RESUMO

This study investigated the effects of fibrin glue ('Tissucol', Immuno Pty. Ltd.) in the middle and inner ear. Small amounts of the adhesive were used in 36 operations performed on 18 rabbits. The light microscopic examination of these ears four, eight and 12 weeks after surgery showed that the adhesive was well tolerated and that it did not have any toxic effect on the middle ear or membranous labyrinth. The fibrin tissue adhesive can therefore safely be used to facilitate reconstructive middle-ear surgery and, with the aid of fascia, also for the closure of labyrinthine fistulae; or to achieve a definite seal of oval window niche after stapes surgery.

41 patients suffering from severe Menière's disease who were previously unsuccessfully treated with either a saccotomy or intensive medical therapy, were treated with intratympanically applied gentamicin sulphate. 16 mg was given daily and the average total dose was 88.8 mg. In 66% of the patients the vertiginous attacks ceased after treatment and in 17% they were greatly improved. The hearing was preserved in 66% of the patients and actually improved in 17%. In 49% of the patients the sensation of aural pressure was abolished and 34% were relieved of their tinnitus.

Osseous fixation of the malleus is discussed as a cause of combined conductive and sensorineural hearing loss. The characteristic clinical, audiological and histological findings, as well as the aetiology of 66 patients with an idiopathic fixation of the head of the malleus is described. Ninety-four per cent of these patients had a sensorineural component to their hearing loss far greater than the loss that could be attributed to presbyacusis. Idiopathic fixation of the head of the malleus was the second most common of the fixed malleus syndrome in the series of 168 ears operated on in the Ear Nose and Throat Department of the University of Tübingen between 1972 and 1977. The treatment of choice is to keep the ossicular chain intact. Reconstruction of the disrupted chain is preferred in patients with poorly pneumatized mastoids. Only 15% of the patients with idiopathic fixation showed a significant improvement in their bone-conduction after surgery.
