[The efficacy of hepatotropic agent Remaxol in oncological patients with postoperative liver dysfunction].

The study of the efficiency and safety of the hepatotropic drug Remaxol in oncological patients with minimal, subclinical manifestations of liver dysfunction in the postoperative period was performed. It is shown that using of Remaxolcontributes to leveling the biochemical liver dysfunction: decrease of the concentration of total bilirubin, a decrease serum activity of lactate dehydrogenase, alanine aminotransferase, aspartate aminotransferase, and alkaline phosphatase. The unwanted effects of Remaxol were not identified.

[Concurrent chemoradiotherapy with moderate dose brachytherapy as remission induction for patients surgically treated for esophageal cancer].

The locally advanced cancer of thoracic esophagus was treated by induction chemo-irradiation therapy (IHRT) with intraluminal medium dose-rate brachytherapy (IMBT). From July 2009 to February 2012 twenty five patients (mean age 54.3 +/- 1.3 years) were included in the study. The length of the primary tumor was up to 6 cm in 10 pts and more than 6 cm in 15 pts. Induction therapy consisted of three IMBT sessions (7 Gy X 3; q7d) and two cycles of chemotherapy (PF; q28d), 26-31 days later the surgery was performed. Subtotal resection of the esophagus type of I Lewis was performed in 23 pts, transtracheal extirpation of the esophagus was performed in 2 pts.In all the cases 3F lymphodissection was performed. After IHRT in 15 of 25 pts. was obtained complete or partial response, in 10 of 25 pts was obtained disease stabilization. Perioperative complications occurred in 17 (68%) patients receiving conservative treatment, one patient (4%) died of treatment complications. In 6 cases (24%) morphology had shown the complete regression of primary tumor. Thus, the combination of intraluminal medium dose-rate brachytherapy and chemotherapy lead to reduction of primary tumor local spread and can be an efficient factor in improving the results of surgical treatment.

[Perspectives of development in anesthesiology and intensive care in oncology].

Despite of anaesthesiology and intensive care progress in oncology there are some problems that need to be solved. These include low individual significance of perioperative risk scale, the lack of approaches for postoperative correction of risk value, estimation of surgical stress value, approach to detection of pain mechanisms etc. The studies which will resolve these problems allow improving the quality of medical care for cancer patients.

[Purine analgesia: experimental results and clinical perspectives].

The high rate of dissatisfaction with analgesia in patients undergoing surgery and by those with chronic pain prompts a search for new pain relief methods. Purine analgesia may considerably ease suffering in these patient groups. This review considers the results of the studies dealing with the analgesic activity of non-selective purine receptor agonists, such as adenosine and adenosine triphosphate, on experimental models and the feasibilities of their clinical application. The paper points to the experimental studies, the results of which are the basis for the clinical use of purines. Much attention is given to the studies using purine analgesia as a component of general anesthesia. It is no less than important that good results of treatment were obtained after administration of purine agonists to patients with neuropathic pain syndromes. Thus, purine analgesia should be regarded as a promising line of the development of anesthesiology.

Cardiopulmonary bypass as a reference flow model for impedance cardiography monitoring is described in 34 children during open-heart surgery. Such an approach not only proved the accuracy and precision of impedance cardiac output measurement, but also gave a possibility to make intra-operative calibration of any cardiac output monitor which maintains its accuracy during the post-bypass period.

[Thermodilution and rheography: the coincidence and reproducibility of results]. / Termodiliutsiia i reografiia: sovpadenie i vosproizvodimost' rezul'tatov.

A comparison of the pulmonary artery thermodilution and tetrapolar impedance cardiography of cardiac output measurements based on 312 dual determinations has shown a good general coincidence, but better reproducibility of the impedance cardiography data and more evident character of the errors and artifacts. The authors' analysis of the mechanisms and causes of such phenomena is based both on their own conclusions and on a wide spectrum of the data of previous investigations.

[Rheographic monitoring of intraoperative hemodynamics]. / Reograficheskii monitoring intraoperatsionnoi gemodinamiki.

The authors summed up the experiences with using noninvasive monitoring of hemodynamics during 420 operations. The method was based on computed procession in the real time of the integral rheogram of the body. The advantages, shortcomings and technological details of the method were analyzed.
