Introduction: Rhinitis is defined as inflammation of nasal mucosa. Allergic rhinitis is the most common type of chronic rhinitis with increasing prevalence. Tests such as nasal smear for eosinophilia, Skin prick test, Serum absolute eosinophil count and Immunoglobulin IgE helps us in diagnosis of allergic rhinitis. Objective: To evaluate the efficacy of the nasal smear for eosinophilia and blood absolute eosinophil count tests to diagnose allergic rhinitis in patients with varied severity of symptoms. Materials and methods: In this prospective study, all patients above 12 years of age with symptoms suggestive of allergic rhinitis were enrolled in our study. Severity assessment was done by visual analog scoring and Total nasal outcome scoring. Diagnostic nasal endoscopy with nasal smear for eosinophilia, complete blood count and blood absolute eosinophil count tests were done. Results: A total of 60 patients were enrolled in the present study. Age ranged from 13 to 55 years with female preponderance. The most common symptom was rhinorrhea found in 100% of study population, followed by sneezing present in 57 (93.5%) patients. 10 out of 12 patients with mild intermittent symptoms had < 5% of eosinophil elicited in nasal smear and 2 patients had negative smear of eosinophils. Only 13% had mild eosinophilia. In this study, in those patients with mild intermittent symptoms neither nasal smear eosinophilia nor Blood absolute eosinophil count was found to be significant. Conclusions: Nasal smear cytology and blood absolute eosinophil count were found to be significant only in patients with moderate-severe symptoms. Those patients with mild intermittent symptoms need not be tested as it will be insignificant. In these patients, we must rely on the visual analogue score based on the severity of symptoms rather than laboratory tests. Depression is a pervasive mental health disorder characterized by persistent sadness and an inability to enjoy activities that were once enjoyable. This study compares the dermatoglyphic patterns of depressed patients with those of healthy, normal individuals in order to determine if dermatoglyphic patterns can be used as biomarkers for early diagnosis and prompt intervention of depression. A total of 100 depressive disorder patients of both sexes between the ages of 18 and 60 were selected for the study. Dermatoglyphic patterns of individual digits were analyzed using the "Ink and Paper" technique. The Whorl, Loop, and Arch patterns, as well as the AFRC, TFRC, and a-b ridge count, were examined using One-Way ANOVA significance and a Chi-Square test using SPSS 20.0. The results showed that the right hand in the depressive diseased group had decreased numbers of Whorls in the 2nd and 5th digits, and decreased numbers of Loops in all five digits. Similarly, decreased numbers of Whorls were found in the 1st, 2nd, 4th, and 5th digits of the left hand, as well as decreased Loops in the 4th digit. However, there were no changes in the Arches of either hand when compared to the control group. The AFRC and TFRC were significantly decreased (p<0.0001), but there was no significant decrease in the a-b ridge count in the depressive diseased group when compared to the control group. Dermatoglyphics, a noninvasive method, can serve as a screening indicator for depressive individuals, the appearance of the decreased count of whorls and loops parameters might be used as an exploratory sign.

ABSTRACT: Castleman's disease is a rare angio-follicular lymph node hyperplasia[1,2] have varying presentation and can mimic malignancy. The common locations are mediastinum, neck, axilla, and pelvis but extra nodal sites can also be affected.[3] A 56-year-old patient incidentally found to have mass in the retroperitoneal region by Contract-enhanced computerized tomography (CT). Positron emission tomography scan showed well defined intensely enhancing fluorodeoxyglucose (FDG) avid lesion in the left iliac region with vascularity. Surgery was performed, and pathological diagnosis was hyaline vascular type-stroma-rich variant of Castleman disease. Here, we report a case of huge retroperitoneal Castleman's disease in a 56-year-old female patient.[4].

The objective of this prospective randomized controlled single-center clinical trial was to prove the efficacy of adjunctive photobiomodulation in improving selected outcomes following the use of laterally closed tunnel technique for the management of isolated gingival recession. Nineteen participants (with isolated gingival recession) each treated by laterally closed tunnel technique were randomized to either add on treatment with control (sham laser application) or test group (photobiomodulation with 660 nm diode, 3.5 J/cm2 per point of application). The primary outcome variable was change in recession depth and secondary variables included recession width, width of keratinized gingiva, periodontal biotype, and VAS score for pain assessment and EHS index for early wound healing assessment. Analysis was performed using a linear mixed effects model. There were no significant differences in the gingival recession depth (p = 0.8324) and recession width (p-0.969) at 3-month follow-up. The VAS scores were significantly lower for the test (laterally closed tunnel technique + photobiomodulation) group as compared to control (laterally closed tunnel technique + sham laser) over time (p = < 0.0001) as well as per site (p = 0.0006) The Early Wound Healing Index scores were significantly higher in the test (laterally closed tunnel technique + photobiomodulation) group as compared to control (laterally closed tunnel technique + sham laser) group (p < 0.0001). The adjunctive use of photobiomodulation did not show a better outcome concerning recession depth but appears to provide faster healing of the surgical wounds and better patient comfort. The result needs further evaluation in particular with respect to long-term effect and due to limitation in sample size. Clinical Trial Registry of India: CTRI/2019/11/022012.

