RESUMO

The activity of a regional reanimation-and-consultation center (RCC) related with transportation of 3219 children (79.5% of them were newborns) is summarized in the paper. The share of children assigned for artificial lung ventilation (ALV) increased from 50.8% to 82.6%, while the lethality rate dropped from 12.8% to 6.9%. The above results were enabled by a maximally early re-hospitalization of patients from therapeutic-and-prevention facilities (TPF) with limited medical care abilities and by early preparations (for transportation) by using the DINAR-2 expert system. The children were transported only after the main vitally important systems had been compensated for and with support of an adequate intensive care therapy. Should the above principles be met, the specialized therapy could be started at he earliest possible time; it promoted the therapy quality and cut the patients' stay in intensive care units.

RESUMO

A model of regional network of pediatric resuscitation has been created in the Sverdlovsk region. The model is based on evaluation of hazards, is staged, standard, and makes use of a universal information space. The unifying structure is a pediatric resuscitation consultation center. Its main tasks are therapy and counseling in the region and introduction of overall training in reanimation and intensive care of children, with improvement of the level of urgent care at therapeutic institutions of the region. The activities of the resuscitation care network promoted a significant decrease in neonatal mortality by 29.5% and of infant mortality by 28.7%.

RESUMO

New methods for the diagnosis of the endogenous intoxication syndrome (EIS) in critical states have been developed and tried in 585 infants. A complex of biochemical, hemorheological, biophysical, and optic polarization parameters characterizing the intensity of catabolic reactions and united under the term "endotoxicogram" is offered. Objective assessment of EIS is expressed by an integral index of intoxication which permits a universal assessment of the severity of patient's status, disease course, and efficacy of therapy and helps predict the disease outcome.

RESUMO

The authors present the structure of resuscitation care of children in a large region (Sverdlovsk district) and analyze its efficacy in recent 5 years. A specific feature is the creation of a novel institution: resuscitation consulting center, carrying out therapy and consultations on a wide scale and regular organizational and methodological activity, consisting in improving specialized care both in the center and in other clinics, in upgrading specialists, and improving the equipment of resuscitation departments. An important element of the system is a universal information base for improving the diagnosis of threatened states and making strategic decisions. Proper functioning of all elements of the system will help not only decrease the level of hospital mortality, but the childhood mortality in general as well.

RESUMO

A case of a successful treatment of a patient with an extremely severe tetanus form is described. The management of the patient included massive specific antibacterial and infusion therapy, hemoperfusion, standard and high frequency controlled lung ventilation. The problems of patient activation and respiration recovery have arisen in the course of treatment. They were dealt with, using hemoperfusion that reduced the residual effect of sedative agents and myorelaxants, as well as high frequency jet ventilation, which accelerated respiration recovery.
