RESUMO

Orthotopic advanced hepatic tumor resection without precise location and preoperative downstaging may cause clinical postoperative recurrence and metastasis. Early accurate monitoring and tumor size reduction based on the multifunctional diagnostic-therapeutic integration platform could improve real-time imaging-guided resection efficacy. Here, a Near-Infrared II/Photoacoustic Imaging/Magnetic Resonance Imaging (NIR-II/PAI/MRI) organic nanoplatform IRFEP-FA-DOTA-Gd (IFDG) is developed for integrated diagnosis and treatment of orthotopic hepatic tumor. The IFDG is designed rationally based on the core "S-D-A-D-S" NIR-II probe IRFEP modified with folic acid (FA) for active tumor targeting and Gd-DOTA agent for MR imaging. The IFDG exhibits several advantages, including efficient tumor tissue accumulation, good tumor margin imaging effect, and excellent photothermal conversion effect. Therefore, the IFDG could realize accurate long-term monitoring and photothermal therapy non-invasively of the hepatic tumor to reduce its size. Next, the complete resection of the hepatic tumor in situ lesions could be realized by the intraoperative real-time NIR-II imaging guidance. Notably, the preoperative downstaging strategy is confirmed to lower the postoperative recurrence rate of the liver cancer patients under middle and advanced stage effectively with fewer side effects. Overall, the designed nanoplatform demonstrates great potential as a diagnostic-therapeutic integration platform for precise imaging-guided surgical navigation of orthotopic hepatic tumors with a low recurrence rate after surgery, providing a paradigm for diagnosing and treating the advanced tumors in the future clinical translation application.

PURPOSE: To investigate the morphological and functional reconstruction of the macular fovea after autologous neurosensory retinal transplantation for recurrent macular hole retinal detachment (MHRD) in highly myopic eyes. METHODS: Ten consecutive cases of recurrent MHRD with high myopia were retrospectively reviewed. All eyes underwent pars plana vitrectomy combined with autologous neurosensory retinal transplantation and were followed up for at least 3 months after silicone oil extraction. The main outcomes were whether or not the retina was reattached and the macular hole (MH) was closed, morphological changes in the retinal graft, best-corrected visual acuity (BCVA), the sensitivity threshold and blood flow signal in the macula. RESULTS: At the one month postoperative visit, there was an obvious boundary between the graft and the surrounding retinal tissue, and some retinal structural layers could be seen in the graft on optical coherence tomography scans. At the final follow-up, eight eyes (80%) showed retinal reattachment and closure of the MH. Optical coherence tomography revealed blurring of the boundary between the graft and surrounding retinal tissue and that the retinal structure in the graft was disordered. The MH was not closed in two eyes (20%), in one case because of partial displacement of the graft and in the other because of incomplete coverage of the MH as a result of a smaller graft. The post-BCVA was significantly better than the pre-BCVA (1.32 ± 0.33 versus 2.01 ± 0.29 logMAR; p = 0.000, paired t-test). CONCLUSION: Autologous neurosensory retinal transplantation can be an effective treatment for recurrent MHRD in highly myopic eyes. 'Fusion' between the neurosensory retinal graft and the original retinal tissue may be the mechanism involved in the closure of the MH and reconstruction of the macular fovea.

To simultaneously determine three components of aconitine, mesaconitine, and hypaconitine in six species of Aconitum genus, an extraction condition for the total alkaloids was specifically optimized and a simple analytical method of reversed-phased highperformance liquid chromatography (HPLC) was developed. The extraction rate of total alkaloids in A. szechenyianum Gay was 98.3% for repeated extracting three times with an acidic alcohol solution (alcohol: pH 3.0 HAc = 85:15, v/v). The chromatography was carried out on a Phenomenex Luna C(18) column by gradient elution with a mobile phase of 0.03 mol/mL ammonium bicarbonate (pH = 9.50) -acetonitrile at a flow rate of 1.0 mL/min. The method for all three alkaloids had good linear relationships (r > 0.999) in the concentration range of 1.0-200.0 µg/mL. The average recoveries were 96.6-103.1%, and the LOQ and LOD were in the range of 25-37 ng/mL and 9-12 ng/mL, respectively. The quantitative results indicated that contents of the three alkaloids varied significantly among crude aconite roots, so quality control of traditional Chinese medicines containing aconite roots should be taken into account.

RESUMO

A novel method of cell affinity screening (CAS), cell affinity capture coupled with LC-MS analysis, was developed for screening the bioactive compounds related to cardiovascular diseases from the natural product libraries. One of the major characteristics lies in its function in affinity-capturing and separating the bioactive components from the natural product libraries in vitro. Another characteristic is its use in analyzing and identifying the target compounds, by employing high-performance liquid chromatography and mass spectrometry. CAS was used for screening the bioactive components from the alkaloid extract derived from Aconitum szechenyianum Gay. Of the five components found to be bound to the oxidative-damaged endothelial cells, the two compounds identified, mesaconitine and aconitine, were recognized in the literature as being related to cardiovascular diseases.

OBJECTIVE: To optimize the conditions for extractions of total alkaloids and aconitine from Aconitum szechenyianum Gay. METHODS: Using L16 (4(5)) orthogonal test and univariant methods, the exrtaction conditions of total alkaloids aconitine in Aconitum szechenyianum Gay. were optimized under the guidance of the content determination of total alkaloids with bromophemol blue colorimetry and aconitine with HPLC method, respectively. RESULTS: The obtained optimum condition of total alkaloids was that the material was refluxed in 6 times (m/v) acidic alcohol solution( alcohol: pH 3.0 HAc = 85:15, v/v) for 1 h with 3 times. The condition of aconitine was that the material was refluxed in 4 times (m/v) acidic alcohol solution (alcohol: pH 3. p HAc = 15:85, v/v) for 0.5 h with 3 times. Contents of total alkaloids and aconitine in Aconitum szechenyianum Gay. were 0.980% and 0.109%, respectively. CONCLUSION: Through the test and verify, the optimum extraction methods of total alkaloids and aconitine are rational.
