INTRODUCTION: Myocarditis is one of the most urgent and in many respects unresolved problems in cardiology. The situation is further aggravated by the ever increasing incidence of the of disease. The lack of knowledge about myocarditis etiology and pathogenesis creates a number of difficulties for the choice of the adequate treatment strategies. AIM: The objective of the present study was to evaluate the effectiveness of quantum photohemotherapy (PHT) with the use of a blue light in the combined treatment of infectious-allergic myocarditis (IAM). MATERIAL AND METHODS: We examined a total of 28 patients including 12 women and 16 men at the age from 16 to 45 years suffering from IAM. The diagnosis of IAM was confirmed clinically, biochemically, and functionally with the application of the blood tests, chest X-ray examination, echo- and electro-cardiography. We used the classification of N.R. Paleev and M.A. Gurevich for myocarditis diagnostics. The diagnosis of ischemic heart disease was excluded according to the WHO criteria. The degree of heart failure in the patients was assessed based on the NYHA classification criteria. All the patients were given therapy with nonsteroidal anti-inflammatory drugs, metabolic drugs, anti-arrhythmics, diuretics, and angiotensin converting enzyme inhibitors. In addition, the generally accepted methods for the characteristic of blood rheological properties were employed. Moreover, the universally recognized rehabilitative and therapeutic measures were supplemented by blue light PHT using an AFS-Solaris light-emitting diode for intravascular irradiation of blood through the disposable light guides. RESULTS: The study has demonstrated that IAM was accompanied by a number of hemorheological disorders that collectively make up a component of pathogenesis of the condition in question. The application of blue light PHT for the combined treatment of the patients with IAM led to the improvement in their rheological status and peripheral blood characteristics. CONCLUSION: Various hemorheological disorders were diagnosed in the patients suffering from infectious-allergic myocarditis. The main effect of blood viscosity reduction documented in the patients with IAM under the influence of blue light PHT was attributable to the reduction in hematocrit decreasing and suppression of the erythrocytes aggregation activity.

Medical fields, in which phototherapy can be used, are specified.

Blood rheology (viscosity of the whole blood and plasm, hematocrit and fibrinogen concentration) was studied in 34 anginal inpatients. All the above indices were found elevated. Medication in 16 patients was combined with photopherapy (PT)--extracorporeal autoblood exposure to blue light (BL). 8 patients' blood was exposed to UV-radiation. Only BL PT produced a positive effect on blood viscosity due, primarily, to hematocrit reduction.

Blood rheology was studied in 108 patients with chronic nonspecific pulmonary disease. It is emphasized that hemorheological disorders should be corrected. Effects of photohemotherapy are analysed.

The paper is concerned with an essential blood characteristic--viscosity which is considered in relation of mechanisms responsible for its normal and abnormal characteristics, its role in the onset of circulatory disorders. High blood viscosity may contribute to development of vascular disturbances. Efficient methods of normalizing high blood viscosity are analyzed with special emphasis on photochemotherapy. Effects of ultraviolet, blue and red laser radiation on blood viscosity are outlined.

The regional haemodynamics was investigated in 39 patients by indirect radionuclide angiography, dopplerography, determination of the rate of elimination of radiodrugs from the leg tissue before, during and after a course of photohaemotherapy. It was shown that rheological properties of blood, lymph circulation, blood flow rate were improved at all stages of chronic insufficiency of lower extremities of the atherosclerotic and inflammatory genesis.

The paper reports glucocorticoid actions in bronchial asthma, covers problems of individual sensitivity to glucocorticoids and resistance to hormones, illustrates the dependence of beclometasone dipropionate efficacy on condition of glucocorticoid receptors in alveolar macrophages and clinical appearance of the disease, describes physiological features of patients with hormone-resistant bronchial asthma and its new treatment (extracorporeal photohemotherapy) allowing reduction of prednisolone dose in 14 of 15 patients from 29.6 to 11.4 mg/day, on the average.

An investigation of blood rheologic properties in 12 patients with acute pneumonia has been made by using low-energy He-Ne laser (LG-79-2) irradiation of vascular fascicle. 37 exposures have been performed, 40 min each. Immediate effects of He-Ne laser were studied comparing blood samples taken before and after the radiation. Results obtained have demonstrated transcutaneous blood irradiation causing prompt and pronounced effects on blood rheologic characteristics: reduced blood viscosity, improved both viscous-elastic properties and osmotic resistance of erythrocytes, activated platelet aggregation. These effects excluding the latter are rather positive for the human body. As for platelet aggregation, its activation following transcutaneous laser irradiation of blood might be expected to grow into a factor of risk provoking pathologic thrombogenesis in venous congestion, hypercoagulation and vascular wall injury.

The results of these experiments demonstrate that extracorporeal ultraviolet and red coherent blood irradiation make the energetics of erythrocyte membrane broken. This deviation has been manifested in changing of erythrocyte's form. The degree of these disorders and of inertia of the recovering process of the erythrocyte's form was greater under ultraviolet blood irradiation.

An analysis of examination and treatment of 36 patients with acute pyo-inflammatory diseases of face and neck has shown the inclusion of UV-irradiated autotransfusions into the complex of therapeutic measures to decrease blood viscosity and to increase deformability of erythrocytes. It elevates the intensity of blood circulation in the system of macro- and microcirculation. The increased arteriovenous oxygen difference after UV-irradiated autotransfusions points to improved utilization of tissue blood and enhanced bioenergetic processes.
