RESUMO

The authors describe a case of spinal intracanal cysticercosis. The cysts were situated intradurally, provoking radicular symptoms: pain, paresis. CT investigations showed herniation of discs L4/L5 and L5/S1 interpreted as the cause of the mentioned symptoms. Surgical treatment included discectomy L4/L5 and L5/S1. Recurrence of symptoms was investigated by means of MRI showing the presence of cysts situated among cauda equina roots. Intraoperative finding--cysticercosis was confirmed by pathologic investigation.

RESUMO

Nucleus pulposus herniation is one of frequent causes in spinal pains. The necessity of undertaking of appropriate treatment, often surgical, requires early establishing of precise diagnosis. The traditional classification of herniations into central, centrolateral and lateral seems to be insufficiently precise, especially from the surgical point of view. For that purpose, the authors, following the classification of Volle et al., applied in their analysis the new classification into central and lateral nucleus pulposus herniae and discerning among the latter ones four types with strictly defined localisation: mediolateral, lateral with recess involvement, intervertebral foramen hernia, outside intervertebral foramen hernia. Seventy-two cases were analysed. Hernia was diagnosed by means of CT. Particular attention was given to the most external hernias, that is into and outside the intervertebral foramen. The more accurate method for the assessment of hernias and bone changes was in CT the Reconstruction in sagittal and frontal planes and also in oblique projections. The authors believe that the diagnosis of lateral nucleus pulposus hernias in low spinal area, particularly those with herniation into and outside the intervertebral foramen should be based on CT in appropriate reconstructions.

RESUMO

Treatment of spinal injuries is a special line in the Dept. of Neurosurgery of Military Academy in Lódz. The authors reviewed the results of surgical treatment of cervical spine injuries in the Dept. between 1982-1987. Follow-up was performed in a group of 49 cases, who vosited the Dept. between 5 to 1 year after surgery. This group of patients underwent detailed clinical analysis in terms of long-term results of treatment. Radiological results were assessed as well. The authors analyzed the relationship between timing of surgical decompression of the spinal cord and clinical improvement of neurological status. The concluded that early decompression in case of incomplete spinal cord injury resulted in better neurological outcome.

RESUMO

The authors present a case of teratoma adultum of unusual intrasellar localisation. The tumour was surgically treated with excellent result. It was confined to sella turcica and partially invaded the suprasellar region. It produced headache, mild bitemporal hemianopsia and did not cause clinically evident hormonal disturbances.

RESUMO

In 1971-1985 surgical treatment was carried out of 29 patients in the Neurosurgery Department WAM in Lódz and 5 in the Rehabilitation Centre in Konstancin for spondylolisthesis complicated with signs of cauda equina damage. In the surgical treatment the technique used was that of a combination of neurosurgical approach (full decompression of the nervous elements in the vertebral canal by laminectomy with foraminectomy or facetectomy, removal of protruding discs and osteophytes and parts of the vertebral bodies narrowing the vertebral canal), and orthopaedic procedures (spondylodesis between vertebral bodies posterior spondylodesis and a combination of both techniques). In one case spring alloplasty was done and in one case Harrington's rods were used. The operation was carried out at one time from the same posterior approach. The follow-up was from 1 to 15 years. A very good result was obtained in 20.6% of cases, good in 55.9%, satisfactory in 23.5%. In no case of worsening or lack of improvement were observed.

RESUMO

After an analysis of cases of tumours situated on the peripheral nerves treated in hospital the authors stress the usefulness of computerized tomography in the diagnosis of these tumours. By demonstrating the internal structure of the tumour, its anatomical relationships to the surrounding tissues the investigation provides many important and practical data in the surgery of peripheral nerves.

RESUMO

Two cases are reported of vascular complications in the cervical rib syndrome. In one case thrombosis developed in the distal part of the brachial artery, and in both cases compression of the subclavian artery could be demonstrated clinically and during arteriography. Attention is called to the necessity of surgical treatment of patients with evidence of vascular compression in view of the possibility of thrombotic complications. Coexistence of several causes of compression and the relationship of good therapeutic result to removal of all causes is stressed.
