RESUMO

This narrative review presents current aspects of diagnosis, epidemiology, aetiology and therapy of Premenstrual Dysphoric Disorder (PMDD). PMDD is characterised by emotional, physical and behavioral changes during the premenstrual phase of the menstrual cycle associated with clinically significant distress and marked impairment of psychosocial functioning. Due to its cardinal symptoms (e. g. marked affect lability and irritability), its specific cycle-dependent course and high symptom-specific stability, PMDD has been outlined as a new diagnostic category in the chapter on depressive disorders in the fifth edition of the "Diagnostic and Statistical Handbook of Mental Disorders". The disorder is still insufficiently known in the German language area; it is not included in the International Statistical Classification of Diseases (ICD-10). The 12-month prevalence varies from 3 to 8% for women of reproductive age. The mechanisms underlying PMDD are still insufficiently known, the findings indicate a multifactorial genesis. It is not possible to provide evidence-based recommendations as there is currently still a lack of randomized controlled interventions in women with PMDD.

OBJECTIVE: Analysis of salivary cortisol concentrations and derived indices is increasingly used in clinical and scientific medicine. However, comprehensive data on these parameters in the general population are scarce. The aim of this study was to evaluate the concentrations of salivary cortisol in a large middle-aged community sample and to identify major factors associated with altered hormone levels. DESIGN: We conducted a cross-sectional study within the Cooperative Health Research in the Region of Augsburg (KORA)-F3 study. A total of 1484 participants aged 50-69 years (52% women) had agreed to provide four saliva samples during a regular weekday. METHODS: We measured salivary cortisol concentrations at wake-up (F0), (1/2) h (F(1/2)), 8 h (F8), and 14 h (F14) after waking. We calculated cortisol awakening response (CAR), slope, and area under the curve (AUC(G)) of the circadian cortisol secretion. Sociodemographic and clinical characteristics were evaluated by interview and questionnaires, sampling conditions by protocol. In total, 1208 participants returned saliva samples, exclusion criteria left 990 subjects for final analyses. RESULTS: Salivary cortisol levels were (means+/-s.d.) F0=13.7+/-7.6, F(1/2)=20.5+/-9.8, F8=5.4+/-3.3, and F14=2.0+/-1.8 nmol/l. Earlier sampling times were associated with higher CAR and smaller slope. Cortisol secretion was also influenced by gender and smoking habits. Higher perceived social support was associated with lower AUC(G) and smaller slope. CONCLUSIONS: We provide data on salivary cortisol concentrations in a large middle-aged community sample. Gender, sampling time, smoking habits, and perceived social support appeared as determinants of cortisol secretion.

RESUMO

The paper reflects central recommendations and methodological issues of the new German guidelines for the treatment of depression, as discussed in the article New German Guidelines for the Treatment of Depression - The Central Role of Psychotherapy (Schauenburg et al. 2009). Members of the steering group for these guidelines disagree with the authors' description in several points, especially with reference to the efficacy of pharmacotherapy with antidepressants and psychotherapy, as well as the relationship between both strategies of therapy and their combination in diverse phases of treatment (acute/maintenance). Furthermore, we try to clarify some misunderstandings in matters of the guideline's methodology which arose in the paper cited.

OBJECTIVE: To compare the prevalence rates of problematic alcohol consumption in students and in the general population. METHODS: A sample of students (n=1 115) and a population sample (n=631) were assessed with the Patient Health Questionnaire (PHQ). The PHQ probes for problematic alcohol consumption with five questions. An "alcohol syndrome" is suspected if at least one of these indicators is present. RESULTS: The alcohol syndrome was present more frequently among students than in the population sample (30,1% versus 8,1%). The odds ratio, adjusted for age, gender and education, was OR=3,7. However, when the five indicators were analysed separately, only two of them were significantly more frequent in students than in the comparison sample. Both indicate a neglect of duties as a consequence of alcohol consumption. CONCLUSIONS: Intervention strategies aimed at students' alcohol consumption should focus on binge drinking and the risks associated with this problematic form of alcohol consumption.
