RESUMO

Ultrasonic assisted liposuction is a new and highly advanced technology that uses sound waves to emulsify body fat thereby allowing an efficient removal compared with the mechanical disruption of fat used in traditional liposuction. Safe application requires an exceptionally sophisticated plastic surgical, anesthetic, and operating room team. The media bombards patients with "magic wand" medical technology but seems less inclined to portray limitations, complications, and deaths. As with any emerging technology, ultrasonic assisted liposuction is producing its share of problems. This article provides an overview of this procedure and guidelines for patient selection. A conservative approach is emphasized for safety.

RESUMO

In a prospective, double-blind, controlled study on 26 consecutive patients who underwent suction lipectomy, the injection of epinephrine (1:250,000, 1:500,000, or 1:1,000,000) was not found to decrease fluid/blood loss when compared with saline injection or no injection at all. Since our study fails to support the use of epinephrine to lessen fluid/blood loss during suction lipectomy, we have abandoned its use in that procedure.

RESUMO

Blood loss usually is not clinically significant during liposuction, but the need for transfusion increases with the amount of tissue removed. Autologous blood donations in preparation for significant blood loss are widely accepted by patients and physicians.

RESUMO

"Pacifier polydactyly" is defined in babies with a large edematous postaxial duplication attached by soft-tissue stalks that contain neurovascular elements. These serve in utero as pacifiers. Imminent ischemic necrosis was induced in all three by local trauma. Simple excision is the treatment of choice.

Lipomas constitute the most common soft-tissue tumors [11] and may occur sporadically or as one of several inherited disorders. Benign fatty tumors can be classified into three major categories: solitary lipomas, familial multiple lipomatosis, and congenital diffuse lipomatosis [4]. The latter has also been referred to as multiple symmetric lipomatosis, Madelung's disease, or Launois-Bensaude syndrome. Within the categories a more vascular varient of the lipoma, the angiolipoma, has been described separately by some authors, but in many reports it is not distinguished from a lipoma. Liposuction has been used for the treatment of solitary lipomas, but to our knowledge liposuction has not previously been reported for excision of angiolipomas or in familial cases involving multiple lipomas as described here.

RESUMO

A case of hemangioma of the nasal bones is reported. Clinical and radiologic findings, including CT scan, are presented and the literature reviewed. Although rare, the lesion often has a characteristic clinical and radiologic presentation that can be recognized preoperatively. CT scanning is helpful in defining tumor characteristics and extent. Surgery appears curative in most cases without significant disfigurement. For smaller lesions, bone graft of the defect appears unnecessary and the presence of intact periosteum may actually contribute to regeneration of normal bone.

RESUMO

Neurofibromatosis includes the common "peripheral" form and a recently documented "central" form. We describe the central form in 130 cases from 9 kindreds personally studied and 15 reported kindreds. Central neurofibromatosis with bilateral acoustic neuroma is an autosomal dominant disorder beginning about 20 years of age, accompanied by mild skin changes. In three kindreds with central neurofibromatosis, we measured nerve growth factor in serum by radioimmunoassay and radioreceptor assay. Only the antigenic activity of nerve growth factor was increased. In contrast, in peripheral neurofibromatosis, only the functional activity of nerve growth factor has been reported increased. Central and peripheral forms of neurofibromatosis are closely related but discrete diseases which appear to have separate alterations in nerve growth factor activity.
