant polysaccharides have beneficial effects on the gut microbiota, particularly in reducing pathogenic bacteria abundance and increasing beneficial bacteria populations. However, there is limited evidence of the effect of plant polysaccharides on age-related gut microbiota dysbiosis and ROS accumulation during the aging process. To explore the effect of Eucommiae polysaccharides (EPs) on age-related gut microbiota dysbiosis and ROS accumulation during the aging process of Drosophila, a series of behavioral and life span assays of Drosophila with the same genetic background in standard medium and a medium supplemented with EPs were performed. Next, the gut microbiota composition and protein composition of Drosophila in standard medium and the medium supplemented with EPs were detected using 16S rRNA gene sequencing analysis and quantitative proteomic analysis. Here, we show that supplementation of Eucommiae polysaccharides (EPs) during development leads to the life span extension of Drosophila. Furthermore, EPs decreased age-related ROS accumulation and suppressed Gluconobacter, Providencia, and Enterobacteriaceae in aged Drosophila. Increased Gluconobacter, Providencia, and Enterobacteriaceae in the indigenous microbiota might induce age-related gut dysfunction in Drosophila and shortens their life span. Our study demonstrates that EPs can be used as prebiotic agents to prevent aging-associated gut dysbiosis and reactive oxidative stress.

RESUMO

The vibration and noise that resulted from turbomachinery, such as fans, compressors, and centrifugal pumps, are known to bring considerable disturbance and pollution to the machine itself, the environment, and the operators. Hence, how to cope with the vibration and noise has become a recent research focus. With the advancement of materials science, more and more new nanomaterials have been applied in the field of noise and vibration reduction. To be specific, carbon-based nanomaterials, such as carbon fibers, carbon nanotubes, and graphenes, have achieved outstanding results. Carbon nanocomposites, such as carbon nanofibers, carbon nanotubes, and graphenes, are characterized by their low densities, high strengths, and high elastic moduli, all of which made carbon nanocomposites the most promising vibration and noise-reduction composites, thanks to their damping properties, compatibilities, noise and vibration absorption qualities, and wide wave-absorbing frequency bands. In light of this, this paper summarizes the progresses and application prospects of such carbon nanocomposites as carbon nanofibers, carbon nanotubes, and graphenes in the field of turbomachinery vibration and noise reduction.

Domestic consumption of biomass fuels has been found as a leading source of polycyclic aromatic hydrocarbons (PAHs) in pristine regions. The biomass ashes would serve as both source and vector for PAHs, which may threaten residents' health. However, research focusing on the behaviors of waste biomass ashes acting as emission resources of PAHs is still lacking. In present study, occurrence and fate of PAHs released from disposal biomass ashes in the Tibetan Plateau were investigated by evaluating their patterns and levels in soils at different distances from the Lhasa landfill. Concentrations of 16 PAHs averaged at 1009 ng/g dry weight and 103 ng/g dry weight in landfill cover soils and surrounding soils, respectively. The spatial distributions of PAHs evidenced the local source of biomass ashes in landfill, while the fractionations of PAHs in soils profiles emphasized the post-deposal leaching. Since PAH compositions altered significantly during their transport and sequestration, the rectification factors were suggested to adjust the source diagnostic ratios. In study area, the emission flux of PAHs from waste biomass ashes to soils was 0.93 ton/year; extended to the Tibetan Plateau, the flux was estimated to be 13.1-32.5 ton/year. This study confirmed that waste biomass ashes would represent significant contributions of PAH emissions in this pristine region, which might remarkably exacerbate the total emissions of PAHs in Tibet by more than 25%.

MSC transplantation has been explored as a new clinical approach to stem cell-based therapies for bone diseases in regenerative medicine due to their osteogenic capability. However, only a small population of implanted MSC could successfully reach the injured areas. Therefore, enhancing MSC migration could be a beneficial strategy to improve the therapeutic potential of cell transplantation. Catharmus tinctorius volatile oil (CTVO) was found to facilitate MSC migration. Further exploration of the underlying molecular mechanism participating in the pro-migratory ability may provide a novel strategy to improve MSC transplantation efficacy. This study indicated that CTVO promotes MSC migration through enhancing ROCK2 mRNA and protein expressions. MSC migration induced by CTVO was blunted by ROCK2 inhibitor, which also decreased myosin light chain (MLC) phosphorylation. Meanwhile, the siRNA for ROCK2 inhibited the effect of CTVO on MSC migration ability and attenuated MLC phosphorylation, suggesting that CTVO may promote BMSC migration via the ROCK2/MLC signaling. Taken together, this study indicates that C. tinctorius volatile oil could enhance MSC migration via ROCK2/MLC signaling in vitro. C. tinctorius volatile oil-targeted therapy could be a beneficial strategy to improve the therapeutic potential of cell transplantation for bone diseases in regenerative medicine.

