RESUMO

This article presents information on the application of apparatus physiotherapy for the treatment of the patients with erectile dysfunction. The authors report the results of the analysis of scholarly research designed to confirm the effectiveness of the physiotherapeutic methods applied for the treatment of the patients presenting with this pathology. Special attention is given to the mechanisms underlying the influence of the procedure with the use of local negative pressure-LOD therapy and shock wave therapy on the main pathogenetic components of erectile dysfunction. In addition, the article provides information about the effectiveness of the modern combined physiotherapeutic techniques including sinusoidal modulated current (SMT) electrophoresis of vasoactive substances and vibromagnetic-laser therapy in the combination with the general purpose methods as exemplified by intravenous laser irradiation. Based on the evidence presented in the article, the authors arrive at the conclusion about the prospects for further research on the development of new physiotherapeutic modalities and their introduction in the practical treatment of the patients with erectile dysfunction.

UNLABELLED: The problem of the development of the new efficient methods for the treatment of the patients presenting with chronic bacterial vesiculitis (CBV) is currently considered among the important priorities. AIM: The objective of the present study was to provide a scientifically sound substantiation for the application of sinusoidal modulated currents (SMC), magnetic fields, and laser radiation in the combined treatment of the patients with CBV. PATIENTS AND METHODS: A total of 121 patients presenting with chronic bacterial vesiculitis were examined and treated during the latent phase of the inflammatory process. They were randomly divided into three groups. Group 1 (main) was comprised of 40 patients treated, in addition to basal pharmacotherapy, by supravascular contact laser irradiation of the cubital vein area followed after 2-3 hours by the application of sinusoidal modulated currents to the pubosacral region. Group 2 included 41patents given, besides basal pharmacotherapy, laser therapy in the same regimen as in group 1 supplemented after 2-3 hours by abdominal magnetic therapy. Group 3 (control) received traditional pharmacotherapy in the combination with antibacterial and anti-inflammatory medicines. RESULTS: It was demonstrated that the patients of group 2 exhibited the most pronounced positive dynamics of the clinical signs and symptoms estimated from the total National Institute of Health Chronic Prostatitis Symptom Index (NIH-CPSI) and of the characteristics of the quality of life evaluated based on the QLS scale. The patients of the two former groups experienced a more conspicuous decrease in the activity of the inflammatory process in seminal vesicles, restoration of their structural and anatomical features (as shown by the transrectal ultrasound study), improvement of testosterone metabolism, and normalization of the spermogram characteristics in comparison with controls. The analysis of the spermograms revealed the tendency toward the increase in the number of actively motile spermatozoa only in the patients of group 2. The combined treatment of the patients of the two former groups resulted in the decrease of the level of sex hormone-binding globulin and the increase of the free androgen index. CONCLUSION: The results of the study indicate that the application of the preformed physical factors for the treatment of the patients presenting with chronic bacterial vesiculitis enhances the effectiveness of pharmacotherapy and decreases both the frequency and the duration of relapses of the disease.

The present manual is intended for the graduate medical specialists who completed a training course in urology, and urology, and physiotherapy based at the state-funded educational institutions and have the relevant certificates. Also, the manual is designed for the specialists engaged in scientific research on urology, restorative medicine, medical rehabilitation, and physiotherapy. The manual expounds indications and contraindications for ultrasound therapy of the patients presenting with pathospermia. Such treatment of genital glands was shown to be effective in 96.8% of the patients with pathospermia. Moreover, its advantages over alternative therapeutic modalities were demonstrated. The criteria for its choice (for differential application and prognosis of effectiveness) include the excretory form of grade I, II, and III pathospermia, the secretory form of grade I, II, and III pathospermia, the combined form of pathospermia, the presence or absence of moderate anatomical and functional changes in the testicles, the presence or absence of partial obstruction of the vas deferens.

Ultrasound treatment in combination with laser irradiation of the blood and antibacterial therapy is known to exert strong anti-inflammatory, antibacterial, and analgesic effects, stimulate metabolism of sex hormones, and improve spermatogenesis in the patients presenting with chronic bacterial vesiculitis (CBV). This treatment is indicated to the patients with CBV at the stage of latent inflammation, to those recovering after acute vesiculities, and to the patients with chronic prostatovesiculitis at the stage of latent inflammation. This therapeutic modality is contraindicated in the cases of acute vesiculitis, acute prostatitis, acute bleeding hemorrhoids, rectal fissures and tumours, and general contraindications for physiotherapy. The above treatment resulted in the substantial or moderate improvement of clinical condition in 42 (82.4%) of the patients in group 1 and in 5 (50.0%) ones in group 2. During the 6 month follow-up period, remission of CBV was documented in 10 (83.3%) of the patients in group 1 and in 1 (33.3%) patient in group 2.

RESUMO

Patients with chronic bacterial prostatitis complicated by erectile dysfunction received combined rehabilitative treatment based on the use of various combinations of physiobalneotherapeutic factors, such as ultrasound, local negative pressure, white and yellow turpentine or sodium chloride baths, supplemented by basal medicamentous therapy in the form of rectal suppositories. Efficiency of therapy involving sodium chloride, white and yellow turpentine baths was estimated at 85, 60, and 75% respectively.

RESUMO

Combined treatment with local negative pressure and pulsating magnetic field conducted in 116 patients with erectile dysfunction aged 20-60 years produced optimal treatment results. Recovery and improvement of the erectile function were achieved in 85.7% patients given local vacuum-magnetotherapy.
