RESUMO

INTRODUCTION: Research utilising artificial intelligence (AI) and cardiovascular magnetic resonance (CMR) is rapidly evolving with various objectives, however AI model development, generalisation and performance may be hindered by availability of robust training datasets including contrast enhanced images. METHODS: NotIs CMR is a large UK, prospective, multicentre, observational cohort study to guide the development of a biventricular AI scar model. Patients with ischaemic heart disease undergoing clinically indicated contrast-enhanced cardiac magnetic resonance imaging will be recruited at Nottingham University Hospitals NHS Trust and Mid-Yorkshire Hospital NHS Trust. Baseline assessment will include cardiac magnetic resonance imaging, demographic data, medical history, electrocardiographic and serum biomarkers. Participants will undergo monitoring for a minimum of 5 years to document any major cardiovascular adverse events. The main objectives include (1) AI training, validation and testing to improve the performance, applicability and adaptability of an AI biventricular scar segmentation model being developed by the authors and (2) develop a curated, disease-specific imaging database to support future research and collaborations and, (3) to explore associations in clinical outcome for future risk prediction modelling studies. CONCLUSION: NotIs CMR will collect and curate disease-specific, paired imaging and clinical datasets to develop an AI biventricular scar model whilst providing a database to support future research and collaboration in Artificial Intelligence and ischaemic heart disease.

OBJECTIVE: Tricuspid annular plane systolic excursion (TAPSE) represents movement of the lateral annulus of the tricuspid valve toward the apex and denotes right ventricular contractility in the long axis. In transesophageal echocardiography with the probe in the mid-esophagus, TAPSE occurs at a significant angle to the M-mode scan line. The authors describe a novel method to assess TAPSE in 2 dimensions (2D) and compare this with an established method. DESIGN: Prospective cohort study. SETTING: Tertiary cardiac unit. PARTICIPANTS: Forty-three patients scheduled for outpatient transesophageal echocardiography. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: TAPSE was assessed from the transthoracic apical window using M-mode (M-TAPSE) and the mid-esophageal 4-chamber view (2D-TAPSE). To assess 2D-TAPSE, the authors placed an annotation arrow at the junction of the tricuspid annulus and right ventricular free wall during diastole, and a second arrow at this junction at end-systole; 2D-TAPSE represented the distance between the arrows. The mean M-TAPSE was 20.5 (5.9) mm and 2D-TAPSE was 20.5 (5.8) mm. The mean angle between the direction of the lateral tricuspid annular motion and the M-mode scan line from the transthoracic apical window was 19.4 (7.6) degrees and from the mid-esophageal 4-chamber view 62.6 (15.7) degrees (p<0.001). There was a close correlation between M-TAPSE and 2D-TAPSE; r2 = 0.93. Ninety-five percent of 2D-TAPSE were -3.2mm to+3.1mm of M-TAPSE (95% confidence interval -4.0 mm to+4.0 mm). The positive predictive value of 2D-TAPSE<17 mm to predict M-TAPSE<17 was 83%. The negative predictive value of 2D-TAPSE≥17 mm to predict M-TAPSE≥17 was 93%. The intra- and inter-observer variability for 2D-TAPSE was 6.7% and 9.7%, respectively. CONCLUSIONS: Transesophageal 2D-TAPSE appeared to be a reliable and simple method to assess right ventricular function, and demonstrated a close correlation with transthoracic M-TAPSE.

BACKGROUND: There are limited data on outcomes of patients with previous coronary artery bypass grafting (CABG) presenting acutely as ST-segment elevation myocardial infarction (STEMI) and undergoing primary percutaneous coronary intervention (PPCI). OBJECTIVES: To compare outcomes in STEMI patients undergoing PPCI with or without previous CABG surgery. METHODS: An all-comer single-centre observational registry from a cardiothoracic centre in UK. All consecutive patients presenting for PPCI between 2007 and 2012 were included. Electronic records were used to extract relevant information. Mortality data were obtained from the Office of National Statistics. Overall median follow-up period was 1.7 years (intraquartile range 0.9-2.5). RESULTS: Complete data were available for 2133 (97%) patients. 47-patients had previous history of CABG. Out of these, the infarct related artery (IRA) was native vessel in 22 and graft in 25 patients. Post re-vascularization TIMI flow was inferior in CABG cohort (

RESUMO

A diagnosis of myocardial infarction is made using a combination of clinical presentation, electrocardiogram and cardiac biomarkers. However, myocardial infarction can be caused by factors other than coronary artery plaque rupture and thrombosis. We describe an interesting case presenting with hypertensive emergency and type 2 myocardial infarction resulting from Pheochromocytoma associated with Capnocytophaga canimorsus infection from a dog bite. We also review current literature on the management of hypertensive emergency and Pheochromocytoma.

RESUMO

Congenital quadricuspid aortic valve (QAV) is a rare cardiac anomaly. Several different anatomical variations of a quadricuspid aortic valve have been described. Aortic regurgitation is the predominant valvular dysfunction associated with QAV and patients tend to present in their 5(th) or 6(th) decade of life. This anomaly is rarely picked up by transthoracic echocardiogram (TTE). A comprehensive transoesophageal echocardiography (TOE) study is more likely to diagnose it. We describe a very rare type of QAV - Type F in a 52-year-old lady who presented with symptoms of shortness of breath and pre-syncope. We include TOE images and intra-operative valve images.

RESUMO

The need to consider myocarditis as one cause of flu-like symptoms in patients taking clozapine should be well entrenched. Suspicion should be heightened if the symptoms develop during the first 6-8 weeks of therapy. The FDA and the drug's manufacturer have strengthened warnings to include that a potentially fatal myocarditis may occur when taking clozapine. We aim to highlight the side effects of clozapine and increase awareness of this condition associated with the usage of the drug.
