Ten (2.8%) asymptomatic carriers of HBsAg and four (1.1%) patients with acute hepatitis B virus (HBV) infection were detected among 356 adults with acute viral hepatitis A (HAV) consecutively admitted to the Athens Hospital for Infectious Diseases from May 1981 to March 1984. These patients did not differ in clinical, epidemiologic (except in age), biochemical or serologic characteristics from patients acutely infected with HAV alone. Transient suppression of the HBV replication and disappearance of the HBV DNA accompanied by seroconversion from HBeAg positive to anti-HBe positive were detected in one and two carriers respectively. The titer of non-class-specific anti-HBc was low (less than or equal to 10(-2)) in all cases. These data suggest that superinfection of HBsAg carriers with HAV does not cause more severe disease or influence adversely the course of chronic hepatitis B disease. However, accurate diagnosis of double infections is necessary for prognosis of the liver disease and appropriate management of the patient's environment. This is quite important in areas with a high prevalence of HBV infections, like Greece, where double infections are relatively common.

The risk of developing the chronic hepatitis B surface antigen (HBsAg) carrier state after primary acute hepatitis B virus infection was examined in a prospective study of adults with acute icteric (clinically apparent) viral hepatitis. Enzyme immunoassay for IgM antibody to hepatitis B core antigen (IgM anti-HBc) was positive in 176 (93.1%) and negative in 13 (6.9%) of 189 patients positive for HBsAg and negative for IgM antibody to hepatitis A virus. The former were considered to be acute hepatitis B infections and the latter chronic carriers superinfected with another virus or acute exacerbation of chronic infection. At six months, all surviving acute hepatitis B patients had cleared HBsAg. All surviving chronic carriers remained HBsAg-positive. In another prospective study, 18 adults with subclinical hepatitis B infection also cleared HBsAg within six months. These data suggest that in contrast to newborns and children, adult patients rarely develop chronic antigenemia after acute hepatitis B infection. The postulated risk should mainly be attributed to misdiagnosis of cases of superinfection of chronic carriers or acute exacerbation of chronic infection.

The susceptibility to autoxidation of red cell lipids was studied before and after transformation of normal red cells to PNH-like erythrocytes. The transformation was effected by treatment of the red cells with the sulfydryl compounds D-penicillamine (DP) and N-acetyl-L-cysteine (NAC). The autoxidation was induced by incubating the cells with H2O2 and was estimated by measuring the generated malonyl dialdehyde. The susceptibility to autoxidation was significantly higher in DP-treated cells, while the opposite was true for NAC-treated cells. However, both DP- and NAC-treated cells showed a similar sensitivity to lysis by acid serum and about the same degree of acetylcholinesterase (AChE) activity decrease, thus indicating that the susceptibility to autoxidation of lipids is not involved in the determination of complement sensitivity or in the AChE activity decrease of the sulfydryl-treated cells. Finally, since, as evidenced from most of the reported cases in the literature, increased susceptibility to autoxidation is a feature of PNH cells, it seems reasonable to suggest that DP-treated cells should be used in preference to NAC-treated cells as a laboratory substitute for PNH cells.

RESUMO

One hundred non-obese and non-diabetic individuals, 50 men and 50 women, were submitted to oral glucose tolerance tests performed in the morning and in the afternoon on different days. Although great individual variations were noted in both sexes, in men the mean blood sugar (BS) values did not significantly differ between the two tests; in women, however, the mean values after glucose loading were significantly higher during the afternoon test. By dividing the test population into those aged 50 and below and those over 50 it was found that only the group of younger women showed significant differences between the two tests with higher mean BS values during the afternoon test.

RESUMO

Brucellosis was diagnosed in two polytransfused, thalassaemic patients who had received blood containing brucella agglutinins from an asymptomatic donor. In one of the patients the onset of the disease simulated infectious mononucleosis. Despite the fact that both cases were splenetomized, the course of illness was not life threatening. However, a tendency for recurrence of the infection and persistence of high brucella agglutinin titres were noted.
