RESUMO

BACKGROUND: Clinically silent uterine rupture with complete fetal expulsion into the abdominal cavity is an extremely rare complication. Diagnosis can be difficult and the risk to the mother and fetus is high. Conservative management has been described only in a few cases of partial expulsion of the fetus so far. CASE PRESENTATION: We present a case of 43-year-old tercigravida with a history of previous laparotomic myomectomy and subsequent cesarean section. The subsequent pregnancy was complicated by uterine wall loosening and rupture at the site of the previous uterine scar after myomectomy and complete fetal expulsion into the abdominal cavity. The diagnosis was made at 24 + 6 weeks of gestation. Considering the absence of clinical symptomatology and the good condition of the fetus, a conservative approach was chosen with intensive monitoring of the maternal and fetal conditions. The pregnancy ended by elective cesarean section and hysterectomy at 28 + 0 weeks of gestation. The postpartum course was uneventful and the newborn was discharged to home care 63 days after delivery. CONCLUSIONS: Fetal expulsion into the abdominal cavity after silent uterine rupture of the scarred uterus may be accompanied by minimal symptomatology making early diagnosis difficult. This rare complication must be considered in the differential diagnosis in women after major uterine surgery. In selected cases and under conditions of intensive maternal and fetal monitoring, conservative management may be chosen to reduce the risks associated with prematurity.

INTRODUCTION: To evaluate the results of the fluctuations of intraocular pressure (IOP) and calculated mean ocular perfusion pressure (MOPP) during the usual steps of standard phacoemulsification. METHODS: Nine human eyes were evaluated. The IOP was measured indirectly by electronic applanation tonometer. The MOPP was calculated using the systolic blood pressure (SBP), the diastolic blood pressure (DBP) and the IOP: MOPP = 2/3x[DBP + 1/3x(SBP - DBP)] - IOP. The operations were performed with the INFINITI® Vision System: main incision 2.2 mm, coaxial handpiece, 2 paracenteses 1.1 mm, bimanual irrigation/aspiration, bottle height 100 cm. As ocular viscoelastic device (OVD) Hypromel 2.5% (UNIMED) was used. RESULTS: The initial and final IOPs were 17-30 Torr (median 18) and 6-16 Torr (median 8), respectively. The IOP values oscillated between 4 and 63 Torr during the procedure. The highest values of the IOP were achieved at the beginning of phacoemulsification (from 42 to 63 Torr). The maximum pressure higher than 50 mmHg and 60 mmHg was found in 89% and 30% of cases, respectively. The mean ocular perfusion pressure (MOPP) at the beginning of the procedure was 46.4-67.0 (median 53.3) and 0.4-42.0 (median 19.3) during the maximum intraocular pressure. CONCLUSIONS: Measured IOP as well as MOPP varied in all normal steps of real phacoemulsification. High values of intraoperative IOL induced by irrigation may compromise the intraocular perfusion. These fluctuations may induce impairment of the optic nerve perfusion, as well as retina, or choroid.

AIMS: To describe the ultrasound features of benign struma ovarii that often mimic ovarian cancer in the background of complex clinical and histopathological pictures. MATERIAL AND METHODS: We retrospectively identified patients with histologically confirmed benign struma ovarii, treated in our institution between 2003-2016 with complete imaging, clinical, nd histopathological data available. Ultrasound findings were drawn from images, and reports using terms and definitions of the International Ovarian Tumor Analysis group and pattern recognition description was applied. RESULTS: In all, 19 patients were identified; 10 with pure and 9 with impure struma. Median age was 47 (range 24-69); 10 (53%) were premenopausal. Only four (21%) patients presented with pain, others were asymptomatic. Using pattern recognition, 74% strumas (14/19) were uni-/multilocular solid or solid tumors. The solid components were roundish with smooth contours. Six struma pearls were detected. The subjective color score was moderate or abundant in the majority of solid components. Only 5 (26%) tumors were purely cystic. CONCLUSIONS: The ultrasound characteristics differ widely from typical mature ovarian teratoma. Features such as, solid roundish components with smooth contours, struma pearls, acoustic shadowing and occasionally signs of dermoid are clues and may help preoperatively to differentiate benign struma from malignant adnexal lesions.

OBJECTIVE: To evaluate histological uterine fibroid incidence among reproductive age women and to deter- mine correlations between fibroid histological type, patient age, and number and size offibroids. STUDY DESIGN: The study cohort consisted of 103 women desiring preg- nancy who underwent myo- mectomy for symptomatic uterine fibroids. The primary endpoints were histological type of fibroid, myomectomy incidence among 2 age groups (18-34 vs. 35-40), solitary or multiple fibroids, and <5 cm vs. >5 cm fibroid diameter. Secondary anal- ysis endpoints evaluated correlations between uterine fibroid histological type, 2 age groups of women,.and uterine fibroid number and size. RESULTS: Following myomectomy, 84.5% exhibited benign histology, and myomatosis malignancy was not detected. Of the 103 women, 50.5% were aged <34 and 49.5% were aged 35-40; 71.8% had a solitary fibroid and 28.2% had α 2 fibroids; 58.3% had a fibroid of <5 cm. and 41.7% had a fibroid >5 cm in diameter. Cellular fibroid incidence was higher (10.3%) in cases of multiple myomatosis in comparison to the solitary fibroid group (n=0) (p=0.021). Among women with multiple myomatosis (n=29), almost all (n=28, 96.6%) had only 1 histological type. CONCLUSION: Among women of. child-bearing age having myomectomy, most have benign histology with no significant differences in histological type with regard to patient age and fibroid size. A higher incidence of cellular fibroids was observed only in multiple myomatosis cases.

