RESUMO

Vaginal leiomyosarcoma is a very rare tumor. The authors report a case of primary presentation in a 51-year-old multipara who complained of the development, since about 12 months previously, of a vaginal swelling accompanied by bloody discharge and pain. The outcome was fatal twenty two months later following treatment combining chemotherapy, radiotherapy, and surgery. The case is discussed in the light of data from the literature.

RESUMO

In a 13-year old girl, an ovarian tumor was suspected of being responsible for causing amenorrhea-galatorrhea. In the absence of any abnormality of the medial umbilical folds, hormonal assessment indicated a high level of prolactin, which was restored to normal after exeresis of the mass. This was an ovarian tumor of the sexual cords with annelated tubules (SCTAT de Scully).

RESUMO

OBJECTIVE: Reevaluation of the usefulness of routine uterine curettage with every cervical cone biopsy. MATERIAL: Retrospective analysis of 250 cervical cone biopsies over an 18 year period. RESULTS: 240 analysable cases were included. 90 patients had undergone cone biopsy only and 150 cone biopsy accompanied by uterine curettage. Curettage material was histologically normal 115 times (76.6%), negative or invalid 26 times (17.3%) and pathological 10 times (6.6%). Pathology consisted of 7 cases of hyperplasia and 3 of endometritis. CONCLUSION: Routine uterine curettage whenever cone biopsy is performed is of no value other than in certain special circumstances concerning clinical status and ultrasound findings.

RESUMO

Ten cases of malignancy of the body of the uterus in women aged 45 and under without risk factors are analysed. These included 4 sarcomas, 4 glandular adenocarcinomas, 1 adenosquamous carcinoma and 1 papillary serious carcinoma. Often advanced and moderately differentiated, these tumours were characterised by their aggressiveness and poor prognosis.

AIM: To determine the incidence of genital hemorrhage, to specify its etiology and define the epidemiological profile of the "hemorrhagic patient". MATERIALS: Retrospective analysis of selected medical case-histories and histological reports for 275 uterine curettages carried out due to a hemorrhagic sign. RESULTS: The incidence was estimated to be 2.7% of consultations. 292 histological lesions were identified, and their severity classified as follows: 53.8% dysfunctional, 20.0% hyperplasia, 5.5% malignant and 20.5% classified as "rare". The profile was that of a perimenopausal, multiparous woman, obese with normal menarche and menstrual cycle. CONCLUSION: In the general population, genital hemorrhage is less and less frequently indicative of a malignant lesion.

RESUMO

OBJECTIVE: Evaluation of the risk of ovarian carcinoma associated with uni or bilateral adnexal preservation following gynecological surgery for benign lesions. MATERIAL: Gynecological surgery history of 190 patients with a primary ovarian carcinoma diagnosed and/or treated in the Navarre Teaching Hospital Group between 1981 and 1991 inclusive. RESULTS: Two patients, i.e. 1%, had undergone conservative surgery prior to the diagnosis of ovarian neoplasia. CONCLUSION: Ovary left in place after conservative surgery undergoes malignant change less than in the population as a whole.

RESUMO

The authors report a case of a primary cervical non-Hodgkin malignant lymphoma. The consultation was the result of post-coital metrorrhagia and leukorrhea. Treatment involved a combination of multiple drug therapy and radiotherapy. Three years later, the patient is in good general health and complete clinical and histological remission.

RESUMO

The authors report a case of primary ovarian non Hodgkin malignant lymphoma in a 41-year-old multiparous woman. An ovarian tumor was diagnosed in a context of hypogastric distention and discomfort. A total hysterectomy was performed with bilateral uterine appendectomy. The intra-abdominal organs were not affected and there were no internal adenopathies. The pathology examination concluded that this was a highly malignant, non-Hodgkin lymphoma. The findings of the extension assessment were negative. Five cycles of drastic chemotherapy combining bleomycin, adriamycin, cyclophosphamide, vincristine, methotrexate, iphosphamide, cisplatyl, VP-16 and ara-C have been administered and will be followed by abdominal radiotherapy. The patient is considered to be in a state of complete remission with a follow-up period of four years.

RESUMO

The authors report a case of advanced rectal cancer in a pregnant woman of 30 years of age. It was a glandular adenocarcinoma discovered in the second trimester of pregnancy. The clinical picture was one of rectal bleeding with digestive disturbances. The patient had a caesarean operation at 29 weeks of amenorrhoea because of a change in her general state. Post-operatively palliative chemotherapy was started. The patient died on the 13th day.

RESUMO

Six cases of hydrometra are reported. The diagnosis could be confirmed only by ultrasonography. Histology revealed two cases of hyperplasia and three of endometrial atrophy. Culture of curettage material remained negative. Blood count was normal.

RESUMO

Catamenial pneumothorax is a clinical variant of spontaneous pneumothorax. Its etiology is attributed either, generally, to endometriosis involving the diaphragm, or to prostaglandin-induced bronchiolar constriction. Clinically, it coincides with the onset of menstruation and resolves during the subsequent days. Treatment is currently based upon progestational agents and antigonadotrophins.

RESUMO

In a young woman, hemiperitoin is nearly always the result of the rupture of an ectopic pregnancy. The authors report the case of the rupture of a cyst of a corpus luteum and they review the literature in order to identify the semiological signs and it highlight the difficulty of reaching a diagnosis by non-invasive methods. The treatment remains surgery, involving ablation of the corpus luteum.

RESUMO

We operated on a patient who had already had total abdominal hysterectomy with bilateral salpingo-oophorectomy who developed a pelvic mass. The Pouch of Douglas was filled with a friable cystic mass of which the anatomo-pathological diagnosis was of a borderline serous cyst adenoma that probably was ovarian in origin. The case is interesting because of the radiopathological hypotheses that are brought up and by the various ways of treatment that have been suggested.

RESUMO

We describe a case of menstrual rectal bleeding due to sigmoid endometriosis. The history led us to the diagnosis and since a small biopsy of the lesion and scanning could not help us to a conclusive diagnosis we carried out histological examination of a piece removed at operation. This case has led us to estimate the incidence, the difficulties of diagnosis and the present therapeutic measures.
