RESUMO

The retinopathy (microaneurysm/small dot hemorrhage) is an early and specific biological indicator to quantitatively evaluate the CS2 exposure. The appearance of retinal lesions was observed among Yugoslavian, German and American workers exposed to CS2. However, among Finnish CS2 workers a positive result was not obtained. We suggested a different response to CS2 exposure between two ethnic populations. We had an opportunity to do a cross-sectional medical and occupational hygiene survey in a Chinese rayon staple plant. Cross-sectional medical examinations failed to show any chronic CS2 effects on the Chinese workers.

Erythrocyte deformability and other hematological indicators were determined in 17 male workers exposed to lead at a secondary lead refinery and 13 controls. Blood lead, urine lead, urine coproporphyrin, delta-aminolevulinic acid and erythrocyte zinc protoporphyrin were determined to evaluate the degree of lead exposure in the lead workers above. For the measurement of erythrocyte deformability, the microfilter method was used. The results were summarized as follows: 1. The mean values of blood lead, urine lead, urine coproporphyrin, urine delta-aminolevulinic acid and erythrocyte zinc protoporphyrin levels in lead workers were 53.5 micrograms/100g, 141.4 micrograms/l, 115.9 micrograms/l, 12.0 mg/l and 68.9 micrograms/dl respectively, suggesting a moderate influence of lead exposure. 2. The mean values of erythrocyte count, hematocrit and hemoglobin were significantly lower in lead workers than those in controls. No significant differences were found in the mean values of mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration and corpuscular natrium and potassium between lead workers and controls. 3. Erythrocyte deformability was significantly reduced in lead workers compared with controls.

The effect of ultraviolet rays on eyes was studied. An ultraviolet laser beam (XeCl 308 nm) was irradiated to rabbit eyes to investigate morphological changes of the lens fibers and molecular structural changes of the lens proteins at immediately, 15 days and 32 days after irradiation. 1. Scanning electron microscopic findings of the lens The lens fibers in the control eye were regularly and densely arranged in neat layers of hexahedrons. In the irradiated eye, the fibers at areas that seemed injured were disrupted and the disrupted ends were spherical-shaped. 2. Raman spectra of the lens 1) The relative intensity ratio (I825/I850) of the Raman scattering for the tyrosine residue and the relative intensity ratio (I3390/I2960) of the OH stretching mode (3390 cm-1) of water contained in the lens cortex showed slight changes in the irradiated eye compared to the control eye. 2) The relative intensity ratio (I2572/I2721) of the SH stretching mode (2572 cm-1) for the cysteine residue was 1.05 in the irradiated eye at 32 days after irradiation. It was lower than that in the control eye (1.22 +/- 0.08). This suggested the synthesis of S-S binding by oxidization.

Rabbit eyes were irradiated by an ultraviolet laser beam in with to investigate its effects on the corneal endothelium, thickness of the cornea, and lens. An Excimer laser beam (gas; XeCl) with wave length of 308 nm and with energy intensity of 0.02, 0.07, 0.2, 0.6, 1.7, 3.0, 4.0, or 5.1 J/cm2 was irradiated to the right eye using 3 rabbits for each energy intensity (24 rabbits in all) in order to examine changes occurring in the right eye compared with the left eye from immediately after irradiation to 32 days thereafter. The results obtained were as follows: 1. Corneal thickness UV laser beam irradiation at 0.02, 0.07, and 0.6 J/cm2 caused immediate thickening of the cornea (p less than 0.01). The cornea thickness returned to normal 14 days later. Irradiation at 3.0 and 4.0 J/cm2 resulted in remarkable thickening, which returned to normal 28 days after irradiation. By irradiation at 5.1 J/cm2, the corneal thickness increased markedly (0.333 +/- 0.019 mm in control eyes and 0.405 +/- 0.05 mm in irradiated eyes) immediately after irradiation. It tended to gradually return to normal and was reduced to the pre-irradiation thickness on the 32nd day after irradiation. 2. Corneal endothelial area and morphology (the number of angles) The area of the corneal endothelium increased after irradiation. However, no significant differences were noted at any energy intensity, because of the larger scattering of the values in the control eyes. The ratio of hexagonal cells tended to decrease even by energy intensities above 0.7 J/cm2. In order to measure the degree of ocular fatigue and general fatigue among the workers using visual display terminal (VDT), pupillary dynamics were measured with open-loop light stimulation for 26 subjects of a mean age of 25.9 years. VDT loads were TV games for 30 min under indirect light of 100 lux. The eight parameters of pupillary light reflex with open-loop pupillometer were examined before and after VDT loads. On the right eyes, two parameters and one value increased 9.2% (Student's t-test: P less than 0.005, max.), 25.7% (Student's t-test: P less than 0.005, Min.) and 14.8% (Student's t-test: P less than 0.005, min/max) just after the load and then gradually decreased and returned to the pre-load level after a 10 to 15-min rest period. Two parameters increased 11.7% (Student's t-test: P less than 0.05, 1/2C) and 12.9% (Student's t-test: P less than 0.05, VD), one parameter decreased -9.0% (Student's t-test: P less than 0.05, 0.63D) and three parameters were constant (Student's t-test: L, A and VC) just after the load. On the left eyes, the same tendencies were observed. But there were no significant differences (Student's t-test) between the right and left eyes. The change pattern of the eight parameters were small in degree and similar to the changes when the sympathetic nervous system was activated.

