We investigated the differences in functional connectivity based on the source-level electroencephalography (EEG) analysis between stroke patients with and without post-stroke epilepsy (PSE). Thirty stroke patients with PSE and 35 stroke patients without PSE were enrolled. EEG was conducted during a resting state period. We used a Brainstorm program for source estimation and the connectivity matrix. Data were processed according to EEG frequency bands. We used a BRAPH program to apply a graph theoretical analysis. In the beta band, radius and diameter were increased in patients with PSE than in those without PSE (2.699 vs. 2.579, adjusted p = 0.03; 2.261 vs. 2.171, adjusted p = 0.03). In the low gamma band, radius was increased in patients with PSE than in those without PSE (2.808 vs. 2.617, adjusted p = 0.03). In the high gamma band, the radius, diameter, average eccentricity, and characteristic path length were increased (1.828 vs. 1.559, adjusted p < 0.01; 2.653 vs. 2.306, adjusted p = 0.01; 2.212 vs. 1.913, adjusted p < 0.01; 1.425 vs. 1.286, adjusted p = 0.01), whereas average strength, mean clustering coefficient, and transitivity were decreased in patients with PSE than in those without PSE (49.955 vs. 55.055, adjusted p < 0.01; 0.727 vs. 0.810, adjusted p < 0.01; 1.091 vs. 1.215, adjusted p < 0.01). However, in the delta, theta, and alpha bands, none of the functional connectivity measures were different between groups. We demonstrated significant alterations of functional connectivity in patients with PSE, who have decreased segregation and integration in brain network, compared to those without PSE.

Previous studies have reported the association between myocardial infarction (MI) and air pollution (AP). However, limited information is available regarding the long-term effects of AP on the relative incidence rates of ST-elevation MI (STEMI) and Non-ST-elevation MI (NSTEMI). We investigated the association between long-term exposure to AP and the incidence of STEMI. Between January 2006 and December 2015, a total of 45,619 eligible patients with Acute Myocardial Infarction (AMI) were enrolled in the Korea Acute MI Registry (KAMIR) and KAMIR-National Institutes of Health. Mixed-effect regression models were used to examine the association between the annual average ambient AP before MI onset and the incidence of STEMI, and to evaluate the association of AP with the incidence of in-hospital cardiogenic shock. After mixed-effect regression model analysis, particulate matter (PM) 10 µm or less in diameter (PM10) was associated with increased incidence of STEMI compared with NSTEMI (odds ratio [OR] 1.009, 95% Confidence Interval [CI] 1.002-1.016; p = 0.012). For in-hospital cardiogenic shock complication, PM10 and SO2 were associated with increased risk, PM10 (OR 1.033, 95% CI 1.018-1.050; p < 0.001), SO2 (OR 1.104, 95% CI 1.006-1.212; p = 0.037), respectively. Policy-level strategies and clinical efforts to reduce AP exposure are necessary to prevent the incidence of STEMI and severe cardiovascular complications.

Sarcopenia is as a non-traditional risk factor for atherosclerotic cardiovascular disease. Further investigation is required to elucidate the prognostic significance of computed tomography (CT)-based sarcopenia assessment in coronary artery disease (CAD). We prospectively enrolled 475 patients, who underwent coronary stent implantation and peri-procedural CT scans within one month. Skeletal muscle index (SMI) was assessed cross-sectionally at the first lumbar vertebra (L1) level. The participants were grouped based on sex-specific L1 SMI quartiles. The primary endpoint was all-cause mortality, and the secondary composite endpoint was major adverse cardiovascular events (MACEs) over a 3-year follow-up period. Three-year all-cause mortality and MACE incidence increased significantly in patients in the lower L1 SMI quartiles compared to those of patients in the higher quartiles (p < 0.001). The individual composite endpoints consistently showed a higher incidence in the lower quartiles of L1 SMI (p < 0.001). In multivariable analysis, the lower L1 SMI quartiles independently predicted 3-year all-cause mortality and MACEs (lowest vs. highest quartiles, respectively: OR 4.90 (95% CI 1.54-15.5), p = 0.007; and OR 12.3 (95% CI 4.99-30.4), p < 0.001). In conclusion, CT-based L1 SMI demonstrated a distinct dose-dependent relationship with future MACEs in CAD patients undergoing percutaneous coronary intervention, thereby enhancing cardiovascular risk stratification.

BACKGROUND: We compared limbic structure volumes and graph theory parameters of the limbic covariance network between patients with transient global amnesia (TGA) and healthy controls, and between patients with single and recurrent TGA events. METHODS: We retrospectively enrolled 122 patients with TGA (single event, n = 107; recurrent events, n = 15) and 50 healthy controls who underwent three-dimensional T1-weighted MRI imaging of the brain. Volumetric analysis of the subcortical limbic structures, including the hippocampus, amygdala, thalamus, mammillary body, hypothalamus, basal forebrain, septal nuclei, fornix, and nucleus accumbens, was performed. We examined the limbic covariance network using a graph theory. RESULTS: Limbic structure volumes did not differ between patients with TGA and healthy controls, and between patients with a single event and those with recurrent events. However, the radius of the limbic covariance network was significantly greater in patients with TGA than in healthy controls (6.595 vs. 4.564, p = 0.040). Furthermore, the radius, diameter, eccentricity, and characteristics path length were greater (4.066 vs. 2.000, p = 0.009; 7.062 vs. 3.645, p = 0.029; 5.633 vs. 2.774, p = 0.013; 3.373 vs. 1.688, p = 0.004; respectively), whereas the average strength, global efficiency, local efficiency, mean clustering coefficient, transitivity, and small-worldness index were lower (5.595 vs. 10.831, p = 0.004; 0.350 vs. 0.642, p = 0.002; 0.531 vs. 1.724, p = 0.004; 0.304 vs. 0.624, p = 0.006; 0.456 vs. 0.935, p = 0.003; 0.913 vs. 0.993, p = 0.017; respectively), in patients with recurrent events than in those with a single event. CONCLUSION: The limbic covariance network shows significant alterations in patients with TGA, as well as differences between patients with recurrent events and those with a single event. These findings suggest that changes in the limbic covariance network could be related to the pathogenesis of TGA.

RESUMO

This essay provides a critical observation of the South Korean government's distinctive management of COVID-19 with particular reference to the state of emergency. It reveals that the success of South Korea's handling of the pandemic is largely attributed by a majority of Western media to the efficient deployment of both information and communication technologies and Confucian collectivism, two components that seem contradictory yet not incompatible under the rubric of techno-Orientalism. Analyzing the intensification of surveillance and the rapid datafication of society, this essay argues that the current state of emergency is not a breakdown of normality but a continuation of the state of crisis and disaster that rules a developing country like South Korea. In doing so, the essay seeks to facilitate a critical discussion about a new mode of democracy in the era of pandemic that increasingly grapples with tensions between individual freedom and public health.