PURPOSE: To compare the extent of corneal endothelial cell loss and pachymetry changes in 2 age-based groups of patients who had cataract removal by AquaLase (Alcon) phacoemulsification in 1 eye and NeoSoniX (Alcon) phacoemulsification in the contralateral eye. SETTING: Department of Ophthalmology, University Hospital, Hradec Králové, Czech Republic. METHODS: This prospective clinical study comprised 28 patients younger than 80 years (Group A) and 28 patients 80 years or older (Group B) with bilateral cataract having lens removal using AquaLase in the right eye and NeoSoniX in the left eye. The nuclei were graded clinically on the basis of hardness. The endothelial cell count (ECC), pachymetry, and best corrected visual acuity (BCVA) were evaluated preoperatively and postoperatively. The mean ECC and pachymetry values (+/-SD) were calculated in each group, with differences between right and left eyes analyzed using the paired t test. RESULTS: In Group A, the differences in the postoperative changes in ECC and pachymetry between AquaLase and NeoSoniX were not statistically significant. In Group B, there were statistically significant differences in postoperative changes in ECC and pachymetry, with the results better in the AquaLase eyes. The BCVA immediately after surgery was better than preoperatively in all eyes. CONCLUSION: The results suggest that AquaLase cataract extraction is safe for the endothelium, even in older patients with harder cataracts and a lower ECC count preoperatively.

PURPOSE: To compare the extent of posterior capsule opacification (PCO) after AquaLase and NeoSoniX phacoemulsification methods using two types of software for PCO quantification. DESIGN: Prospective, randomized clinical trial. METHODS: At the Department of Ophthalmology, University Hospital in Hradec Králové, 50 patients (100 eyes) were analyzed one year after surgery. AquaLase was used in the right eye and NeoSoniX in the left eye of each patient. One year after surgery, digital retroillumination photographs of anterior segments were obtained. The Evaluation of Posterior Capsule Opacification (EPCO) 2000 software (Berlin, Germany) and the Open-Access Systematic Capsule Assessment (OSCA) system (Edinburgh, United Kingdom) were used for PCO assessment. Best-corrected Snellen visual acuity (BCVA) was evaluated before and after surgery. Statistical analysis was performed using parametric tests. RESULTS: The EPCO 2000 results were as follows: mean value for right eyes, 0.324+/-0.305; mean value for left eyes, 0.298+/-0.341; no difference was proved (P=.532). The OSCA results were as follows: for right eyes, 0.7097+/-0.3777; for left eyes, 0.8584+/-0.4323; significant difference (P=.046), worse for left eyes. No correlation between EPCO 2000 and OSCA results was established (P>.001; correlation coefficient, 0.347). BCVA for the right eyes was 0.837+/-0.262 and for the left eyes was 0.849+/-0.224. Neodymium:yytrium-aluminum-garnet capsulotomy was performed in one eye in the NeoSoniX group, and in no eyes in the AquaLase group. CONCLUSIONS: There was no significant difference in PCO measured by EPCO 2000, however, PCO after AquaLase as assessed by EPCO 2000 was slightly denser. The OSCA system gave significantly higher scores in the NeoSoniX group. No correlation between EPCO 2000 and OSCA outcomes was proved.

AIMS: To compare the metrics and surgical outcome when using Infiniti AquaLase and NeoSoniX cataract removal modalities. METHODS: This prospective clinical study involved 50 patients with bilateral cataracts and lens removal using AquaLase in the right eye and NeoSoniX in the left eye. Best corrected visual acuity (BCVA), endothelial cell density and pachymetry were evaluted pre- and postoperatively. Statistical analysis was performed using the Wilcoxon Signed- Rank Test. RESULTS: Preoperative mean pachymetry was 569 +/- 31 mu in the right eye (RE) and 560 +/- 37 mu in the left eye (LE), mean endothelial cell density 2744 +/- 418 cells/mm(2) (RE) and 2730 +/- 472 cells/mm(2) (LE). One week after operation pachymetry was 576 +/- 52 mu (RE) and 583 +/- 72 mu (LE) and endothelial cell density 2388 +/- 586 cells/mm(2) (RE) and 2463 +/- 615 cells/mm(2) (LE). One month after surgery pachymetry was 556 +/- 43 mu (RE) and 559 +/- 44 mu (LE) and endothelial cell density 2368 +/- 52 cells/mm(2) (RE) and 2495 +/- 548 cells/mm(2) (LE). BCVA improved in all eyes and was 0.8 or better on the first postoperative day. CONCLUSIONS: Both the NeosoniX and AquaLase minimize intraoperative damage to ocular structures.
