[Problems in the treatment of peritonitis].

The spreaded purulent peritonitis can not be separated from abdominal sepsis. There were enlighted the unsolved problems of peritonitis treatment, including the patient's state severity estimation, indications for the staged programmed sanation relaparotomy performance and the abdominal cavity drainage methods.

[The ratio of nosological forms and surgical tactics in the treatment of the thyroid gland diseases].

The dynamics of morphofunctional changes of the thyroid gland tissue was analyzed in patients with the organ diseases in 1995-2003. It was established, that geochemical environmental factors constitute the cause of the nodous goiter occurrence. The environment soiling causes the thyroid gland tissue dysplasia and the cancer occurrence. The surgical tactics in the thyroid gland diseases was adduced.

[Application of plasmapheresis in complex of preoperative preparation in patients with autoimmune thyroiditis and diffuse toxic goiter]. / Ispol'zovanie plazmafereza v komplekse predoperationnoi pokgotovki bol'nykh s autoimmunnym tireoiditom i diffuznym toksicheskim zobom.

The article is devoted to actulal problem of endocrinology and surgery-treatment of severe diseases of the thyroid gland. Results of plasmapheresis application in complex preoperative preparation in 90 patients, aged from 20 to 60 yrs, in severe course of the disease, were adduced. After plasmapheresis preparation conduction the reduction of the autoimmune processes in the patients organisms, the antibodies titer lowering were noted, witnessing the stable remission achievement. Plasmapheresis application in complex of treatment of patients with autoimmune thyroiditis and diffuse toxic goiter causes significant positive influence on hormonal and immune state.

[Apparatuses for the wound closure in modern surgical practice]. / Ustroistva dlia zakytiia ran v sovrennoi khirurgicheskoi praktike.

The method of the wounds treatment, using apparatuses for their edges rapprochement, was elaborated, basing on analysis of clinico-experimental investigations conducted. The method was successfully applied for the wounds closure after excision of coccygeal channel, the soft tissues abscess and phlegmon, postoperative ligature fistula and in temporary closure of laparotomy wound in patients with purulent peritonitis.

[Influence of surgical intervention on reactivity of organism of patients with perforative gastroduodenal ulcer]. / Vliianie khirurgicheskogo vmeshatel'stva na reaktivnosst' organizma bol'nykh s perforativnoi gastroduodenal'noi iazvoi.

Influence of operative intervention of various kinds on state of cellular and humoral immunity was studied in 49 patients with perforative gastroduodenal ulcer. The immunity indexes in patients constitute the prognostic test for estimation of the disease course severity and postoperative therapy efficacy. Significant lowering of postoperative complications frequency was promoted by application of preparations, stimulating the immune system (metrogyl, tactivin, gamma-globulins) in patients after the operation.

[Ultrastructural changes in gastroduodenal mucosal cells in patients with perforating ulcer and effect of serotonin adipinate on their postoperative rehabilitation]. / Ul'trastrukturnye izmeneniia kletok slizistoi obolochki zheludka i dvenadtsatiperstnoi kishki u bol'nykh s perforativnoi iazvoi i vliianie serotina adipinata na dinamiku ikh reabilitatsii v posleoperatsionnom periode.

Consideration is given in the paper to the state of the mucosal cell ultrastructure in the gastroduodenal zone in patients with perforating ulcer. Apparent inflammatory-degenerative type ultrastructural alterations have been found to be precent in the gastric and duodenal mucous coat in the zone of perforating ulcer. The above alterations are noted to persist by day 10 in these postsurgical patients having been administered conventional treatments. The use of serotonin adipinate promotes earlier restoration of the ultrastructural picture in the mucous coat of the gastroduodenal zone.

[Treatment of perforated gastroduodenal ulcers]. / Lechenie probodnykh gastroduodenal'nykh iazv.

Results of treatment of 751 patients with perforated gastroduodenal ulcers are analyzed. In addition to routine tests, ultrasonography, computed tomography and laparoscopy were used to diagnose difficult cases. The optimum surgical aids are chosen by taking into account the interval between the onset of perforation and admission, a history of peptic ulcerous, the pattern of peritonitis, age, comorbidity, and surgical findings. Organ-saving operations with a vagotomy are preferable. 1% serotonin-adipinate (1 ml dissolved in 200 ml saline solution) was intravenously used to prevent early postvagotomic disorders (gastric atony, dynamic ileus, etc.) in the early postoperative period. Organ-saving operations with vagotomy were found to have some advantage over routine suturing.