AIM AND OBJECTIVES: To assess the effectiveness of neo-adjuvant chemotherapy and its impact on the clinical and pathological response in locally advanced breast cancer. To compare molecular subtypes of breast cancer with response to neo-adjuvant chemotherapy. PATIENTS AND METHODS: This was a prospective study on patients who received neoadjuvant chemotherapy for breast carcinoma for a 3-year period. A total of 60 patients who presented with locally advanced breast cancer (LABC) were treated with neoadjuvant chemotherapy. Forty patients were treated with the 5-fluorouracil, epirubicin, and cyclophosphamide (FEC) schedule, 16 patients were treated with Adriamycin and cyclophosphamide (AC), and four patients were treated with oral cyclophosphamide, intravenous methotrexate, and fluorouracil (CMF). Taxol was added in all node-positive cases, triple negative breast cancer (TNBC), and Her 2 positive cases. The clinical response was assessed with RECIST criteria after neoadjuvant chemotherapy. The response was compared with molecular subtypes of carcinoma breast and receptor status individually. RESULTS: A total of 60 female patients receiving primary chemotherapy for locally advanced breast malignancy were studied. The median age of the patients at the time of diagnosis was 44 years (range=24-73). In terms of menopausal status, 25 (42%) patients were pre-menopausal and 35 (58%) patients were post-menopausal. Histological classification showed invasive ductal carcinoma in 72% of patients, invasive lobular carcinoma in 15% of patients, and other types including mixed patterns in 13% of patients. Among 60 patients, 16 patients (26.6%) had clinically complete remission (cCR), 30 patients (50%) had partial remission, eight patients (13.3%) had stable disease, and six patients (10%) had progressive disease. Following neoadjuvant chemotherapy, 46 (76.6%) patient underwent Modified radical mastectomy surgery. Target therapy was given for Her2 neu patients after surgery. Hormonal therapy was added to hormone ER PR positive cases postoperatively. Eight patients (13.3%) among this operated cases attained complete pathological response. CONCLUSION: Preoperative chemotherapy downstages the primary tumors and axillary metastasis in patients with locally advanced breast carcinoma. Comparison of molecular subtypes with chemotherapy response is a better way to find out the predictors of response to chemotherapy.

Nitrogen-doped reduced graphene oxide (NRGO) stabilized copper nanoparticles are designed to assist Cu(i)-catalyzed Huisgen [3+2] cycloaddition "click" chemistry (CuAAC). This study demonstrates a robust route for the synthesis of vastly dispersed heterogeneous catalysts (NRGO/Cu2O), achieving CuAAC at low temperature without any external additive (oxidizing/reducing agent) with high stability and recyclability. Underlying mechanisms are analysed using DFT calculations, confirming the experimental results.

Demand for green energy production is arising all over the world. A lot of emphasis is laid in making the buildings green. Even a small amount of energy savings made contribute to saving the environment. In this study, an idea is proposed and studied to extract power from the high head water in the pipelines of a building. A building of height 15 m is considered for this study. Water flowing in the pipe has sufficient energy to run a micro hydro turbine. The feasibility of producing electrical energy from the energy of pipe water is found. The motivation is to find the feasibility of generating power using a low-cost turbine. The experimental setup consists of micro turbine of 135 mm diameter coupled to a 12-V DC generator; LEDs and resistors are employed to validate the results. The theoretical calculations were presented using the fundamental equations of fluid mechanics. The theoretical results are validated using experimental and numerical results using CFD simulation. In addition, exergy analysis has been carried out to quantify the irreversibilities during the process in the system.

In this report, we have successfully fabricated an immunosensor for detection of Pseudomonas aeruginosa in water. The monoclonal antibody was immobilized on the surface modified with CCLP (Calcium Cross-Linked Pectin)-Au NPs (gold nanoparticles)/Glassy Carbon Electrode. The building of the immunosensor was evaluated in each step by cyclic voltammetry (CV) and impedance spectroscopy (EIS). The electrochemical detection was done based on the anti rabbit IgG HRP (Horseradish Peroxidase) which binds to the immune complex and the response was observed using Hydro Quininone (HQ) and Hydrogen peroxide (H2O2) in PB (Phosphate Buffer) electrolyte. From the results, the sensitivity range is from 10(1) to 10(7)CFU/ml and LOD is calculated as 9×10(2)CFU/ml. The developed immunosensor also have high selectivity, stability, reproducibility and reusability.

RESUMO

Oral tuberculosis is very rare and when present they are usually secondary to pulmonary tuberculosis. Tuberculous lesions of the tongue have become so infrequent that they are virtually a forgotten disease entity and may pose a diagnostic problem. The case reported in this paper emphasizes the importance of including tuberculosis in the differential diagnosis of any chronic oral ulcer. The low number of oral infections by M. tuberculosis could be due to underreporting.

Presentation of colon ischemia is even more subtle, and this review article details about the various presentation patterns of ischemic colitis and their management.