In the background region of the Tibetan Plateau (TP), the rapid urbanization probably results in the massive generation of persistent organic pollutants (POPs), which lacks monitoring and evaluation. Since landfill could serve as an important sink of the locally used POPs, the analysis of POPs in the Tibetan landfill area might help us to understand the source composition and their transport in the TP. In this study, the concentration variations of polybrominated diphenyl ethers (PBDEs) in five soil profiles and seven surficial sediments around the largest Tibetan landfill were investigated. The total concentrations of PBDEs ranged from 128 to 1219 ng/kg in soils, and from 447 to 7295 ng/kg in sediments. The dominance of nona- and deca-BDEs possibly indicated the wide usage of deca-BDE as flame retardant in the TP. The vertical and spatial distribution patterns of PBDEs within soils plausibly revealed their main transport pathways by atmospheric dispersion and leachate seepage from landfill. Based on principal components analysis and multiple linear regression, these two pathways were estimated to account for 61% and 39% of the total concentrations, respectively. Additionally, the spatial and vertical distributions of octa-to deca-BDEs within soils were significantly influenced by soil particle size. Although the PBDEs inventory in the study area was comparatively low, the rapid urbanization in the TP might dramatically accelerate the PBDE emissions in the future. This study firstly introduced the presence of local PBDEs in the TP, and the inventory already influenced the surrounding environment. Once involved in the regional cycle of the TP, the local source of PBDEs from waste might significantly serve to raise background level resulting otherwise primarily from long-range atmospheric transport.

Based on RS and GIS techniques, and combining with digital terrain index and land use degree composite index, this paper studied the changes of land use pattern in Qixia City of Shandong Province form 1987 to 2003, and quantitatively analyzed the land use degree on terrain gradient. The results indicated that in the City, construction land had obvious advantageous on lower terrain niche, the advantageous terrain niche section of orchard increased by six terrain niche and distributed from aggregation to uniformity, while the advantageous terrain niche of crop land, forest land, grass land, water area, and unutilized land generally remained stable during the study period. According to the quantitative expression of land use degree on terrain gradient, terrain niche was divided into three sections, i.e., 1-15, 16-30, and 31-50, which occupied 49.48%, 43.58%, and 6.94% of the total study area, respectively.

<p><b>OBJECTIVE</b>To investigate the effect of extract of Gingko biloba (EGb) on soluble intercellular cell adhesion molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1) in patients with early diabetic nephropathy (DN).</p><p><b>METHODS</b>Sixty-three patients with DN in early stage were randomly assigned to the control group (29 cases) and the treatment group (34 cases). Both groups were treated by routine treatment, and with EGb given to the treatment group additionally. The treatment course was 2 months. Serum sICAM-1 and sVCAM-1 were determined with ELISA before and after treatment, and urinary albumin excretion rate (UAER), fasting plasma glucose (FPG), serum creatinine (SCr) and blood lipids, etc. were examined as well.</p><p><b>RESULTS</b>The levels of serum sICAM-1 and sVCAM-1 were significantly lower in both groups after treatment than those before treatment (P < 0.05 or P < 0.01), and the decrement in the treatment group was more significant than that in the control group (P < 0.01). And the levels of UAER, SCr and blood lipids decreased in the treatment group after treatment (P < 0.05 or P < 0.01).</p><p><b>CONCLUSION</b>EGb could retard the development of early DN through decreasing the levels of serum sICAM-1 and sVCAM-1.</p>

<p><b>OBJECTIVE</b>To observe the efficacy of combination therapy of clomiphene and Chinese drugs for nourishing Shen and activating blood circulation (NSABC) in treating Stein-Leventhal syndrome caused sterility.</p><p><b>METHODS</b>Sixty-two patients with anovulation caused sterility were randomly divided into the treated group (n = 32) and the control group (n = 30). The treated grop was treated with the combination therapy and the control group treated by the same dosage of clomiphene alone.</p><p><b>RESULTS</b>After treatment, when comparing with that before treatment, the endocrine hormones in the treated group improved significantly, showing a markedly decrease of androgen and luteotropic hormone, and increase of estrogen (P < 0.001). The periodic ovulation rate in the treated group reached 87%, the total pregnancy rate being 65.6%, with no occurrence of ovarian hyperstimulation syndrome (OHSS) and luteinized unruptured follicle syndrome (LUFS), while in the control group, the periodic ovulation rate was 66%, the total pregnancy rate 36.6%, with LUFS occurred in 4 patients. Comparison of the therapeutic effects between the two groups showed significant difference (P < 0.05).</p><p><b>CONCLUSION</b>The combination therapy of clomiphene and NSABC has a better therapeutic effect in treating Stein-Leventhal syndrome caused sterility than that of using clomiphene alone.</p>