After nitrite treatment, various kinds of pickled vegetables and sun-dried fishes produced in Japan showed direct-acting mutagenicity on Salmonella typhimurium TA100, inducing 1900-18000 revertants/g. Kimchis, sun-dried fishes, sun-dried squid, soy sauces, fish sauces, bean pastes and shrimp paste produced in Korea, the Philippines and Thailand also showed direct-acting mutagenicity after nitrite treatment. All soy sauces and fish sauces tested contained as much tyramine as 17-1020 micrograms/ml, but very low or undetectable amounts of (-)-(1S,3S)- and (-)-(1R,3S)-1-methyl-1,2,3,4-tetrahydro-beta-carboline-3-carboxylic acids.

To clarify the relationship between morphological changes in the corneal endothelium and ultraviolet (UV) radiation, specular microscopic examinations were performed on both eyes of 118 welders and 85 controls. The results showed: a decrease in the hexagonal cells in welders (20-29 years) in comparison with the controls (20-29) (p less than 0.05); an increase in the mean cell size of the endothelium and a decrease in the hexagonal cell population with increasing age in both groups; increases in standard deviation (SD) and the coefficient of variation (CV) of the mean cell size in both groups; increases in SD and CV of the mean number of cell cell sides in both groups; and no difference in the mean cell size between the two groups. These results show that UV radiation damages not only the corneal epithelium but also the endothelium, and suggest that it causes more pleomorphic change (a decrease in hexagonal cell population) than enlargement of the mean cell size.

RESUMO

Pterygium is thought to be hyperaemia of fibrovascular tissue on to the cornea caused by ultraviolet radiation of sunlight. We observed a significantly high incidence of pterygia in welders who were exposed occupationally to excess ultraviolet radiation and found a close relationship between the incidence and the length of employment as a welder (r = 0.975, p less than 0.05). This study attempts to clarify the causative relationship between ultraviolet radiation and pterygia.

To assess long-term vascular effects of occupational carbon disulfide (CS2) exposure, we examined 376 exposed and 325 unexposed Chinese workers with a medical and work-history questionnaire, a chest-pain questionnaire, blood pressure and blood cholesterol measurements, routine urinary tests, monochromatic fundus photography and other tests. Data from 354 exposed and 177 age- and sex-matched reference workers were evaluated. Personal exposure monitorings were performed simultaneously with a passive dosimeter using gas chromatography. Monochromatic fundus photography revealed no CS2-retinopathy characterized by microaneurysms and/or small dot hemorrhages, and no effects of CS2 on blood pressure and blood cholesterol were found. These negative results may have been due to the low daily exposure averages, which varied between 0.01 and 12.8, with a mean of 1.5 ppm. The present study suggested that the no-effect level of CS2 exposure on the development of retinopathy was around 2 ppm (8-h time weighted average), and the absence of adverse CS2 effects among the workers studied supported the current standard for occupational CS2 exposure of 10 mg m-3 in China.

To clarify the pathological difference of retinal changes between carbon disulfide (CS2) and diabetic retinopathies, fluorescein fundus angiograms of 92 workers with CS2 retinopathy (Group I) were compared with those of 37 cases with diabetic retinopathy (Group II) and 40 workers who had not been exposed to CS2 (Group III). The results were: 1) Ellipsoid or loop-shaped microaneurysms were observed in 25% of Group I, and in 5% of Groups II and III. 2) Fluorescein leakage from vessels was observed in Group II, but not in Groups I and III. 3) An avascular area of the retina was observed in Groups I and II. The avascular area in CS2 retinopathy was small and local and showed no tendency of enlarging. 4) Atrophic/degenerative changes of the pigmentary epithelium were observed in 40% of Group I, 11% of Group II and 15% of Group III.

Erythrocyte Na+,K+-ATPase activity increased significantly in lead workers of a lead refining factory when measured with EDTA and compared to the controls without EDTA. The enzyme activity measured with EDTA increased in the following order: controls less than office workers in a lead refining factory less than lead workers. A positive correlation existed between blood lead and enzyme activity with EDTA (r = 0.380, p less than 0.10), and the activity without EDTA (r = 0.398, p less than 0.05). A negative correlation was found between sodium in erythrocytes and enzyme activity with EDTA (r = -0.437, p less than 0.05), and the activity without EDTA (r = -0.416). But no relationship was observed between enzyme activities and potassium in erythrocytes. A positive correlation between enzyme activity with EDTA and that without EDTA was observed (r = 0.452, p less than 0.05). With addition of lead to fragments of erythrocyte membranes, a significant decrease occurred in the activity of the enzyme without EDTA, whereas no change was observed with EDTA. No significant change occurred in the enzyme activity with and without EDTA upon addition of lead to blood. The maximum level of lead in membrane fragments (lead combined with membranes) of workers exposed to lead was 0.60 microgram/mg protein, and that in the experiment of addition to blood was 7.0 micrograms/mg protein.

The effect of blood lead on erythrocyte membrane proteins was studied in 28 workers from a scrap lead refining factory and in 18 controls working in railway construction. Sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) of the polypeptides in the erythrocyte membrane showed that bands 3 and 4.1 had significantly decreased while bands 2.3, 6, and 7 had significantly increased in the lead workers compared with the controls. For the lead workers, the correlation coefficients between blood lead and bands 2.3 and 3 were r = 0.545 (p less than 0.01) and r = -0.51 (p less than 0.01) respectively. These results suggest that the decrease in erythrocyte membrane permeability results from a decrease in the membrane transfer protein responsible for band 3.
