RESUMO

Postamputation pain syndrome significantly embarrasses prosthetics. In the article is described the transposition of the nerve trunks of the brachial plexus with terminal neuromas into the formed tunnel under the musculus pectoralis major, applied by the authors at 47 invalids (52 operations) with pain in the arm stump. The results of an operation have been studied in comparison with the results obtained after resection of the terminal neuromas. The data allow to recommend the translocations of the nerve trunks with the terminal neuromas in case of the pain syndrome after an arm amputation as more effective ones.

RESUMO

An analysis of data of 348 patients with malignant tumors of soft tissues has shown that 128 of them had been subjected to the amputation of extremities at different levels. The use of microsurgical techniques favours the improvement of the method of amputation of lower extremities, in particular the transplantation of a calcaneoplantar flap onto the stump end. In order to make the treatment of phantom limb pains more effective acupuncture should be included into the complex of therapeutic measures in addition to traditional methods. Close contacts with prostheses-makers can facilitate earlier prosthesis which can give positive effects on the following rehabilitation of such patients.

RESUMO

Under study were results of treatment of 150 patients with multiple and associated injuries of the upper extremities. Shock took place in 44% of the patients. Early fixation of the fractures of the upper extremities allowed a successful treatment of other injuries as well. The primary invalidization made up 12% mainly because of the amputation of the upper extremities.

RESUMO

Ten observations of replantations of the hand, forearm and shoulder after their traumatic abjunction are analyzed. Seven patients had complete reimplantation. Failures resulted from prolonged anoxia of the abjuncted segment or from extensive lesions to soft tissues of the replant. Success of the operation was mostly related with the quality of the vascular sutures (microtechnique), careful primary treatment of the wound and organization of the urgent surgical aid.

RESUMO

Histological and histochemical investigations of muscles in 18 patients have shown that when performing primary amputations of lower extremities according to the type of surgical treatment in the wound stump there necessarily remains muscular tissue of lower viability with necrotic areas.

RESUMO

Diaphyseal shin amputations with transposition of the plantar skin and calcanean tuber on the neurovascular bundle were performed upon 4 cases. The late results have been studied. The patients use prothesis with the support on the end of the stump, and walk without the walking-stick. The authors believe it possible to carry out such operations, under the conditions of a specialized medical institution, for severe injuries to the lower extremities complicated with shock after a thorough debridement of the wound.
